OVERNIGHT AND OUT OF STATE FIELD TRIPS Teachers wishing to apply to the FSD1 School Board for approval of overnight out of state field trips need to provide the following information for review by the school board: 1. Name of individual requesting approval. Please include number of years of teaching experience and number of years teaching experience in the district. Larry M. Jackson, music/band teacher at Wallace Gregg Elementary School/Moore Intermediate is requesting that the Moore Band travel to Florida to possibly perform at an area theme park. This is my 11 th year of teaching-8 years in Florence School District 1 and 3 years in Florence School District 5. 2. Provide a list of past trips (out of state) sponsored/chaperoned. I have sponsored many 4 th grade trips to the Francis Marion Performance Arts Center and Civic Center for the 4 th grade orchestra trip. I have also sponsored a band trip for Moore to go to Atlanta, GA may 2015. 2005- Carowinds, North Carolina 2006 Atlanta, GA 2007 Orlando, GA 3. Destination and dates of the field trip. Date of field trip is May 19, 2016 May 22, 2016. We will be traveling to Orlando, Florida. 4. Provide the purpose of the field trip. The purpose of this trip is two-fold. It serves as a behavior and reward incentive for those in band, to work and strive hard, while contributing to the welfare of Moore Intermediate School. Students may also get the chance to participate in a performance in the parks at Disney World pending audition tape. 5. Please include state standards that will be taught in reference to the trip. How are students going to be assessed on these standards? An example would be: journals, videos, pictures, group presentations, diaries, etc. Analyzing. Students will A. Identify musical concepts as well as perceptual skills by moving, use appropriate terminology B. Identify musical instruments of the orchestra and band Making Connections. Students will explain the role of music in life experiences, celebrations, community functions and special events. C. Introduce students to performance opportunities as well as critiquing professional musical performances Students will be assessed by video journal entries. 6. Transportation: How will the students be transported? We will take one motorcoach (Carolina Tour and Travel, Kingstree, SC) to transport to and fro Florida. We have used CT&T for most band or class trips within FSD1 and FSD5. 7. Number of students participating in the activity. I anticipate that there will be between 25-30 students who will be eligible to experience this trip. We plan to have 1 chaperone per 3 students. This will give us a combined total of up to 40 students and chaperones.
8. Accommodations: Where will the students be staying? How many students in each room? Are the rooms all located with interior doorways? Are students and chaperones located on the same hallways with chaperones located in strategic locations? Students will stay at Holiday Inn Maingate East, in Orlando, Florida with 3 students and one parental chaperone in each room. Parents must notified and sign off on their child rooming with an adult chaperone who has pass SLED verification. All rooms are located within the interior hallways. All student and chaperone rooms will be on the same floor. This hotel is designated as a student hotel or hotel that caters to student school groups. 9. Cost of the trip per student. List any fundraising activities to offset costs of the trip. Students will be participating on the World s Finest Chocolate, Krispy Kreme and Orange Leaf Fundraiser. This will help provide the cost of the motorcoach ($3,850.00). Two fundraisers (one for advanced and beginner band will be done in December. For the 2016 year, we will do the World s Finest Chocolate as the final fundraiser. 11. Chaperones (Number and Names): If chaperones are on staff with FSD1, please include their position, number of years teaching experience and number of years in FSD1. If chaperones are parents, each parent has to have gone through the background check process. We plan to have between 10-12 chaperones to help with this trip. I have at least 3 teachers that are interested in being chaperones: one is a former administrator with more than 15 years experience; one teacher who has had 4 years teaching experience in FSD1 and 5 in Hartsville; and that last teacher with at least 11 years experience. 12. Written itinerary. Provide a detailed description of what sites you will be visiting. This should be done for each day of the trip. Itinerary is provided as attachment. Things to remember: Parental permission slips: Have each student complete a permission form. See attached form.
Check with the building principal for standardized testing dates and other potential conflicts. Remember to have a copy of the insurance coverage for the bus company you are using. (When planning the trip keep in mind that drivers can only drive for ten (10) hours per day.) Include a rooming list for each chaperone and if the hotel did not generate a rooming list, provide the hotel with said list. Remember to include the cost of any substitutes needed to cover classrooms. Medical Information: Have the parents complete a form detailing the medical concerns for their child. The school nurse will have the forms that will need to be filled out by the doctor for any medications the child will need to take on the trip. Provide the school nurse with a list of students who are going on the trip and he/she will provide any information you will need for students with particular issues. The school will send a first aid kit for the trip.

Your 3-Day PARK HOPPER Ticket allows the group unlimited admission to the Magic Kingdom Park, Epcot, Disney s Hollywood Studios and Disney s Animal Kingdom Theme Park. A Disney Meal Coupon is included in the tour package for lunch (OR) dinner. Group will stop on their own and have breakfast en route to Orlando. 9:00 PM Epcot s "Illuminations" - A rhapsody of lasers, lights, fountains, music and fireworks encompasses World Showcase Lagoon. Usually performed at 9:00 PM unless the park has extended hours. We strongly suggest that the group views Illuminations in front of the lagoon between Mexico and Canada. The group will then be towards the front of the park to exit to the buses at the conclusion of Illuminations before a majority of the people leave the park. Otherwise, by viewing Illuminations back by the World Showcase area, it will take the group at least an extra ½ hour to leave the park. Get a seat at least 45 minutes before the show. 9:45 PM Depart to the Hotel Your Summit Tour & Travel representative will greet you at the buses to transfer the group to the hotel for a pre-registration orientation. For the tour package prices to be considered firm, you must return your payments in accordance with the dates listed on your contract from Summit Tour & Travel. Transportation is not included in the tour package rates. Package rates are based on a 40 paid persons. Groups with 30-39 paid persons will be assessed an additional fee of $400.00. Groups with less than 30 paid persons will receive a total new tour package price quotation. Tour package rates are valid for proposed dates. Hotel requests must be made at the time of contract and will be honored depending upon availability. If the deposit and signed contract are not received in our office by the due date requested in the contract, there cannot be any guarantee that the requested hotel will still be available. An alternate choice may be necessary at that time and the group will be charged accordingly. All attempts will be made to find alternate hotel accommodations at a rate equivalent to the group s first choice. Holidays, peak travel times and requests for indoor hallways may increase the package cost. All Non-Performers will receive the same tour package inclusions for the same package price provided they purchase the entire tour package. All attraction tickets purchased will have a printed expiration date. Unused days on the tickets will not be valid after the expiration date. No refunds will be given for unused tickets. A minimum number of persons may be required for some meal functions. If the function is available, it may become necessary to combine your group with another group to ensure participation in that particular function. Complimentary tour packages do not include transportation and are based on double occupancy. If double occupancy rooms are not assigned on the rooming list, the complimentary packages will default to triples and then to quads. The price difference will not be refunded to the group in the event that double rooms are unavailable. Single occupancy complimentary tour packages are available; however, it will require an additional charge. Assignment of the complimentary tour packages is left up to the discretion of the director. Summit Tour & Travel will not make these assignments. Summit Tour & Travel Trip Application & Health Form must be returned to our office with the final payment.
Transportation Transportation is not included in any of the package costs listed in your tour proposal.
