1 Conversion at a Glance Midland States Bank will be converting Heartland Bank accounts over the April 17 weekend. After the close of business on Friday, April 17, your accounts held at Heartland Bank will be integrated into Midland States Bank. Branches will remain open on Saturday, April 18 with normal business hours. Former Heartland Bank branches will open as Midland States Bank on Monday, April 20. During the conversion weekend, you will be able to access your bank accounts through ATMs, writing checks and using your debit card. Limited balance and transaction details will be available through ATMs during the conversion. Your transactions, including deposits and withdrawals, will continue to be processed like normal, but will not be available through the ATM, Online Banking or Bank by Phone until the conversion is complete. Legend Look for this symbol for new enhancements to the way you bank. Please pay special attention to this symbol which indicates changes that could affect you. This symbol informs you of the few services that will no longer be offered. Questions? Through April 18, please contact Heartland Customer Service at Customer Care Center: Our toll-free Customer Care Center number to call after conversion weekend is MY-MIDLAND ( ) Important Dates and Information First Month Service Charges Waived On personal and business deposit accounts, we will waive monthly account service charges for the month immediately following conversion. Beginning June 1, normal charges will apply. This will allow you to become familiar with your new accounts and make account changes if you would like before monthly account service charges are incurred. Conversion Weekend Balances During Conversion Weekend, April 18 - April 19 - If you perform a balance inquiry at an ATM, the balance will reflect the end-of-day balance on April 17. Balances will not reflect the items posting to your account throughout the conversion weekend. For example, your transactions, including debit cards, deposits and withdrawals, will continue to be processed like normal, but will not be added to or withdrawn from your end-of-day balance until the conversion is complete. Balances will be refreshed on April 20. Account Number Changes Unless you have received a letter about an account number change, your account number will remain the same on deposit and loan accounts. Checks For your convenience, you may continue using your Heartland Bank Personal, Business, Home Equity Lines of Credit and Money Market checks. You can order new checks from our website, If you place your next order through another company, you will need to provide that company with the Midland States Bank routing transit number ( ) and your account number Customer Care Center Hours: 8:00 am - 5:30 pm, Monday-Friday 8:00 am - 12:00 pm, Saturday Routing Transit Number Your Account Number Conversion at a Glance 1

2 New Debit Cards April 15 - If you currently have a Heartland Bank Debit Card, your card will be replaced with a new Midland Debit MasterCard. This card will arrive in the mail no later than April 15. You can activate your new Midland card beginning on Monday, April 20. April 19 - You may continue using your current Heartland Bank debit card through midnight on Sunday, April 19. April 20 - You may activate your new Midland debit card beginning on Monday, April 20. Details explaining how to activate and change your Personal Identification Number (PIN) will be included with your new card. April 20 - If you currently authorize companies to use your Heartland debit card as a payment source, you will need to provide them your new debit card number prior to payments due on or after April 20. MoneyPass ATM Network April 17 - Access to MoneyPass ATM Network benefits will be discontinued effective end of business day April 17. If you use an ATM outside of Midland s ATM network, normal fees will apply. For a list of available ATMs in your area, visit Service Fee information is available within the enclosed Welcome Guide. Bank by Phone April 17 - April 19 - Bank by Phone will not be available during conversion weekend. April 20 - You can begin your Bank by Phone access by enrolling in Midland s Bank by Phone system. We have developed a special process which provides expedited enrollment. If you have not enrolled within 30 days after conversion, please refer to the instructions provided within the enclosed Welcome Guide for our standard enrollment and access instructions. Online Banking and Bill Pay After this conversion, you will have access to your new Midland Online Banking and Bill Pay service at www. midlandsb.com. Online Banking Schedule - Prior to April 17 - We recommend you download or record your account history, statements, recurring transfer schedules and bill payment information prior to the conversion. 90 days of transaction history will convert. April 17 at 5 pm - Heartland s Online Banking access will be turned off. April 20 - You can begin accessing your accounts through by logging into online banking through the Sign In link located in the upper right corner beginning on Monday, April 20. Using Online Banking for the First Time - You will be able to continue to use your current user ID (user name) and password. You will have multiple prompts that will be required the first time you sign in to Online Banking, including acceptance of the new terms and conditions of Midland s online banking. Note: You will need to be on a Personal Computer (not a smart phone device or tablet) the first time you sign in to Online Banking. After accessing the first time, you will be able to use different devices. April 20 - Your previous 12 months of Heartland online statements will be available. April 21 - Transactions performed over the conversion weekend will be available for viewing through Online Banking. Check images processed after April 17 will be available online. Scheduled Recurring Transfers and Alerts - Scheduled recurring transfers between your Heartland accounts will continue as scheduled after conversion. Online Bill Pay Schedule - Prior to April 17 - We recommend you download or record your payees and payment history prior to the conversion. 