The prescribed application format may be collected from our offices at above addresses or can be downloaded from our official web site www.pnbindia.in. Last date for submission of duly filled prescribed format with all necessary credentials/particulars in a sealed cover is on or before 03/05/2016 up to 5.00 P.M. Instructions to the intending agencies/firms who apply for empanelment: 1. a) Punjab National Bank desires to prepare a panel of contractors for the jobs to be undertaken by Bank for its Branches/offices. Category, subcategory(amount wise) for which panels to be prepared & basis prequalification criteria thereof are given hereunder:- Sl Category Estimated cost of work Average financial turnover for last 3 years (Rs. in las) 1.1 Furnishing/Interior decoration/renovation. 1.2 Electrical Work / Lan cabling. a. Rs.1.00 lacs to Rs.15.00 lacs b. More than Rs.15.00 lacs up to Rs.30.00 lacs a. Rs.1.00 lacs to Rs.5.00 lacs. b. More than Rs.5.00 lacs up to Rs.10.00 lacs 1.3 Civil & Structural work a. Up to Rs.5.00 lacs 1.4 Sanitary and plumbing work b. More than Rs.5.00 lacs to Rs.25.00 lacs. a. Rs.1.00 lac to Rs.5.00 lacs. General Instruction to the applicants: a) Contractors for Interior and Electrical Works:- 1. Firms should have experience in the same line and having successfully executed at least three works of similar nature in the last three years for any reputed Govt./PSU. 2. Electrical Contractors shall possess valid Electrical License from State Electricity Authorities. 3. They should be empanneled / registered with any Government/ Public Sector Banks /PSU and hold a valid registration. 4. Separate applications shall be submitted for empanelment of contractors towards electrical and interior furnishing works. 5. Applications shall be submitted in prescribed format only.
6. Credentials of the applicants shall be compulsorily supported by related documentary evidence-registration of the firm, empanelment with other organizations, details of clients along with copies of work orders, work completion reports, certificates from clients about the quality of their services, details of man power, financial capabilities etc. 7. Existing panel members also shall apply afresh if they desire to continue or else they will be automatically deleted from the panel once new panels are finalized. 8. Late and incomplete applications shall not be entertained. 9. The following aspects will be considered prima facie while forming the panels: a) Experience in the related field. b) Bank s / PSU/Any GOVT. Organizations in which the firms are already empanelled c) Location of the firm d) Our past experience with the applicants( if any ) e) Financial soundness and annual turnover 10. Bank reserves the right to reject any of the applications or all application without assigning any reason at any time. 11. Each application should accompany with a demand draft for Rs. payable at Bhubaneswar. 12. Applications without demand draft will be disqualified. 13. Envelope containing the application form shall be super scribed as application for empanelment of Contractor (please specify relevant field & category of empanelment) 14. Applicants may apply with different category with each different applications with each a different DD.

If so, mention number and date. Also furnish copies of sales tax clearance certificate and copy of TIN number. 9. Whether an assessee of Income Tax. If so, mention Permanent Account Number. Furnish copies of Income Tax certificate. Please submit ITR returns filed for last 3 financial years ended March, 2015.
10. Furnish copies of audited Balance sheet and Profit & Loss (audited) for the last three year. 11. Service Tax Registration No. (enclose self certified copy of service tax) 12. If you are registered in the panel of other organizations /statutory bodies such as CPWD, PWD, MES, Banks, Govt. Undertakings/Insurance co. etc. Furnish their names, category and date of registration with past experience in last 5 years: i) ii) iv) iv) vi) 13. What are your fields of activity? Note: Where copies are required to be furnished these are to be certified copies preferably by the concerned agencies or a Government Officer. 17. Average annual financial turnover during the last 3 years ending 31 st March of the previous financial year. 18. Have you been ever disqualified/delisted or levied penalty by the bank/any other organization/institutions in past for non fulfillment of the contractual obligations. If yes, please provide details. 19. Whether any civil suit/litigation arisen in contracts executed/being executed during the last 10 years. If yes, please furnish the name of the project, employer, nature of work, contract value, work order and brief details of litigation. 20. Information relating to whether litigation is pending before any Arbitrator for adjudication of any litigation or else any litigation was disposed off during the last 10 years by an Arbitrator.
