2 BEST PRACTICES FOR Opiate Maintenance Treatment Programs (OMTP) Avoid Abuse, Limit Liability Who Should Attend? This special intensive course is designed for OMTP or Buprenorphine certified physicians or Program Managers who seek to enhance their ability to effectively, efficiently and safely provide Opiate Maintenance Treatment (OMT), while minimizing abuse and limiting liability. Statement of Need Addiction treatment providers who prescribe controlled drugs in the treatment of addictive disease, whether a licensed OMT provider or a certified buprenorphine prescriber, are faced with the unique clinical challenge of providing daily doses of addictive medications to patients with addictive disease. 1. There are challenging clinical issues involved: stabilization v. maintenance doses, treating opiate addicted patients who have concomitant chronic intractable pain, treating opiate addicted patients with co-morbid depressive or anxiety disorders or sleep disturbances, and management of opiate medication side-effects are a few examples. 2. There are unique ethical issues involved: patient safety v. minimization of disruption to patient daily life and the obligation to the individual patient v. the obligation to the community well being. 3. There are unique Patient-Doctor and Patient-Treatment Program issues: staff splitting by patients, appropriately supervising toxicology testing while maintaining dignity, confidentiality concerns yet need to have data from other providers regarding patient stability, and issues about dose adjustment and take-home privileges. Participant Learning Objectives After attending this course, participants will be able to: Describe the pharmacologic profiles of benzodiazepines, opiates and other controlled drugs, and drug-drug interactions Identify the diagnostic criteria for anxiety disorder, chronic pain, insomnia, depression, and management strategies within an OMTP Employ techniques relating to minimizing the diversion of controlled substances with regards to OMTP Discuss case management strategies for the above diagnoses in opiate maintenance patients Discuss risk management and best practice guidelines for OMTP quality improvement initiatives Educational Design and Methodology The course uses lectures, participative discussions, case studies, a personal case-based reflective essay, and simulation to create an intensive learning environment for a small group of physicians. Each registrant will complete a pre-test prior to the course and a posttest at the end of the course. Participants will only receive credit for the sessions attended.
