General Information The Rapid Online system allows you to obtain death benefit information and report death claims for policies assigned to an SCI Funeral Home or with an SCI location indicated as the preferred funeral home. Reporting a claim using Rapid Online will stop billing on the policy, if applicable. Policy status and death benefit values are refreshed each business day, during which time the site may be unavailable for a brief period of time. Getting Started To navigate to Rapid Online, you may enter https://www.assurantpreneed.com/rapidonline/ in your browser address line. We recommend that you then establish a bookmark or favorite for future use. Note: You can access the User Manual online through the link at the upper right corner of any screen. Policy Search Once you have logged-in, the Policy Search screen will appear. Each screen allows the user to Logout of the system by clicking the button below the picture. You may search for a policy using the American Memorial Policy Number, Mayflower Policy Number, Assurant Life of Canada Policy Number, Goods & Service Contract Number, or Last Name. When searching with an Assurant Life of Canada (Administered by American Memorial) policy number you will need to preface the policy number with zero and a dash, example: 0-4XXXXXX. Clicking the Search button will prompt search results. 1-2012 Page 4 of 15
Policy Search Results The selected policy information will appear in the Policy Search Results screen. Please use the information shown to confirm you have the correct policy and then click the associated Select. Note: Data displayed in example is fictitious. Actual data has been removed to protect the privacy of our policyholders Searching by Last Name or Contract Number may result in a list of policies. Messages may be displayed in the Status column if the policy is inactive or a claim has previously been reported. Only policies that have not been paid will allow the Select option. 1-2012 Page 5 of 15

To obtain a quote and/or report a claim and stop billing: 1. Verify the information on the Policy Search Results page to confirm correct insured 2. Validate the date of birth by selecting yes if it is correct 3. Choose the Cause of Death 4. Choose the State/Province of Legal Residence 5. Answer whether death occurred within State/Province of Legal Residence 6. Click on the calendar to choose a Date of Death 7. Validate the servicing location number 8. Click Proceed to Request Quote. If the cause of death is Suicide and the policy is within the first two policy years, a pop-up box will appear to alert you to that condition. When you click Ok you will be returned to the Policy Search screen and no quote will be given. For various other conditions, a pop-up box may appear to alert you to other information, but when you click Ok you will proceed to the Death Benefit Quote screen and a quote will be provided. It is recommended you call our office to confirm the benefit in any of these cases.? Note: You have not yet reported the claim and stopped billing. At this point you have just moved on to the quote screen which has the heading of Death Benefit Quote. 1-2012 Page 7 of 15
Death Benefit Quote The Death Benefit Quote screen will provide a noncontestable or contestable death benefit quote for underwritten US policies, and only the noncontestable death benefit quote for underwritten Canadian claims. At this point, the claim has not been reported.? Note: A claim is contestable if the date of death falls within the two-year period from the issue date on certain plans. In order to determine the amount of the benefit, the cause of death on the death certificate and/or medical records must be reviewed. To report the claim, stop billing and/or retrieve a partially completed Claim Form and/or Medical Release Form if applicable, click on Report Claim.? Note: Clicking Report Claim to proceed with this request will prompt an update to the associated policy which will suspend policy activity. If the request has been made in error, please contact Customer Service to have the policy s status reset.? Note: On Premium Paying Policies there will be a message displayed showing the date the policy is paid to. 1-2012 Page 9 of 15
Claim Reported When you select Report Claim you may be prompted to enter the At-Need contract number, the At-Need cost and Choose from one of the three payment options. This information will be used to determine payment of benefits. If no additional documentation is required: The final screen prompts the user to enter their name certifying that they performed the funeral services for the insured. Type in your name to certify performance of funeral services and the claim is complete using the digital signature, no further action is required. 1-2012 Page 10 of 15
After you enter Confirm and Submit a new selection of names may appear based on the original search criteria. This list suggests other policies that may also belong to the insured. Please validate and select any that is the same insured so a claim will be processed on each policy. If additional documentation is required: 1-2012 Page 11 of 15
Display Claim Form and/or Display Medical Release button will prompt display of a partially completed, printable form if needed which will need to be faxed in for payment. Note: Any further claim documentation, such as a death certificate, that needs to be faxed please write the policy number on it. 1-2012 Page 12 of 15
US Contestable Claims On certain plans if death occurs within the two-year period past the issue date, the claim is considered contestable. A Death Certificate and Medical Release Form are required. The Death Benefit Quote screen will provide a Contestable Death Benefit which is the limited benefit available if the claim is rescinded. A message will also appear in the Comments column explaining that a Medical Release form and Death Certificate are required. You may also view the full benefit that may be paid on the policy, should the review of records result in the claim not being rescinded. To view the full benefit quote, click on View. A window will appear with the full benefit quote and a posted Disclaimer. You may obtain a partially completed Medical Release Form at this point by clicking on authorization for release of health-related information. 1-2012 Page 13 of 15
To exit the screen, click on <<Close. You may now proceed to report the claim, stop billing if applicable and print a partially completed Claim Form and/or Medical Release Form by clicking on Report Claim. Please ensure signatures are completed before submitting. The completed Claim Form, Medical Release Form and photocopy of the Death Certificate should be forwarded to our office. 1-2012 Page 14 of 15
Flex and Annuity Policies For Flex or Annuity policies that are still in the premium paying period it will list the date of which the contract is paid to below the quote.
