This immunization includes coverage for the following diseases: Tetanus, Diphtheria and Pertussis (Whooping Cough). Immunizations are given by appointment only. Please call the Findlay City Health Department at 419-424-7441 for an appointment. They are located at 1644 Tiffin Avenue, Suite A, Findlay, OH. Please understand that appointment times fill up quickly due to start of the school year. Please do not wait until the last minute to make your child s appointment. Also keep in mind the 7 th graders will be attending a camp at Camp Berry in October and will need to have the Tdap in order to go. When you call for an appointment, if your child has insurance or Medicaid please have the card information available. Most insurances cover immunizations. We will call and verify what your policy coverage is. If you do not have insurance, each immunization will need to be paid by you either with cash or local check.
