POSITION DESCRIPTION: REGISTERED NURSE (TEAM LEADER) Salary as per Nurses and Midwives (Victorian Public Health Sector) (Single Interest Employers) Enterprise Agreement 2012-2016. Introduction to Tweddle Services: Tweddle Child and Family Health Service is located in Footscray and offers a range of specialist early parenting programs to families who are experiencing difficulties with parenting their children (up to school age). Programs include residential, day stay and sessional groups delivered at Tweddle or in the local communities, collaboratively with local government, community organisations or health services. Reports to: Residential Services Nurse Unit Manager Position Summary: Team Leaders are responsible for coordinating the delivery of parenting services in an efficient and effective manner and in accordance with statutory requirements. They provide support to Early Childhood Practitioners in the delivery of parenting services to families. In addition they ensure effective and efficient utilisation of resources in the provision and delivery of high quality parenting service to families and deliver early parenting programs that meet staff, client and stakeholders objectives. The position supports the smooth operation of Tweddle services through provision of high quality nursing services. Position responsibilities Clinical role Allocation of staff to families based on specialist staff skill and needs of families. Consultation with Early Childhood Practitioners and parents on the development of care plans and the provision and evaluation of interventions. Assessment of the needs of individuals with complex presentations and where appropriate providing care. Assessment of the physical and emotional well-being of the child/or children and parents. Providing education to staff, clients and groups. Ensuring that client rights and needs are being met. Maintain currency of all documentation in accordance with Tweddle policies and procedures. Community Liaison and discharge planning Liaise and refer families to appropriate community services / agencies in consultation with families. Communication Foster / facilitate good interpersonal relationships and partnerships with families, relatives, visitors, and other health professionals. Ensure that open communication occurs about the needs of families within the team and where appropriate across the organisation. Communicate with the Residential Services Nurse Unit Manager and PASDS Care Coordinator on issues that can impact on families or the organisation. Provide feedback on policies, procedures and current work practices ii. identify areas for improvement iii. contribute to the development of creative, innovative and practical initiatives Participate in Tweddle quality program and initiatives. Professional support direction and development of the Early Childhood Practitioners Provide leadership to the Early Childhood Practitioners. Support all team members to function as a team. Review the allocation of tasks on the shift e.g. groups. Management of resources Monitor the need for resources to assist in the achievement of care plans. Where appropriate develop or assist other appropriate staff to develop the resources. Participate with the Residential Services Nurse Unit Manger in the development of the staff roster. After hours co-ordination of rosters Facilitate professional development Maintain a professional knowledge and skills base related to child and family health and be responsible for own professional development. Demonstrate a commitment to continuing education and professional development including: i. Regular attendance at staff meetings and Team Leader meetings ii. Attendance at In-Service programs iii. Keeping up-to-date with knowledge and practice Achieve competency in Tweddle s emergency procedures Participation in the annual performance appraisal with the Residential Services Nurse Unit Manager. Corporate Confidentiality Staff are required to maintain Corporate Confidentiality at all times during and after their employment at Tweddle. Key Selection Criteria Division 1 Registered Nurse possessing current and appropriate endorsements to practice as a registered nurse, midwife, maternal and child health nurse or Mental Health Nurse. Recent experience in the provision of paediatrics, midwifery and or maternal and child health nursing or adult mental health. Knowledge of infant mental health and attachment and Best Interest Practice Framework. Working in partnership with families Experience in working with families experiencing relationship difficulties A strong clinical knowledge and knowledge in the health and family services sector Experience in staff supervision Skills experience and qualifications Tertiary qualifications in related nursing disciplines of paediatrics, midwifery and or maternal and child health nursing. Sound knowledge and experience and ability to provide clear concise objective and legally defensible documentation as required. Basic level of computer literacy and knowledge of word processing.
