Issuance f a Level 1 Certified Service Technician (CST-1) certificatin signifies that the hlder has met certain requirements as set frth by the Internatinal Assciatin f Trampline Parks (IATP). IATP des nt emply r supervise the certificate hlder. Prper perfrmance f the duties assigned t a certificate hlder is the respnsibility f the certificate hlder and the certificate hlders emplyer. 2. The fllwing schedule f examinatin and renewal fees shall be used; Examinatin and Certificatin Fee IATP Member.. $ Examinatin and Certificatin Fee- Nn-Member. $ Renewal Examinatin and Certificatin Fee IATP Member. $ Renewal Examinatin and Certificatin Fee Nn-Member $ Certificate Replacement Fee..$ Certificatin Card Changes/ Replacement Fee. $ Prir t taking the examinatin, applicants must preregister and pay the apprpriate fees t the IATP. All fees are nn-refundable. 4. Arrive during the scheduled check-in perid. D nt arrive late. Yu will nt be allwed int the rm if yu arrive after the test has begun. Yu must have permissin frm the test prctr t leave the rm during the examinatin. Yu will nt be allwed additinal time t make up fr time used. Cmmunicatin with ther applicants r persns utside f the examinatin rm is prhibited during the exam. 5. At check-in, yu must present ne frm f legal identificatin that has yur phtgraph n it. Wrk Identificatin and Schl Identificatin will NOT be accepted. Acceptable frms f identificatin include: Military Identificatin Passprt State Driver s License State Issues Pht ID 6. The examinatin shall be in writing and shall be prctred by a minimum f tw members f the IATP Staff, Bard f Directrs, Educatin Cmmittee, Certificatin Cmmittee r a minimum f tw representatives f a third party testing entity authrized by the IATP t prctr the examinatin. 2 7. The examinatin has tw cmpnents: An answer sheet and a testing bklet. Bth must be returned t the test prctr at the cmpletin f the exam r at the end f the exam perid. Any ntes, diagrams, calculatins must be made in yur test bklet. 8. The test prctr cannt answer questins cncerning the cntent f the examinatin. Listen carefully t the instructins given by the test prctr and read all directins in the examinatin bk thrughly. 9. The test prctr may dismiss an applicant frm the examinatin fr any f the fllwing reasns: If an applicant des nt have prper authrizatin t take the examinatin. If an applicant des nt have the applicatin paperwrk prperly filled ut and signed If an applicant gives r receives assistance, r is suspected f ding s. If an applicant attempts t take the examinatin fr anther applicant. If an applicant attempts t remve any test materials r ntes. If an applicant is abusive r creates a disturbance. If an applicant is fund in pssessin f a prhibited electrnic device. 10. All questins are multiple chice with three wrng answers and ne right answer. Reach each questin cmpletely befre answering and remember t mark the best answer fr each questin. 11. The examinatin shall cnsist f sixty (60) questins and have a ne (1) hur time limit. Yu may wish t bring a simple watch t help pace yurself (electrnic watches are nt permitted). The test prctr will keep the fficial time and ensure that yu are given the alltted time f ne hur fr the examinatin. 12. The examinatin shall be cnfined t questins based n the current versin f ASTM Internatinal standard F2970, all dcuments referenced within F2970, any current service bulletins r safety alerts pertaining t Trampline Curts and any written recmmendatins issued by the IATP. 13. Applicants may elect t reference a clean, paper versin f the current versin f ASTM Internatinal Standard F2970, dcuments referenced within F2970, any current service bulletins r safety alerts pertaining t Trampline Curts r any written recmmendatins issued by the IATP. A simple binder with tabs designating the dcument number r title shall be permitted. The text must be presented t the test prctr at the time f check-in. The texts f standards may nt be altered in any way except t highlight passages with a highlighter. N additinal ntes may be written n the text sheets that are t be used. Applicants will nt be allwed t share the texts f standards during the examinatin. The use f electrnic versins is prhibited. ASTM Standards, referenced dcuments, service bulletins, etc. will nt be prvided. 14. Applicants shall prvide tw (2) sharpened number tw pencils fr the examinatin. Yu will nt be permitted t use ballpint pens, felt tip markers r clred pencils. The pssessin r use f electrnic devices including but nt limited t cell phnes, calculatrs, electrnic watches, cmputers, cameras, pagers, recrders r tablets is prhibited during the examinatin. Other items may be excluded at the discretin f the test prctr. 15. 3 16. The passing scre reflects the amunt f knwledge that the IATP has determined t be apprpriate fr certificatin. Yur ability t pass the examinatin depends n the amunt f knwledge that yu display, nt cmpared t ther individuals take the test. A minimum scre f 75% is required t pass the examinatin. 