1 JOB DESCRIPTION CLINICAL NURSE SPECIALIST- REFERRAL AND DAY HOSPICE COORINATOR Date: September 2015 Responsible to: Responsible for: Functional Relationships: Director Clinical Services Clinical Administrator Internal: SLMT Clinical Charge Nurse CNS Education Practice Coordinator All Clinical Teams HPCT Quality Clinical Administrator External: ARC District Nursing Service General Practice Teams Secondary Care Services Community Health Services Community Agencies Purpose This Clinical Nurse Specialist (CNS) role is an advanced nursing practice role responsible for providing specialist nursing assessment and coordination of care to patients entering our specialist palliative care service. Will provide clinical expertise and advice, demonstrating a sound understudying of the referral criteria for the specialist palliative care service provided by Arohanui Hospice Will provide a clinical visible profile; having input into the triaging of referred patients, and their the initial assessment requirements based on local, demographic, cultural and environmental factors To develop and manage a Day Hospice Service that will meet patient and family needs. Will supervise and support the Clinical Administrator in the management of referrals, clinic bookings, weekly VIP reports, and Palliative Care Partnership administration requirements

2 POSITION RESPONSIBILITIES Domain One: Professional Responsibilities This includes accountabilities for professional, legal, ethical and culturally safe practice. Includes being able to demonstrate judgement and accountability for won actions and decisions through the management of the Referral Process Facilitates the timely and accurate management of all referrals to the service from all referral sources and assesses for appropriateness - ensuring all relevant clinical information is available Establish and maintain positive relationships with all referral sources Liaises with referrers when the referral is not assessed as appropriate Represents referrals at the morning meetings, working within a team, enlisting the skills, support and advice of colleagues as appropriate Manages the allocation of the referral to the appropriate Community Team leader; including a recommendation regarding the place of the initial assessment and the roles involved (Medical, PCC, and/or Social Worker) Supports the Clinical Charge Nurse and Inpatient Unit Team Leader with the management of acute referrals/admissions to the IPU Collaborates with colleagues and external multi-disciplinary teams regarding patients who may not meet Hospice eligibility and identify appropriate referrals to other agencies Will liaise with referral sources to provide current information related to hospice referral and admission Facilitates the management of the referral & discharge processes; ensuring all steps are completed and data accuracy and quality is maintained. Domain 2: Management of Nursing Care Patient assessment and management of nursing care that is supported by nursing knowledge and evidence based research through the management of the Day Service Development and management of a Day Service which meets the needs of Arohanui Hospice patients and their families to ensure equitable access Undertakes or delegates the initial assessment of patients identified as appropriate for the Day Service Manages the ongoing need requirements of patients and families to point of transfer to the appropriate area team

3 Domain Three: Interpersonal Relationships This includes accountability for interpersonal and therapeutic communication with patients and members of the health care team Promotes effective teamwork and collaborative relationships with internal and external stakeholders to achieve best health care outcomes for our patients Provides expert clinical advice and education to nursing teams and other health care professionals within the community and hospice Attends identified organisational meetings, communicating all appropriate and relevant information Works in a collaborative and inclusive manner with the patient, family, whanau, friends, community, GP s, other primary care services and the MCDHB Domain Four: Inter-professional Healthcare and Quality Improvement Evaluates the effectiveness of care and promotes a nursing perspective within the interprofessional activities of the health care team Evaluates the effectiveness, efficiency and safety of the Day Service Demonstrates commitment to quality improvements, risk management and resource utilisation Participates in the development of strategic and operational plans as appropriate Engage in research, audit and quality initiatives, evaluation of service, encouraging participation and awareness with colleagues and the wider organisation Introduce research based findings into clinical practice Maintains own professional development and contributes to the professional development of others Management of the Principles of the Treaty of Waitangi within the philosophy of palliative care and Arohanui Hospice Demonstrates an understanding of the Treaty of Waitangi and its principles and encourages its recognition within the delivery of hospice services o Integrates the principles of the Treaty of Waitangi into all practices and management procedures where possible. o Is aware of cultural protocols and practices when working with Māori patients and their whanau and Māori health providers.

PERSON SPECIFICATIONS Qualifications Registered Nurse with the Nursing Council of New Zealand Current Practicing Certificate Proven and extensive clinical background in speciality Palliative Care A minimum 5 years post graduate experience Has achieved or working towards completion of a Masters Degree in nursing or other health related areas Continuing commitment to personal development ensuring that own skills and knowledge are maintained and advanced Must have a current unrestricted vehicle drivers licence Essential Competencies Level 4 of Professional Development Recognition Programme (PDRP) or working towards this Have extensive experience in a clinical specialist setting including sound nursing assessment skills Awareness of current trends in palliative and end of life care Demonstrated leadership skills Developed understanding of quality improvement Proven ability to problem solve, mediate and resolve issues Proven ability to make difficult decisions High standard of computer literacy with experience in patient management software and MS Office software Well-developed verbal and written communication skills Commitment to the effective management of the Principles of the Treaty of Waitangi and the need for cultural safety
