1 JOB DESCRIPTION: PHARMACIST Background Central PHO and its associated organisations deliver and support primary health care services across the MidCentral region. Primary functions (purpose of position) Work collaboratively within health teams to enhance health outcomes via optimal and proactive clinical medicines management, with emphasis on management of chronic conditions and care of the older person Work with, and be supported by, a wider team encompassing a range of health professionals including other Clinical Pharmacists Work in partnership with the Director of Māori Health and the Māori Health team to identify objectives relevant to this role that will contribute to strengthening Māori Health outcomes Reports to Senior Clinical Advisor Functional relationships Patients General Practice Teams/Integrated Family Health Centre (IFHC) teams Community Pharmacy teams Central PHO clinical staff/ managers Healthcare Development Team/Clinical networks Specialist and acute care (MidCentral Health) teams Community organisations Other agencies government/non-government agencies e.g. PHARMAC, Medsafe, bpac. This may or may not involve seeing individual patients but requires a sound knowledge of complex medical conditions and interpretation and application of laboratory and diagnostic information. Utilise clinical audit to reduce the variation in use of pharmaceuticals and align more closely to best practice and population need. Provide timely responses to clinical questions. Demonstrate proactive and practical clinical application of population based evidence at an individual level. Support practices in accessing appropriate resources, tools and information. Practice, and support others to practice, the highest standard of evidence based medicine for patients with complex medical conditions via research and critical appraisal. Multi-Disciplinary Clinical Education General: Provide education that is tailored to meet the needs of the primary healthcare team. Guide best practice clinical management of patients by participation in multidisciplinary clinical discussion. Utilise such discussion to inform and provide evidence based medicine educational activities. 4 PERSON SPECIFICATION Essential A minimum of three years post registration experience. Excellent applied critical appraisal skills. Ability to apply consideration to the role of pharmaceutical interventions in achieving the NZ Triple Aim. Advanced written and verbal communication skills, at individual and group level. Demonstrated flexibility and ability to adapt to change. Good time management and organisational skills. Ability to work independently and as part of a team. An understanding of the application of population and individual health needs and the wider determinants of health to clinical management. Awareness of the need to prioritise high needs populations including Maori and Pacific People. Proven ability to apply principles of the Treaty of Waitangi. Patient centred approach. Current, full driver s license. Desirable Ability to apply constructive change management techniques. Knowledge of NZ Primary Health Care Services and System. Experience in leading and advising health professionals. Experience in implementing and maintaining quality processes and systems. Ability to review and develop policy and procedural documents. Experience in Primary healthcare Patient Management Systems, e.g., MedTech, VIP and Houston. Knowledge of the Whanau Ora Model Qualifications Qualified Pharmacist with current NZ Annual Practising Certificate (no restrictions). Postgraduate Diploma in Clinical Pharmacy (or working towards).
