(Example: cntent must be distributed in HTML frmat. MS Wrd and Adbe Acrbat can nly be used under certain circumstances.) Establish Deadlines / Timelines FIRST SUBMISSION OF COMPLETED COURSE WITH QUALITY ASSURANCE INFORMATION FOR INITIAL REVIEW Submit the Quality Assurance infrmatin t the Instructinal Develpment Specialist accrding t the established deadline. Instructinal Develpment Specialist will return the Quality Assurance dcument back t yu with cmments and needed revisins. SECOND SUBMISSION Of COMPLETED COURSE WITH QUALITY ASSURANCE INFORMATION, AND REVISIONS COMPLETED FROM FIRST SUBMISSION Resubmit the Quality Assurance infrmatin with curse revisins cmpleted t yur IDT member fr review. Yur IDT member will return the Quality Assurance dcument back t yu with cmments and needed revisins. THE QUALITY ASSURANCE FORM IS SENT TO DISTANCE EDUCATION DEPARTMENT The IDT member will send the Quality Assurance infrmatin t the directr f distance educatin fr review and apprval. The Directr f Distance Learning will review the curse and infrm yu if changes are needed. The Directr f Distance Educatin will als infrm yur supervisr that it can be added t the schedule. THE AUTHOR AND THE INSTITUTION AGREE THAT: 1. Title and Cpyright Assignment (a) Authr and Institutin intend this t be a cntract agreeing that services and results f the services t be rendered by Authr hereunder (the "Wrk") are within the Authr s scpe f emplyment with the Institutin, and cnsequently the Wrk is agreed t be a wrk made fr hire. Authr acknwledges and agrees that the Wrk (and all rights therein, including, withut limitatin, cpyright) belngs t and shall be the sle and exclusive prperty f Institutin. (b) If the Authr fails t deliver the Wrk n time, the Institutin will have the right t terminate this Agreement and will nt make any payment t the Authr if the agreed upn cmpensatin t paid as a stipend. Nte: The authr must cmplete the wrk by the established deadlines and evaluated fr quality assurance by the Instructinal Design Team befre it can be added t the curse schedule. The authr will be paid after the curse is apprved. If the agreed upn cmpensatin is in the frm f released time, the Authr will teach ne verlad sectin at n expense t the Institutin within the next tw academic semesters. 4. Quted Material With the exceptin f shrt excerpts frm thers' wrks, which cnstitute fair use, the Wrk will cntain n material frm ther cpyrighted wrks withut a written cnsent f the cpyright hlder. The authr will be respnsible fr btaining such cnsents. Only with prir written apprval f the President f Institutin will expenses incurred in btaining such cnsents be paid by Institutin. 5. 5 As this wrk is utside f the Authr s scre f emplyment with Institutin, but is being perfrmed at the request f the Institutin pursuant t the terms f this Agreement, in cnsideratin f and upn delivery and acceptance f the Wrk in accrdance with the prvisins f this Agreement, Institutin shall pay Authr $, which is the maximum amunt f cmpensatins that the Institutin shall we t the Authr pursuant t this Agreement. 6. Revisins and Use by Others The Authr shall retain the right t revise the Wrk [at ne year intervals] fr a minimum perid f [three (3)] years in accrdance with academic standards. The Authr further agrees t update the Wrk within ninety (90) days upn the receipt f a request frm the Institutin. The prvisins f this Agreement shall apply t each revisin f the Wrk by the Authr as thugh that revisin were the Wrk being published fr the first time under this Agreement. The authr will have the respnsibility fr maintaining the integrity f the master curse; including revisins t update materials and incrprate new technlgy as needed. As a prfessinal curtesy, the authr will assist ther faculty teaching the curse with an verview f hw the curse is taught. 7. Amendments The written prvisins cntained in this Agreement cnstitute the sle and entire Agreement made between the Authr and the Institutin cncerning this Wrk, and any amendments t this Agreement shall nt be valid unless made in writing and signed by bth parties. 8. Cnstructin, Binding Effect, and Assignment This Agreement shall be cnstrued and interpreted accrding t the laws f the State f Tennessee and shall be binding upn the parties heret, their heirs, successrs, assigns, and persnal representatives; and references t the Authr and t the Institutin shall include their heirs, successrs, assigns, and persnal representatives.
