1 LPN/RN Burlington, NC Alamance Academy LLC Health CareNursing Employment Type Contractor Academy of Alamance Home Care Job Title: LPN/RN/- In-Home Aide Department: Nursing Reports to: Supervising Registered Nurse Job Description Details: Statement of Position/Position Summary This position will provide personal care service to the terminally ill patient and perform related tasks in accordance with the plan of care as assigned and delegated by the Supervising Registered Nurse, (SRN). Qualifications and Education requirements: High school diploma or GED. Successful completion of a state approved nursing assistant training program and a state approved trained medication aide program. Current, active certification on the North Carolina Nursing Assistant Registry. Previous experience working in hospice, home health or long term care preferred. Must be able to work independently and have strong written and verbal communication skills. Computer proficiency and a working knowledge of Microsoft programs required. Current CPR certification. Valid NC Drivers License and daily access to a reliable and insured vehicle. Belief in the in-home Aid concept. Position Role and Responsibilities

2 Provide personal care to the patient in a compassionate and understanding manner consistent with the hospice philosophy. Under the written and/or verbal direction of the SRN, the In-home aid performs the following duties: Provide bed bath, shower or tub bath as appropriate Provide appropriate care of skin, hair, mouth and feet (excluding nail care); nail care for hands will be provided only as directed by the SRN. Provide good body alignment, positioning, and range of motion exercises for non-ambulatory patients Assist patient in and out of bed and with ambulation (including gait belt, sliding board, Hoyer Lift, E-Z Stand) with family or facility staff assistance as indicated. Assist patient with use of bedpan, urinal, commode and bathroom Assist with routine catheter care and enemas according to the plan of care Assist with feeding and dressing patients Monitor and record temperature, pulse, respirations and blood pressure, as directed by the SRN Significant changes in the patient's condition will be reported to the SRN immediately Change simple dressings Apply Ace wraps as directed by Supervising Registered Nurse and/or Clinical Director. Remind and assist patient in taking pre-measured medications. May assist with medications that are self-administered. Perform various housekeeping duties related to patient care. Perform simple procedures as an extension of therapeutic services as delegated by the SRN and in accordance with the plan of care. Attend patient care conferences as requested by the SRN. Order and maintain supplies as assigned Responsible for maintenance of own entries in to paper and electronic medical records, assuring accuracy, completeness and compliance with regulations, certification standards, legal and ethical standards. Participate in ongoing filing and maintenance of the paper medical record as directed. Identify and address comfort care needs within scope of Home Care Aide role. Support and instruct the patient, family, and caregiver.

3 Maintain the dignity of the dying patient. Support the patients and family's unique spiritual and cultural beliefs. Provide holistic, family-centered care across treatment settings to improve the quality of life. Consult and collaborate with the interdisciplinary team and others involved in the patients care. Provide care consistent with North Carolina DSHR standards of practice for Home Care programs. Agency Expectations: Adhere to all Academy of Alamance Home Health LLC policies and procedures. Maintain absolute confidentiality of all information pertaining to patients, families, and staff and adhere to all HIPAA rules. Display a positive and professional image and attitude in all relationships with patients, families, peers and in the community. Serve as an ambassador for Academy of Alamance Home Health Care at all times. Provide verbal and written performance evaluations for assigned staff members, including peer and self-evaluation. Demonstrate ongoing responsibility for self-education. Assume responsibility for the promotion of the hospice concept in the community and with other agencies and professionals. Participate in the Quality Assessment Performance Improvement Program (QAPI), as requested. Maintains patient and personal safety regarding occupational health and safety. Participate in safety program, as requested: Participate in employee job safety analysis to eliminate hazards and prevent accidents. Cooperate to investigate employee injuries. Promote and support safety awareness and implement safety operations. Encourage staff to make suggestions on safety practices. Participate in orientation of new employees. Participate in volunteer orientations, training and education as requested.

4 Participate in orientation and supervision for students seeking experience in hospice care. Attend regularly scheduled department meeting and agency meetings. Perform other related duties as assigned.