24 months of bill pay history will convert. April 17 at 6 am - Your ability to schedule or pay bills online at Heartland will be turned off in order to bring your bill pay and payee information over to Midland. April 20 - You can begin accessing your Online Bill Pay account through We recommend you check your payees (including all payee details) and your scheduled payments to make sure your information converted correctly. April 20 - You may notice your payment date may have changed because Midland uses a deliver by verses a send on date method. A deliver by method pays based on when your payment is due. A send on date method requires you to calculate the time to send before the date the payment is due, as well as the way it will be sent. Midland will calculate when the payment should be sent based on whether it is to go by check or electronic methods, according to the deliver by date. After April 20 - Set up bills to be presented for payment within Online Banking. This is called e-bills. You will be notified by when your bill arrives. Pre-scheduled Bill Payments - Your pre-scheduled online bill payments will continue even though access will not be available during conversion weekend. 2 Conversion at a Glance

3 Mobile and Text Banking April 20 - For our Online Banking users, you can begin accessing your accounts through our Mobile App and Text Banking services beginning on Monday, April 20. April 20 - Online Banking users can begin making mobile deposits through our Mobile App beginning April 20. External Transfers - Popmoney Prior to April 14 - We recommend you download or record your external transfer activity and settings. Your Heartland Bank settings, including account set-up will not convert. Once the conversion is complete, you will be able to reestablish them using Midland s Online Banking service and clicking on the Move Money tab. April 14 at 5 pm - Your ability to schedule External Transfers or Popmoney transfers will be turned off. Transfers scheduled to deliver after April 17 will not be processed due to the conversion. If you would like to initiate a Wire Transfer between April 15 - April 17, please contact Heartland s Customer Service team. April 17 - This is the last day External Transfers and Popmoney transfers will be delivered via Heartland s Online Banking system. Transfers scheduled for delivery past April 17 will not be processed. Online Statements and Notices April 20 - You can register to receive Online Statements when you sign in to Online Banking. Once you register for Online Statements, online notices will also present within the same area. You can turn on or turn off electronic statements and notices by account. Purchase Rewards April 20 - You can view your Purchase Rewards information online. Learn more about Purchase Rewards at www. midlandsb.com/mobile, under Earn Cash Back. FinanceWorks April 20 - FinanceWorks information will convert. You will need to refresh your external accounts through FinanceWorks beginning April 20 to see balances and account transaction history. Certificates of Deposit (CDs) and Individual Retirement Accounts (IRAs) Changes Early Withdrawal Penalties - The terms of your CD or IRA as established with Heartland Bank will not change until the first maturity after the conversion to Midland. However, new early withdrawal penalties apply at the time of conversion. Please refer to details in the enclosed Welcome Guide. Loans Personal Loans, Home Equity Loans and Mortgages. The terms and conditions of your loans will not change. Automatic payments will continue. Home Equity Lines of Credit. Automatic payments will continue. You will receive additional information in a separate mailing as the terms and conditions of your loan will change. A Home Equity Discover Card is available for more convenient access to your line of credit. Overdraft Reserve Line of Credit. If you currently have an overdraft line of credit, the terms and conditions of your line of credit will convert to Midland s Reserve Line of Credit. The new terms and conditions have already been mailed to customers with an overdraft line. Business Loans. The terms and conditions of your loans will not change. Automatic payments will continue. Health Savings Accounts (HSA) April 20 - Midland s Health Savings Account provides account owners the option to use checks and HSA debit cards to pay for qualified medical transactions. You may order Midland HSA checks beginning April 20. Savings Account Transfers April 20 - Service Fees associated with Savings Account Transfers for overdraft purposes, will post to your account receiving the transferred funds beginning April 20. Statements After April 17 - You will receive a final Heartland Bank statement reflecting your account activity and balance through April 17. Personal, Money Market and Business Accounts - You will receive your Midland statement within 30 days after this conversion. Your statement will then cycle on the same day each month thereafter. Home Equity Lines of Credit - Your statement date will change, and your payment due date may change. Please check your first Midland Home Equity statement carefully to understand your next due date. After April 20 - If you would like to combine multiple account information into one statement, please contact our Customer Care Center after April 20. Combined statements will not convert and will need to be reconnected. Safe Deposit Box The terms and conditions of your safe deposit box will not change with the exception of the annual fee. The new annual fee will be effective as of April 17. Please refer to details in the enclosed Welcome Guide. Business Online Banking and Bill Pay After this conversion, you will have access to your new Conversion at a Glance 3

