JOB DESCRIPTION Job Title: Pharmacy Technician Base: Pharmacy Band: 4 ( 19,217-22,458) (Band 4 to 5 development post) Reports to: Team Leader Accountable to: Head of Medicines Management JOB PURPOSE 1. To participate in the dispensing and checking of medicines in accordance with national and local policy and locally agreed performance standards. 2. To participate in the delivery and development of ward based medicines management services KEY RESULT AREAS Dispensary based duties To dispense in and outpatient prescriptions in accordance with Local Policy, Procedure and Performance Standards. To check the accuracy of medicines dispensed by other members of staff in accordance with local procedure To issue dispensed medicines to outpatients and counsel them on the correct and safe use of their medicines. To ensure that all dispensing transactions are entered accurately and in a timely fashion onto the pharmacy computer system so that stock discrepancies are minimised. To be involved in the training and assessment of all new staff within the pharmacy department in accordance with local procedures. To support the day to day provision of dispensary based services including dealing with enquiries, responding to urgent requests, etc
Ward based duties 1. To undertake the reconciliation of patients medicines at the point of admission in accordance with Trust policy. 1. To facilitate the use of patients own medicines in accordance with Trust policies 2. To be responsible for the efficient supply of items for one stop dispensing 3. To aid discharge planning in conjunction with clinical pharmacists and ward staff in including facilitating the preparation of electronic discharge summaries. 4. Identifying patients for discharge, checking the discharge prescription and dispensing/relabelling as necessary. 5. To train nursing and pharmacy staff in respect of the use of one stop dispensing and re-use of patients own medicines. 6. To undertake patient counselling (including those initiated on warfarin) and to work in conjunction with clinical pharmacists to ensure that patients understand their discharge medicines. 7. To use the electronic prescribing and medicines administration system (EPMA), ensuring that patient data is updated in a timely way, and that ward staff receive the necessary training. 8. To liaise with ward staff with regard to any relevant medicines management matters. 9. To liaise with the pharmacy dispensary, distribution service and technical services unit as required in respect of the supply of medicines for use at ward level 10. To carry out a three monthly checks of controlled drugs held in clinical areas. GENERAL RESPONSIBILITIES 1. Any other duties that may be requested by the Head of Medicines Management in consultation. 2. To participate in Weekend and Bank Holiday in accordance with departmental rotas 3. To undertake continuing professional development 4. To maintain confidentiality at all times relating to the patients, staff and the Trust. 5. To be familiar and comply with relevant Operational, Personnel, Health and Safety policies and procedures, including Fire, COSHH, No Smoking and Alcohol. 6. Maintain a safe environment by adhering to local infection control policies.
