The salary and allowances will be paid in accordance with the Victorian Public HeathSector (Health Professionals, Health and Allied Services, Managers and Administrative Officers) Enterprise Agreement 2011-2015. As a permanent staff member, the opportunity to package up to $9,095 of your salary is offered. In addition Otway Health will pay superannuation as per the Superannuation Act. How to apply: 1. POSITION CONTEXT Otway Health and Community Services is located in the township of Apollo Bay on the Great Ocean Road within two hours travel from the major centre of Geelong. Otway Health s focus is Community First and our commitment is to respond promptly and effectively to evidence of need in our catchment area. Our priority is to work with our client base and community to strengthen the community and ensure that people are well supported and able to improve and enhance their well-being. 2. POSITION OBJECTIVES The Community Care Coordinator will develop, implement and evaluate the home and community care program, ensuring that it is responsive to the needs of clients and our community. The Community Care coordinator will also supervise all home and community care workers. The Home and Community Care Coordinator will be supported by the home and community care health professionals. STAFF RESPONSIBILITIES All staff are responsible for compliance with organisational policies and procedures and for complying with all relevant legislation, specifically legislation applying to Equal Employment Opportunity, Privacy and Occupational Health and Safety. All staff are expected to contribute to and participate in Otway Health s processes relating to Cultural Diversity, Quality Improvement, Risk Management and Consumer Participation.
3. SPECIFIC ACCOUNTABILITIES: Provides leadership and motivation to staff towards the achievement of service goals and objectives as per home and community care team work plan. Facilitates regular staff meetings and regular professional development of home and community care staff. Completes annual performance appraisals of all community care staff. Assists in the recruitment, selection and induction of home and community care staff in accordance with Otway Health s policies and procedures. Ensures community care staff have current and valid position descriptions. Ensures all community care staff timesheets are accurate and sighted by the Manager of Community Services. Ensures service delivery output is maintained and measures are taken to grow home and community care service. Ensures all internal and external reporting is complete for home and community care within required timeframe. Contributes to the development of an annual budget that reflects the service targets based on identified need. Ensure home and community care service meets HACC National Standards, QICSA Standards and Active Service Model. Co-ordinates roster for the Home and Community Care Workers. 4. KEY PERFORMANCE INDICATORS Develop, implement and evaluate Home and Community Care Team Workplan within agreed timeframes each financial year. Coordinate, develop and evaluate at least two training days for home and community care staff per year. Demonstrated evidence for active participation in home and community care regional network. Demonstrated evidence of continuous quality improvement to abide by accreditation standards. 5. ORGANISATIONAL RELATIONSHIP (see Organisation Chart) Reports to: Manager of Community Services Supervises: Home and Community Care Workers Planned Activity Group Officer Planned Activity Group Assistant Supported By: Home and Community Care Health Professional Team External Liaisons: Represents Otway Health to the community as required. Establishes and maintains relevant networks and links with appropriate agencies.
6. SELECTION CRITERIA Selection is based on ability to demonstrate that skills, knowledge, qualifications, experience and ability criteria (listed below) can be met and the ability to undertake the key responsibilities of the position is evident. Qualifications A tertiary qualification in an appropriate discipline with relevance to this position is preferred.
