1 POSITION DESCRIPTION POSITION: Registered Nurse - Staff REPORTS TO: Director of Nursing Status: Non-exempt PURPOSE OF POSITION: This position to ensure that quality care is delivered through assessment and monitoring of staff performance, physician contact, care planning and daily resident assessment. This position also provides direct care to residents. All this is done according to nursing standards of practice, standard procedures, and established State and Federal regulations for skilled nursing facilities. NATURE AND SCOPE: This position reports to the Director of Nursing and is staff in nature. The position provides routine day-to-day direction and monitoring of the LPNs and nursing assistants assigned to the unit and provides direct care to residents. In performance of duties, the registered nurse is entrusted with the following responsibilities: 1. Provide routine daily direction and monitor the work of the LPNs and nursing assistants on the unit, assuring a high standard of nursing care and satisfactory completion of tasks. 2. Assist in the orientation of new nursing personnel and reinforce training standards to maintain a high quality of care. 3. Assist in motivating LPNs and CNAs assigned to the nursing unit. 4. Provide explanation of nursing procedures and assistance to staff to assure ongoing understanding and training. 5. Communicate on a regular basis with the Director of Nursing regarding assessments, observations, and suggestions of problem areas, resident and employee activities, concerns, needs, conditions, etc. 6. Observe and evaluate residents' health, mental and emotional conditions and communicate significant changes to appropriate staff. When indicated is responsible for notifying the physician and family members according to prescribed procedures. 7. Provide direct patient care as appropriate including, but not limited to, procedures such as catheter placement and care, colostomy care, suction, dressings, naso-gastric tubes and other treatments. 8. Participate in report at the beginning and end of shifts to assure proper information about residents is communicated. 9. Assist in the admission, orientation, and assessments of new residents, assure that physician orders for diets, therapy, medication, etc. are transcribed and communicated according to procedure. 2 10. Participate in resident care conferences, formation of the care plan and goals and update nursing care plans as appropriate. 11. Provide explanation and offer understanding of nursing care to residents and families according to procedures and provide instructions relating to medical care as appropriate. 12. Administer emergency care when necessary. Contact emergency medical services for assistance when required. 13. Obtain specimens and samples as ordered by physician, complete requisitions and assure timely delivery to appropriate laboratory. 14. Makes rounds with physician, assisting with examinations as requested. 15. Assure physician's written and verbal orders are complete and promptly documented and communicated and care planned appropriately. 16. Maintain clinical and medication records. 17. Responsible for security of medications within the facility. 18. Supervise the preparation, administration and recording of medications according to physician's orders. 19. Assure the inventory of medication, re-ordering as needed. 20. Count and record controlled substances to be stopped or prescriptions that are expiring. 21. Assure the functional and sanitary maintenance of the nursing unit and equipment. 22. Report routine and emergency maintenance needs of the unit according to established procedures. 23. Assure the safety and security of patients through the proper use of equipment, safety devices and knowing and following emergency procedures. 24. While the position has no direct budgetary accountability, is responsible for economical and efficient use of equipment, supplies and staff on the unit. 25. Communicate with other department supervisors, physicians, consultants, staff, residents and families to coordinate care of residents. 26. Complete other duties according to applicable procedures of the assigned shift. 27. Stimulate interest and attitude in personnel to develop understanding of residents. 28. Attend committee meetings as necessary. 29. Function as Building Supervisor as scheduled. Page 2 of 5