17. Results f the examinatin shall be cnfidential and cnveyed t applicants within thirty (30) days f the examinatin. Ntificatin f results will indicate an applicants verall percentage scre and a PASS r FAIL. Results are available nly t members f the IATP Staff, Bard f Directrs, Educatin Cmmittee, Certificatin Cmmittee and the individual applicants. 18. Applicants wh fail the examinatin may be admitted t a future examinatin, prvided they cmplete all current registratin prcedures and pay applicable fees. Applicants may nt retake the examinatin within 180 days f the previusly attempted exam. Unsuccessful applicants must pay an additinal certificatin and examinatin fee. 19. The IATP is cmmitted t prtect the cnfidentiality f applicants recrds and have adpted plices t ensure their privacy. A general listing f IATP member Level 1 Certified Service Technician (CST-1) that have successfully cmpleted the requirements and are in gd standing may be made public. The list shall include the certificate hlders name, business infrmatin and certificatin status. IATP members in gd standing that want t be added t the list must ntify the IATP Bard f Directrs in writing with yur request t be included. Please allw thirty (30) days fr prcessing requests. 20. Applicants must be able t read, write and cmprehend the English language. 21. Applicants wh have disabilities as defined by the American Disabilities Act that prevent participatin in the standard examinatin prcess shuld infrm the IATP in writing when they register t take the examinatin. Applicants with special needs must request accmmdatins in writing t the IATP n less than thirty (30) days prir t the examinatin date. When requesting accmmdatins please prvide a brief written identificatin f the impairing cnditin and a request fr specific adjustments t the usual examinatin prcess. This infrmatin will be shared nly with the test prctr, but be kept cnfidential therwise. 22. Successful applicants will be issued a Level 1 Certified Service Technician (CST-1) certificate by the Chairman f the IATP. A Level 1 Certified Service Technician (CST-1) certificate is valid fr tw (2) years frm the date f testing. T cntinue certificatin yu must meet the requirements fr re-certificatin applicable at the time f re-certificatin. 23. Requirements fr certificatin as a Level 1 Certified Service Technician (CST-1). Applicants fr certificatin shall satisfy the requirements f this sectin. In rder t take the Level 1 Certified Service Technician (CST-1) Certificatin Examinatin, the applicant shall meet the fllwing qualificatins: A. Obtained a high schl diplma, GED, a degree in higher educatin r the equivalent. (A cpy f the degree shall be deemed sufficient t satisfy this requirement) B. Prvide evidence f a minimum f 8 hurs f IATP apprved training in the past tw (2) years. 4 C. The applicants emplyer, r qualified spnsr, shall prvide verificatin that all infrmatin prvided by applicant is true and crrect. D. Applicants must be able t read, write, and cmprehend the English language. E. All new and renewal applicants must pass a written examinatin with a minimum scre f 75% as referenced abve. F. Payment f the current examinatin and certificatin fee prir t the examinatin. 24. Requirements fr renewal f certificatin as a Level 1 Certified Service Technician (CST-1). Certificates may nly be renewed by: A. Submitting satisfactry evidence f a minimum f 16 hurs f IATP apprved training in the past 2 years fr renewal. -and- B. Successfully passing the current versin f the certificatin examinatin with a minimum scre f 75%. -and- C. Payment f the current renewal examinatin and certificatin fee prir t the examinatin. 25. The fllwing is a list f training prgrams and frums that are recgnized by IATP as apprved fr new applicants and certificate renewals: A. Amusement Industry Manufacturers Assciatin (AIMS) Safety Seminar B. Natinal Assciatin f Amusement Ride Safety Officials (NAARSO) Safety Seminar C. Nrthwest Shwmen s Safety Seminar D. Pennsylvania Amusement Ride Safety Seminar E. Participatin - F24.60 TG fr Trampline Curts (1 hur f credit = 10 hurs active TG participatin). The IATP Certificatin Cmmittee may add additinal training prgrams t the apprved list. 26. The IATP Certificatin Cmmittee reserves the right t revke r suspend an applicants certificate withut cause by a 2/3 vte f the Certificatin Cmmittee members present at a regular r specially called certificatin cmmittee meeting. We means the Internatinal Assciatin f Trampline Parks (IATP), and any and all ther persns and/r entities engaged by them as agents, cntractrs, instructrs, members and/r directrs, including any heirs, assciated entities, successrs, r estates fr the purpses f carrying ut any services under the Internatinal Assciatin f Trampline Parks Certificatin Prgram(s), r any ther related r unrelated prgram spnsred by the IATP. Yu have engaged us t prvide certificatin and/r training under the IATP spnsred prgram(s). Yu cllectively and separately agree t release and frever discharge us frm all knwn r unknwn claims r lawsuits f any kind fr any damages whether cmpensatry, punitive, r therwise arising ut f ur services whether negligent r therwise.
