1 Position Title and Position Number Supervisor Community Services (P ) CEO Classification and Salary: Base Salary: $113,000 Total Salary: $142,734 Staff Direct Supervision: 8 Location (Note: Total salary includes leave loading, superannuation and allowance for motor vehicle) Indirect Supervision: 21 Malak Date Reviewed 15 th August 2013 SUMMARY OF POSITION: The Community Services will be directly responsible for programs aimed at improving the health status of the community by incorporating the principles of a population health approach which plans and executes programs, policies, and interventions along the entire spectrum of health action, including: Health promotion and early intervention Disease and injury prevention Risk management Policy coordination Medical treatment Rehabilitation Palliative care The Community Services is also responsible for Emotional and Social Wellbeing Programs and Services including but not limited to: Bringing Them Home (BTH) Counselling Service; Dare to Dream mental health program, and Youth program. The population health approach is a unifying force for the entire spectrum of health system interventions - from prevention and promotion to health protection, diagnosis, treatment and care - which integrates and balances action between them. OVERVIEW OF DANILA DILBA HEALTH SERVICE (DDHS) AND THE WORK ENVIRONMENT Background Established in 1991, Danila Dilba Health Service (DDHS) is a community-controlled organisation providing comprehensive primary health care services to Biluru communities in the Yilli Rreung Region of the Northern Territory.

2 The mission of DDHS is to improve the physical, mental, spiritual, cultural and social wellbeing of its clients through innovative comprehensive primary health care programs that are based on the principles of equity, access, empowerment, community self-determination and inter-sectoral collaboration. DDHS is funded primarily by OATSIH (Department of Health and Ageing) to deliver health care services to Aboriginal and Torres Strait Islander people and employs approximately 117 staff (FTE). Clinical services are delivered from three (3) facilities in Darwin: Knuckey Street, Darwin Primary health care Palmerston - Primary health care and Women s and Children s Health programs and Dental McLaughlin Street, Darwin - Men s Clinic Aboriginal Health Practitioners, General Practitioners, Registered Nurses and ancillary staff deliver holistic primary care health services within the following clinical streams: Acute Care and Chronic Disease management Male health Women s health Child and Maternal health Specialist/allied health services (including visiting physicians, pharmacy, and child development) A community services team consisting of Aboriginal health workers, family support workers and health practitioners provide a range of complementary services including: Community health engagement/ health promotion Alcohol, tobacco and other drugs program Youth services Emotional and social wellbeing services. Annually DDHS assists over 7,000 individual clients, providing over 42,000 episodes of health care. PRIMARY RESPONSIBILITIES: Strategic Management Contribute to the strategic planning for Community Services within DDHS Program Management Facilitate Population Health initiatives in the community that promote a healthy lifestyle; Identify and foster development opportunities in Primary Health Care (PHC) including Health Promotions; Identifying funding streams for further development of Community activities and programs; Ensure community and public input into the development of plans and projects; Provide linkages and access to PHC services through the delivery of appropriate community health activities in order to improve the health and well-being of families and individuals; Facilitate community consultation / input in order to identify and establish opportunities that will increase health access and allow introduction of preventive strategies to better manage their health; Manage and lead programs relevant to Danila Dilba Health Service s (DDHS) Community Services programs and initiatives; Develop Strategies and processes that allow and support community and family participation in Community Health activities; Develop, strengthen and implement appropriate Community programs and services in order to maintain, promote and improve the health status of the community; Conduct community health needs assessments with relevant groups; Partner with appropriate service providers and community resources to develop ways to meet identified health needs in the community; and, Conduct program and service evaluations with partner groups where required.

3 Administration Contribute to the efficient and effective functioning of the workplace by working collaboratively with team members in order to meet organisational objectives. This includes demonstrating appropriate and professional workplace behaviours, providing assistance to team members as required and undertaking other key responsibilities or activities as directed by the CEO or Executive Management Team. Take personal responsibility for meeting objectives and shows initiative in forward planning; Developing professional, productive working relationships with the range of internal and external stakeholders; Promote the role and importance of Aboriginal Community Controlled Health sector within government and the community. Advocate for and represent the interests of Danila Dilba Health Service locally and nationally where needed. Promote a collegial working environment towards achieving a collaboration and coordination of services and contribute to internal and external stakeholder forums. Manage the Community Services budget; Explore and create innovative approaches for maintaining and improving existing capability within the work unit environment; Initiate periodic review and evaluation of the work area s procedures and practices; and Promote, lead and direct accreditation processes, quality activities and other organisational activities as appropriate. Human Resource Management Manage, supervise, develop and mentor Community Services staff; In collaboration with other senior staff, manage; Recruitment, selection and orientation of new staff Leave schedules, time sheets and leave approval Performance management through effective and timely supervision Provide leadership to all staff in; Effective delegation Goal setting Training, development and support Ensure the requirements of the Equal Opportunity and Workplace health and safety Legislation are met within the working environment. SELECTION CRITERIA: Essential: 1. Extensive understanding of the role of community control processes within the context of Aboriginal Health; 2. Demonstrated skills and ability to effectively engage and encourage Aboriginal families and community groups to participate in health promotional programs and activities; 3. Understanding and knowledge of issues that impact Aboriginal and Torres Strait Islander families and communities on a day to day basis; 4. Demonstrated experience and ability to manage conflict and grievances within a community or family group; 5. Demonstrated organisational skills in planning, managing and prioritising tasks and resources to deliver outcomes within time and budget considerations; 6. Demonstrated ability and experience in working with and managing multidisciplinary teams; 7. Proven high level oral and written communication skills including liaison, negotiation and conflict resolution with people from a wide range of cultural and social backgrounds; 8. Demonstrated skills and ability in fostering leadership within your team and the community;

4 9. Sound analytical and problem solving skills; 10. The ability to develop processes that show resource deficiencies within the service as well as the ability to implement strategies that resolve these issues; 11. Preparedness to undertake a Police Check; 12. Ability to obtain a Working with Children Clearance; 13. Current Drivers Licence, and 14. Current First Aid Certificate or the preparedness to gain one.
