DEPARTMENT OF NURSING AND ASSISTED LIVING The staff in these departments will assist residents in the health center and buildings designated as Adult Care Homes or Assisted Living. All positions in these departments have common requirements. We are looking for those who have: The passion for the care of the older adult The ability to remain calm and reassuring to residents and/or others in an emergency or very busy time The willingness to smile and greet people in a positive way The flexibility when on the spot changes may be necessary for resident care purposes The willingness to seek help from others when needed to solve concerns in a positive manner The ability to admit an error and learn from it We staff three eight hour shifts each day: Day Shift 7:00AM to 3:30PM; Evening Shift 3PM to 11:30PM; and Night Shift 11:00PM to 7:30AM. All weekend and holiday shifts provide differential pay as well as all evening and night shifts. Applicants must have a minimum of 6 months experience in a similar health care setting for all relief and part time positions
2 DEPARTMENT OF NURSING DIRECTOR OF NURSING (LEAD NURSING ENGAGEMENT COACH) Do you want to transform long term care? Carol Woods Retirement Community is seeking an innovative and dedicated Lead Nursing Engagement Coach (Director of Nursing DON) to lead a culture of engagement and learning in our assisted living and skilled nursing areas. This position is not a typical DON! It s about relationships, not regulations. The role will focus on coaching nursing staff to deliver exceptional care through individualized, innovative and holistic approaches and as well as serving as a resource to staff, residents and families. In addition, this position plans, organizes, develops and directs the overall operation of nursing services, to assure excellent quality of care in accordance with our philosophy. Oversees 24 hour nursing care Supervision of Licensed Nursing staff. Effective management of resources to meet resident care needs and to maintain quality levels of resident care. Creates an environment conducive to and which facilitates staff development and the enrichment of resident life. Incorporates and teaches language and methods that supports a resident s personhood and nurtures a compassionate team approach. Serves as the Director of Nursing of record for the organization. REQUIREMENTS Education: BSN required, MSN preferred Current, unencumbered license as Registered Nurse in North Carolina, or qualified to obtain NC license (NC license must be granted prior to hiring). Current certification in CPR Adult Module (American Health Association or American Red Cross). Preferred: Experience in Geriatric Nursing Additional Requirements: Maintain technical skills needed to provide hands on care as a Registered Nurse, particularly for infrequently performed procedures to provide technical assistance to staff as needed. Experience: Minimum five years experience as Registered Nurse. Two four years of experience as supervisor in long term care or equivalent healthrelated field. 3 NURSING ENGAGEMENT COACH Leads daily nursing operations. Resource for residents, family and staff with questions or concerns regarding resident care that cannot be answered by the direct service staff. Collaborates closely with the Lead Nursing Engagement Coach (Director of Nursing) and Resident Life Coordinator to ensure that resident needs and preferences for clinical care and a meaningful daily routine are integrated and met and a culture of engagement is nurtured and upheld. Coordination/assistance to maintain quality documentation by the nursing staff. Collaboration with primary provider (or designee) and nursing staff to clarify orders and concerns about physician assessments and findings, diagnostic/lab values, etc. Performs nursing assessments including MDS and Admissions assessments Oversight of nursing plan of care and collaboration with other disciplines to ensure resident well being. Walking rounds and oversight of hands on care with a special emphasis on the most acute and end of life residents to ensure the highest quality of care Coaches and teaches skill sets and appropriate approaches in the moment. Three positions available (all full time): Monday Thursday, 7:30am 6pm; Friday through Sunday, 7am 7:30pm; Friday Sunday, 7pm 7:30am Occasional requirement to cover open shifts. REQUIREMENTS: Education: BSN required Current, unencumbered license as Registered Nurse in North Carolina, or qualified to obtain NC license (NC license must be granted prior to hiring). Current certification in CPR Adult Module (American Health Association or American Red Cross). Additional Requirements: Maintain technical skills related to medication administration, as well as all skills needed to provide hands on care as a Registered Nurse, particularly for infrequently performed procedures to provide technical assistance to staff as needed. Attend courses/classes as needed to enhance needed Knowledge, Skills and Abilities for the position. Experience: 2 3 years experience as RN 1 2 years supervisory experience Long term care and/or geriatrics experience preferred

6 WELL BEING DEPARTMENT CLINIC NURSE Responsibilities: Performs clinical nursing care. Respond to medical emergencies on campus (outside of skilled nursing area). Provide individualized resident education about health condition, treatment, etc. Communicate with staff, family, residents, hospital, pharmacy, etc. as required to provide quality, coordinated healthcare. Document as required. Qualifications: Graduation from an accredited school of nursing. Current unencumbered license as a Registered Nurse in North Carolina or able to obtain a North Carolina license (NC license must be granted prior to hiring). Current CPR Adult Module certification Minimum of two years of experience preferably with gerontology patients and ambulatory care services. Relief: 8:30am 5pm unscheduled basis in our Relief Pool as our needs meet your availability PHYSICAL THERAPIST PRN The PT is an integral part of the interdisciplinary healthcare team. This position: Develops, implements, evaluates and manages physical therapy services, policies and procedures and resident care plans Evaluates and treats residents in both outpatient and sub acute rehab settings Conducts functional assessments of residents for purposes of determining level of service needs or to recommend interventions to delay decline. Participates in interdisciplinary team meetings and other work teams, as assigned, which may deal with the functional abilities/challenges of residents. Participates and assists in departmental studies and projects as assigned. Evaluates and implements recommendations from the standing committees of Carol Woods as requested. Documents clinical interventions appropriately in electronic health record
7 Additional Requirements: Graduation from an APTA accredited physical therapy program, current unencumbered NC Physical Therapist license, CPR certification Adult Module,3 5 years of experience preferred (geriatric experience preferred), possesses ability to multitask, excellent written and verbal communication skills, detail orientation, and computer skills. Relief: Weekend rotation in sub acute setting (~every 6 8 wks.), unscheduled basis in our Relief Pool as our needs meet your availability SECURITY/COMMUNICATIONS/TRANSPORTATION SECURITY OFFICER Duties include patrolling campus, working communication desk, completing reports, monitoring parking and other related activities. Requires considerable walking, lifting and carrying of heavy objects, standing for long periods of time, and assisting residents. Hours Worked: Relief As Needed
