1 QUALIFICATIONS 1. Must possess a Masters in Social Work or Counseling, or in a related field. Must have ability, or must have ability to immediately obtain a license necessary for the billing of psycho-therapy services (LCSW). 2. Must possess active License at all times. 3. Education and in-depth experience in Hospice, and the acceptance of the Hospice concept care and dissemination of Hospice philosophy and policy. 4. Must have formal education, continuing education, and/or experience in Family Systems theory. 5. Must have a minimum of three years experience in the provision of community social services. 6. Must have two years experience in psycho-social assessment of patient/family. 7. Must have a combined work experience of a minimum five years of experience, including Case Management, and/or psycho-therapy, and/or program coordination and management. 8. Must have formal education, continuing education, and/or experience in Family Systems theory and counseling, and Bereavement Support services. Must operate within his/her profession s Code of Ethics and Licensing Board requirements, including continuing education. 9. Current understanding of, or willingness to learn, and commitment to the Systemic Strength Based practice of Social Work. 10. Must possess a sensitive and compassionate attitude towards accepting death as a part of life, and enhancing the quality of life for the patient assigned to his/her care. 11. Demonstrated ability to work with and serve as a mature representative of the Hospice organization. 12. Must operate within his/her profession s Code of Ethics & Licensing Board requirements, including continuing education. 13. Must understand and be comfortable working with organizational policy and daily operations, including the development of policy and procedure drafts. 14. Ability to work as a team member and the ability to function as an integral part of the Interdisciplinary Team. 1

2 15. Strong organizational, interpersonal, leadership, and team building skills. 16. Must have previously demonstrated skill in conflict resolution. 17. Ability to cooperate and form professional working relationships with personnel assigned to various departments. 18. Demonstrated work ethic founded upon personal/professional integrity and behavior. 19. Must have successful experience working with professional colleagues across professions and service delivery contexts. 20. Must have professional liability insurance coverage. 21. Must have current CPR certification. 22. Must possess, or in 30 days be able to possess, an Oregon driver s license, dependable transportation, and proof of automobile insurance. 23. Must be able to operate basic office equipment necessary to performance of duties, e.g. fax, copier, multi-phone line, and computer. Must have basic keyboarding skills. 24. Detail oriented, able to multi-task and remain flexible with assignments. 25. Must be able to recognize, support, and adhere to SCH & PCS mission, philosophy, policies, and procedures. 26. Must maintain professional integrity throughout all operations of Social Worker practice, able to adhere to the practice of confidentiality regarding patients, families, Staff, and the entire organization. 27. Must demonstrate a strong work ethic founded upon personal/professional integrity and behavior, able to uphold attendance and performance standards. 28. Present a kind and respectful attitude and approach in the daily workplace, while upholding SCH Policy. 29. Ability to work flexible hours, including evenings and/or weekends as needed or requested. 30. 3 4. Excellent communication skills, written, verbal, and listening, allowing clear and concise communication with Staff and patients. PHYSICIAL REQUIREMENTS 1. Must be able to lift a maximum of 25 lbs., using a 2-person assist with any awkwardly shaped or heavier weights. 2. Must be able to safely negotiate in a variety of environments, e.g. steep stairs, uneven surfaces, and cramped physical quarters. 3. Must be able to do extensive driving in a variety of road and weather conditions, and at night. WORK ENVIRONMENT/HOURS 1. Typical office setting. 2. Diverse surroundings in patient homes with unpredictable environments, such as uneven surfaces, and potential hazards such as animals, smoke, and exposure to communicable diseases, etc. 3. Nursing homes, assisted living facilities, etc. 4. This is a salaried position. PURPOSE The Manager of Social Work and Chaplaincy Services is responsible for the oversight and management of direct services to Hospice patients and family members, as well as Grief Services provided to grieving families, couples, adults, children, and youth. This includes maintenance of the Family Services Department and Community Bereavement and Education Center infrastructure, as well as service delivery outcomes. The Manager of Social Work and Chaplaincy Services has the responsibility for management and oversight of the program sub-components operating within this department. Provides Staff supervision to all employees in the Family Services Department, as well as direct participation and/or delegation involving the Interdisciplinary Team case review process, and Primary Care Team functioning. In addition, the Manager has responsibility for the Professional Development of Staff in the Family Services Department through implementation of the Systemic Strength Based Practice model, with documentation in Consolo (electronic medical record system). The Manager is responsible to ensure adherence to agency policy and procedure, Medicare standards, QAQI requirements, client-centered service delivery, Professional Code of Ethics, and procedural requirements of contracts such as insurance billing documentation. Finally, the provision of assistance with various assignments from the Executive Director, having to do with organizational, departmental, and program development is from time to time a part of this position. 3

4 To accomplish the above, interdepartmental collaboration, cooperation, and respect for department and program parameters are expected. FAMILY SERVICES DEPARTMENT RESPONSIBILITIES 1. Provides oversight and program management to all program sub-components within the Family Services Department, including Hospice Social Work, Social Work admissions, and Chaplaincy. 2. Provides direction, guidance, support, review, and evaluation of Staff, including Social Workers, Chaplains, and Bereavement services to Hospice bereaved. 3. Provides program development, implementation, monitoring, and modifications of subcomponents including Admissions, Trauma Incident Team, and Chaplaincy program. 4. Provides supervision to Family Services Department Coordinator, as well as to all Staff in the Family Services Department. 5. Provides guidance and direction, on both the team and individual levels for Professional Development. 6. Monitors and assists with policy development reflecting any/all department changes, in collaboration with the Quality Improvement Department, and other agency Department Managers. 7. Maintains and supports Department s kind and respectful approach to patient/family advocacy and care. 8. Available for Staff backup when needed. BEREAVEMENT CENTER RESPONSIBILITIES 1. Responsible for the clinical and non-clinical operations of CBEC services and programs. 2. Oversights the pathway for services for potential clients/group participants, involving information from Inquiry to Informed Consent for 1:1 therapy and Group enrollment. 3. Provides oversight & program management to the grief support and counseling services of the CBEC. 4. Oversights the management of CBEC Medical Records. 5. Reviews and provides supervision to Grief Therapists and Grief Group facilitators with their clinical assessment, treatment plan, ongoing therapy, group screening and group maintenance challenges. DIRECT BEREAVEMENT SERVICES 1. Provides direct grief therapy to clients involving clinical assessment, treatment plan, and ongoing therapy 2. Provides, delegates, and oversights services to Children and Youth. 3. Completes timely case documentation and continuity of records management. 4. Participates in the provision of services to members of a Trauma Support Group. Adheres to all SCH Personnel Policies including, but not limited to: Please initial SCH Standards of Conduct as described in the agency s Compliance Plan. 4

SUPERVISION EXERCISED Provides individual and group supervision to the Family Services Department Staff, which includes Hospice Social Workers and Chaplains, as well as Staff providing group based services such as Grief Psycho-educational Support Services.
