1 STB PORTAL Affidavit Report Step : Go to STB Portal via the STB website (achieve.lausd.net/testing) Log-in using your LAUSD SSO and password (ex: wsc7897@lausd.net) Under Coordinator Apps, Click on Security Forms Step : Step 5: Click on Reports option to generate a list of staff members who electronically signed an affidavit From the drop menus select the following:. School Year: Test Program: ELPAC/CAASPP. Document Type: Leave blank. Active Location: Pre-populated with your school location. Add multiple location selection for schools with a magnet, dual language and or academies. 6. Print your report list by selecting print or export to an excel file.

2 Navigating MyToms How to Access MyTOMS To Access MyTOMS, go to: Scroll down and choose Test Operations Management System (TOMS) and log in. Creating MyTOMS Accounts In MyTOMS,. click on Users.. click Add.. Enter User Name and Contact Information and;. Click NEXT. Assigning Role(s):. Click the drop down menu under * Role, and enter role(s) as ELPAC Test Examiner.. Click the drop down menu under * Administration Year(s) and change to Click NEXT. Confirm and Save User.. Click SAVE. 3 Eligibility Reports Initial ELPAC In MyTOMS,. Click the Reports tab,. Click on LST Reports,. Click on the Initial ELPAC Student Eligibility Report and. DOWNLOAD REPORT. Wonderful School [967056] Reports will download as a.csv file. Feel free to sort reports in any order as needed. Entering Scores into LST In MyTOMS,. Click on LST.. Enter the students SSID # and click Search.. Click on enter scores next to student name. Wonderful School [967056] Great Student Wonderful School

4 Entering Scores into LST Cont.. Verify Student Demographic information.. Enter test examiners name and date completed.. Using the student score sheet from the back of the answer book carefully enter scores for each domain.. Verify entries click on preview score reports. Good Student SSID: Grade: KN Local Student ID: 00000X000 District Name: LAUSD Step: Verify once again and click on Lock Scores and print official Student Score Report (SSR)

5 LST Roster In MyTOMS,. Click the Reports tab,. Click on Site Reports,. Click on Initial ELPAC Local Scoring Tool Score Audit Report. Download Report Wonderful School (967056) Reports will download as a.csv file. Feel free to sort reports in any order as needed.

6 Generating Reports Summative ELPAC Site Reports In MyTOMS,. Click the Reports tab,. Click on Site Reports,. Click on any of the following: ELPAC School-Level Student Test Assignment Report Shows if student has been assigned to take online assessment. ELPAC School-Level Student Test Settings Report Shows what Designated Supports or Accommodations have been entered for a student. ELPAC Summative Student Eligibility Report Shows eligible students that must take ELPAC Summative Assessment. DOWNLOAD REPORT. Reports will download as a.csv file. Feel free to sort reports in any order as needed.

7 Completion Status Reports Step : Sign-in with your MyTOMS account. Click on Links and select Completion Status and Roster Management Click on Completion Status Reports and select Plan and Manage Testing Plan and Manage screen A. In the Choose What section click on the Test Instrument field and select ELPAC. B. In the Get Specific section filter out the types of report you want to run selecting one of the radio bullets and changing the drop menus. C. Select Export Report it will download as an Excel file. Please note the most common reports to run are: st radio bullet: Change to have not completed. Report produced will show students whom have not started or not finished an assessment. nd radio bullet: Leave the Any in the first filter and change the last filter to Any. Report produced will show students whom have started, completed, currently testing and have expired assessments. rd radio bullet: Report produced will show student testing information only for the SSID entered.

8 Student Login Credentials In MiSiS sign in as a Categorical Program Coordinator or Office Manager. Click the Reports tab,. Click on Testing,. Click on Smarter Balanced,. Click on SBA Label & Student Roster, 5. Select Local District Campus then School/s and Grade Level. Filter by Teacher and Period. In Report Option select SBA Label. 6. View Report 7. Export as a PDF. Please Note: Depending on your printer, in the print window you will need to select either Fit or Actual Size in the page sizing and handling section in order for the labels to align correctly.
