The post holder will be required to deliver assessment and treatments within a range of facilities Acute, subacute and community settings. DUTIES & RESPONSIBILITIES PROFESSIONAL 1. To accept direct referrals from multi-disciplinary team members within the acute hospital settings in accordance with the referral criteria set out for the service. 2. To independently manage a caseload of patients with complex and highly complex needs evidencing a high level problem solving and reasoning skills approach. 3. To provide a specialist response by formulating and implementing treatment plans for patients within the area of stroke who require rehabilitation either in a hospital setting or their own home/community setting. 4. To utilise advanced interpersonal skills to encourage clients and their families/carers to participate in the treatment process, ensuring specialist client centred response and involvement at all stages of intervention. 5. To initiate, administer and interpret clinical observations and standardised assessment, which will establish a level of physical, sensory, perceptual and cognitive functioning and assist in clinical diagnosis of complex conditions.
4 6. To provide risk assessment in routine and complex moving and handling situations and implement the necessary actions. In doing so the therapist must develop a high level of understanding of manual handling legislation and a thorough knowledge of manual handling best practice and associated equipment. 7. To continually assess/reassess the complex needs of patients and provide them and their carers with the relevant information to enable them to make decisions about their future. 8. To fabricate and monitor static & serial splints for the remediation and management of neurological disorders. 9. To liase with other Health & Social Care Professionals and other related statutory, private and voluntary organisations, providing written and verbal reports and onward referrals where necessary. 10. To consult with Community Occupational Therapy, housing bodies and other agencies in making recommendation for individual clients regarding housing needs relating to client s therapeutic outcomes to ensure the appropriate provision of housing adaptations. 11. To provide support, help and advice to service users, their families and other carers teaching and demonstrating techniques to optimise the users functional ability, independence and/or management. 12. To have highly developed communication skills with clients who have altered perception, expressive and receptive dysphasia, pain, sensation loss and visual defects. General Management 1. To operationally lead and manage designated junior staff and students on practice education placements providing guidance, supervision and performance. 2. To contribute to the Trust s and Teams Clinical Governance and Quality Assurance Programmes setting and monitoring practice standards within the OT Service and in liaison with interdisciplinary colleagues. 3. To identify unmet needs and gaps in service provision through the initiation and implementation of audit and outcome measures and participation in other quality and service improvement initiatives. 4. To maintain accurate comprehensive and up-to-date documentation in line with legal and departmental requirements and communicate assessment and treatment results to the appropriate disciplines in the form of report and letters. 5. To provide statistical information on OT treatments, activities and outcomes on a weekly basis.
