Box 600 Trenton, New Jersey WOST FORM Instructions JAMIE FOX Commissioner Please follow the instructions below to properly fill out form 1. Need ADOBE READER (We recommend you install the latest version of Adobe Reader). If you have ADOBE READER and are having trouble filling in the form please update to the latest reader OR reinstall ADOBE READER. a. It is a free install. b. Go to Adobe site to install the latest Adobe Reader. OR c. Go to google and search for Adobe Reader. Then select the result that is the adobe site. 2. We recommend you use IE (Internet Explorer) ver 8 or above to fill out ADOBE READER browser version of the form. a. If you do NOT have IE or another Operating System like MAC then we recommend you save the file locally and fill it out form in ADOBE READER NOT ADOBE READER BROWSER. i. NOTE: If you fill out the form locally in ADOBE READER and NOT the browser you will be prompted to save a copy when entering a digital signature (see digital signature instructions). 1. Save a signed copy 2. Open SIGNED copy and use the SIGNED copy to click the appropriate button 3. If required you may need to attach to the PDF form a copy of your driver s license and Insurance Information. Please have that available for selection on your computer. 4. Filling out the form: a. Follow the instruction on the form Fill out all fields to the best of your knowledge. b. Attach a copy of driver s license and insurance information (if required). c. Provide a digital signature. (see Digital Signature Instructions on page 3) d. 2 Page 2 Troubleshooting Issues 1. If you are having trouble providing a digital signature: a. Clicking inside the digital signature box and NOT being prompted with SIGN DOCUMENT dialog box 2. Having trouble attaching driver s license and/or insurance information inside designate box 1 and 2 INSTALL LATEST ADOBE READER VERSION OR IF YOU HAVE THE LATEST VERSION REINSTALL ADOBE READER 3. If you are having trouble ing the form using the Form s button. a. Save a copy of the filled out signed form to your machine (If you haven t already - Users may have filled out from a file on their machine if they didn t use the browser) b. Go into your mail account and create an manually. c. Copy the appropriate address from the PDF Form. d. Paste it in the TO box of your form e. Attach the signed saved copy f. Send it manually.

3 Page 3 Digital Signature Instructions Please follow the instructions below to create a digital signature 1. Need Adobe Reader (We recommend you install the latest version of Adobe Reader). a. It is a free install. b. Go to Adobe site to install the latest Adobe Reader. OR c. Go to google and search for Adobe Reader. Then select the result that is the adobe site. 2. Now that you have this document opened in Adobe Reader and it s the lasts version you can begin creating a Digital ID. 3. Click Signature Box on page Sign As Dropdown SELECT- New ID (Some versions of Adobe will take you directly to step 5) OR You may be prompted to Step 5 if you are doing this for the first time. 5. I want to sign the document using SELECT A new digital ID I want to create now THEN CLICK NEXT.

4 Page 4 6. Where would you like to store your self-signed digital ID? VERIFY New PKCS. is selected CLICK NEXT. 7. Enter your identity information: ENTER Name (FIRST LAST Name), Organizational Unit (BUREAU), Organization Name (DIVISION), and Address CLICK NEXT.

5 Page 5 8. Create a password for your signature. Click FINISH. 9. Now that you have a DIGITAL ID we want the signature to appear a particular way. a. Click the signature box if Sign Document dialog does not appear b. At sign-in Sign As: Select your newly created digital id. c. Appearance dropdown SELECT Create New Appearance. d. Configure the Appearance of your signature. Title: Name the appearance configuration what you want OR NJDOT Appearance

6 Page 6 Configure Graphic: SELECT Name Configure Text: Only DATE, NAME, LABELS should be selected (see picture below) CLICK OK. Sign documents 1. Click Signature Box. YOU ARE NOW READY TO SIGN DOCUMENTS!!!!!!! IF YOU FORGET YOUR PASSWORD CREATE NEW ID. 2. Select Your Created ID. 3. SIGNATURE WILL BE ENTERED. IF YOU FILLED OUT A SAVED COPY THROUGH ADOBE READER YOU MAY BE PROMPTED TO SAVE THE DOCUMENT PLEASE SAVE THE DOCUMENT WITH THE WORD SIGNED_ AS THE PREFIX OF THE FORM NAME wost_agreement2015.pdf SO signed_wost_agreement2015.pdf

7 Page 7 Instructions Please follow the instructions below to the completed form 1. To the completed form please click the button of the appropriate region to which you are applying: Based on Preference selection the will go to the appropriate region. Ba North Region WOST Coordinator SnowVolunteers.North@dot.nj.gov Central Region WOST Coordinator SnowVolunteers.Central@dot.nj.gov South Region WOST Coordinator SnowVolunteers.South@dot.nj.gov 2. This will bring up the dialog box: 3. If your machine is set up with OUTLOOK use outlook as you method for ing the form. 4. You can also use Webmail (Hotmail, Google, and Yahoo) to send your . a. Google and Yahoo are already set up BUT you may have to set up HOTMAIL. b. To set up HOTMAIL i. Select USE WEBMAIL Then select OTHER ii. ENTER: IMAPI: imap mail.outlook.com SMTP: smtp.live.com

8 Page 8 USERNAME: <user account> PASSWORD: <User password> c. For Google and Yahoo the IMAPI and SMTP are already set up simply enter your account info 5. If using OUTLOOK the will be setup for click SEND 6. If you use webmail account it will be sent automatically. You will NOT have to click SEND on a message