3 30. Perform other related duties as assigned. The customer service expectations for this position are as follows: Ensures the skilled nursing unit complies with all state and federal regulations at all times. Complies with the Nurse Practice Act and Resident Bill of Rights. Reports to work on time and as scheduled. Oversees the accuracy and thoroughness of all documentation on the nursing unit. Participates in employee training. Completes employee performance appraisals within the time frame established by policy. Makes daily rounds on the unit to connect with staff and residents. Keeps current on all changes in the industry, particularly regulatory changes. Maintains complete confidentiality. Reports resident change in condition to the Director of Nursing and Administrator. Keeps current on changes in the industry, including clinical and regulatory changes. Exemplifies department dress code and has a presentable, professional and well-organized desk area and unit. Ensures standards of nursing practice are consistently met. Complies with established Infection Control and Safety Guidelines. Works with other disciplines to maintain high standards of resident care. Knows residents and staff on skilled unit by name; addresses residents by their preferred name. Establishes personal contact with new admissions or transfers within 48 hours of move in. Listens to staff concerns and offers solutions within established protocols. Responds to resident and family requests in a timely manner. Responds to staff concerns within established protocols. Responds to voice mail and messages within 24 hours of receipt or by date requested by sender. Responds to all memos and written requests within time frame specified. If out of the office for more than one business day, an appropriate voice mail message should be recorded and an Out of Office reply should be set up through their account. QUALIFICATIONS This position requires a graduate of an approved school of nursing, the staff RN must be currently licensed as a Registered Nurse in the State. A minimum of 1 year nursing experience necessary. The position requires a person with devotion to standards and principles of professional nursing, as well as the following knowledge and skills: clinical knowledge and technical expertise clinical systems Page 3 of 5

4 problem solving skills receptive to new ideas time management self-motivation for own professional development and improvement. The position further requires skill in assessing problems, planning and evaluating solutions in relationship to routine supervision of LPNs and nursing assistants and resident needs. It requires understanding, patience and tact in dealing with people. The Staff RN must be organized and have the ability to work with limited supervision, take initiative, and have the ability to plan and carry out responsibilities, organizing own schedule and duties in order to complete responsibilities. The ability to maintain confidentiality and good judgment are necessary. The position demands attention to detail, accuracy and organizational skills. This position requires the ability to work under pressure, ability to work independently and arrange work schedule to meet objectives and deadlines. Must also have good judgment in determining needs of residents and in maintaining resident care delivery in conformance with recognized standards; and is expected to understand and follow all regulatory and corporate policies related to confidentiality of the resident's medical and personal information. Must be able to work within standard policies and procedures. The individual must be dependable, have excellent inter-personal relationship skills, flexibility, and the abilities to deal well with others and handle a variety of tasks and responsibilities in a professional, prompt manner are essential. The person must be tactful and possess excellent communication skills (written and verbal, English), and be able to communicate effectively with administration, staff, residents and others with whom this position has contact. The individual must be courteous and relate well to others, have genuine compassion for senior adults and the disabled, project a friendly character, understand customer service and team building concepts. This position further requires good human relation skills and the ability to monitor and direct the activities of others; the ability to enlist cooperation of unit personnel and staff in other departments; the ability to maintain good professional relationships with physicians and to relate well to residents and families. This position may require availability when necessary to work beyond normal working hours and in other related positions on a temporary basis; as well as availability on weekends and holidays, or on days and shifts other than those for which hired. PHYSICAL REQUIREMENTS This position requires frequent standing and mobility to move throughout the facility. Manual dexterity and keen vision are necessary in use of a computer and to perform various nursing duties including the use of a thermometer, blood pressure cuff and other related nursing equipment. This position requires occasional handling of lightweight materials and the physical ability to use the telephone, computer and keyboard, printer, facsimile machine, copy machine, and other related office equipment. The position requires an individual who can support and lift residents (support up to 100 pounds, lift up to 25 pounds independently and lift up to 50 pounds with assistance). May be exposed to unpleasant conditions and is at risk of exposure to blood, bodily fluids, and other potentially infectious material. EQUIPMENT Equipment used in performance of duties includes general office equipment including a telephone, computer and keyboard, printer, facsimile machine, copy machine, and other related office Page 4 of 5

5 equipment, as well as routing nursing equipment which may thermometer, stethoscope, blood pressure cuff, wheelchair, patient lifts, IV equipment, suction machines, nebulizer machines, and oxygen equipment ROLE-BASED SECUITY AND ACCESS In compliance with HIPAA standards, role based access and security guidelines have been established for this position. Reference Role Based Security and Role Based Access grids found in section 7 of the HIPAA Resource manual. COMPUTER ACCESS In performance of duties this position needs access to the phone system, GroupWise , Vision, CareWatch, RiskWatch. LifeConnect, Omniview, CovBuy and AccuNurse.