4 Midland Business Online Banking and Bill Pay service at After conversion, if you would like a free demonstration, please contact your local bank representative to schedule the best time to meet. Remote Deposit Capture for Businesses Deposit Window - On Friday, April 17, your company can make deposits until 5 pm for the current business day. After the conversion, the cut-off time will occur at 6 pm for the posting to occur on the same business day. Incoming Wire Transfers April 20 - You will need to communicate new Midland wire transfer instructions to parties sending wire transfers to your account. Bank Name: Midland States Bank 1201 Network Centre Drive Effingham, IL ABA Credit to: Customer Name Account Number ADDENDUM TO THE WELCOME GUIDE We have made a few changes since the printing of the enclosed Welcome Guide. Changes are highlighted in orange text. The changes listed below are effective upon conversion, April 17 and replace the identified sections within the Welcome Guide. Your New Account Guide NEW! IOLTA Checking, CO will convert to the Lawyers Trust Account listed on page 13 of the Welcome Guide. PAGE 4 Liv Unlimited Checking. CHANGE! Trusted ID is no longer offered with Liv Unlimited Checking accounts. Any and all references to Trusted ID within the Welcome Guide are not applicable. Identity theft protection with Trusted ID has been replaced by an Identity Theft Assistance Package. If you have a Liv Unlimited Checking Account, you will automatically receive Identity Theft Assistance and registration is not required. Identity Theft Assistance is available to you at no additional cost with your Liv Unlimited Account. Included with your Identity Theft Assistance Package: Personal Identity Protection. If your identity is stolen, this benefit offers reimbursement for some expenses you may incur to restore or repair your identity. There is a $2,500 lifetime benefit. Payment Card Fraud Protection. Fraud specialists are available to assist you if your credit or debit cards become lost or stolen. They will provide assistance in canceling and reissuing your cards, and provide guidance to help protect your finances. Identity Theft Aid. For other fraud issues and concerns, this service offers unlimited access to a personal fraud specialist who will provide education, guidance, identity recovery support and resolution services, as well as concierge assistance. PAGE 5 Liv Rewarded Checking. CHANGE! Trusted ID is no longer offered with Liv Rewarded Checking accounts. Any and all references to Trusted ID within the Welcome Guide are not applicable. Identity theft protection with Trusted ID has been replaced by an Identity Theft Assistance Package. If you have a Liv Rewarded Checking Account, you will automatically receive Identity Theft Assistance and registration is not required. Identity Theft Assistance is available to you at no additional cost with your Liv Rewarded Account. Included with your Identity Theft Assistance Package: Personal Identity Protection. PAGE 19 Courtesy Overdraft Policy A $31 fee is assessed for each item, whether the item is paid or returned. The total of your overdraft balance, including any and all bank fees and charges, is immediately due and payable when incurred, and you will continue to be liable for such amounts pursuant to the terms of the account agreement. 5 phone the following three ways: By accessing your online banking account and using your web browser, By downloading the Midland States Bank mobile app, and/or By SMS Text Banking commands You may then: Transfer funds between authorized accounts Make payments from checking to loan accounts with us Get information about account balances in your checking, savings, loans and/or money market accounts. PAGE 21 MasterCard Debit Card Limits The standard daily limits (per card) are: ATM cash withdrawals - $ POS (PIN) or signature - $2, MasterCard Debit Liability Additional Limits on Liability for MasterCard Debit Cards. Zero Liability Protection applies to transactions made in the store, over the telephone, or online. As a cardholder, you will not be held responsible for unauthorized transactions if your account is in good standing, you have exercised reasonable care in safeguarding your card for loss or theft and you promptly notify us of such loss or theft. The additional limit on liability does not apply to ATM transactions or to transactions using your Personal Identification Number (PIN) which are not processed by MasterCard; however, commencing October 17, 2014, Zero Liability Protection will apply to all transactions conducted with your card, including PIN-based transactions as POS and ATM. In addition, Zero Liability does not apply to cards used for commercial, business or agricultural purposes but do apply for small businesses.
