BARNET, ENFIELD & HARINGEY MENTAL HEALTH NHS TRUST JOB DESCRIPTION CONSULTANT IN CHILD and ADOLESCENT PSYCHIATRY The Beacon Centre, CAMHS TIER 4 INPATIENT UNIT (Substantive, whole time) Applications are invited for the post of full time Consultant in Child and Adolescent Psychiatry in the Beacon Centre, an established 14 bedded, Tier 4 Adolescent inpatient Unit within the London North Central Sector. Following the departure of the previous post holder we now need to recruit a dynamic consultant to jointly clinically lead the service with another consultant colleague. The successful applicant will work within a multidisciplinary team and be supported by a specialty grade doctor. BARNET, ENFIELD AND HARINGEY MENTAL HEALTH NHS TRUST BEHMHT provides mental health services to a population of just over one million people in the boroughs of Enfield, Haringey and Barnet. It also caters for a wider population in surrounding areas with specialist mental health services. In addition, a full range of child and adult community health services are provided in Enfield, covering a population of 330000. The Trust employs over 2900 staff across 40 locations. The Trust s vision is to be a lead local provider, coordinator and commissioner of integrated and enablement focused services to improve the health and well- being of the people of North London and beyond. Unit Philosophy The philosophy underpinning the Unit is that young people, struggling with severe mental illness, cope better living in a family environment. We believe it is less frightening and less stigmatising, and ultimately better, coping and recovering at home, rather than living in a psychiatric unit, which adds another level of complexity to the recovery process. However we recognise that for some young people an admission to psychiatric hospital, as part of the process of recovery, is inevitable. We would wish to keep the inpatient segment of this process as short as possible. For some young people their home environment is not conducive to recovery at the point of admission. We would wish to make brief and focussed interventions to make the home environment more conducive to recovery and adjustment and to ensure that family relationships and other social factors in the home setting did not 1
aggravate the mental health problems of the young person in a way which extended their stay in hospital. We believe young people should have access to specialist adolescent mental health interventions in the community from the specialist community adolescent teams (SCATs). Staff Composition The tier 4 service provides a multidisciplinary approach to assessment and/or treatment with different professionals contributing their respective skills, providing a unified and comprehensive service. The Multidisciplinary team consists of Registered Nurses (Mental Health, Children and Learning Disability), Psychological therapists, Psychiatrists, Systemic Therapist, Social Worker/AMHP and group workers. To fulfil our aims, staff work in a multi-disciplinary team, respond quickly to requests for assessment or consultation, and put young people and their carers at the centre of the process. The roles of different professional groups are clearly defined, but staff work collaboratively and flexibly. Good communication is paramount in effecting the delivery of good quality care. The service supports and encourages staff to co-facilitate group work to share and learn new skills. Joint working is also a highly effective way to learn more about the approach taken by staff from other professions different from one s own. Duties of the Post This full-time post is a well established post to jointly lead the clinical team with the other consultant child psychiatrist at Tier 4 Inpatient unit to serve the young people between the ages of 12 and 18 from national regions which also include the London boroughs of Barnet, Enfield and Haringey. This post is in addition to another substantive consultant in this service. The Beacon Centre is managed by Barnet, Enfield & Haringey Mental Health NHS Trust within its Specialist services Service Line which also includes the North London Forensic Service, Eating Disorder Service and Substance Misuse Services. The Unit provides 14 inpatient beds, operating 24 hours per day, 7 days a week. The unit averages approximately one new admission per week. The duties will include assessment and treatment of inpatients, consultation and liaison with CAMHS and collaboration with other agencies where appropriate. The appointee will, together with the senior nurse on the unit, take a leading role in recruiting and training other staff. Senior staff on the Unit will be expected to take an active role in the planning and development of the Unit. The post will initially be supported by a 1 w.t.e specialty grade psychiatrist, with the plan for a 1 w.t.e higher specialist trainee to join the team. This trainee will be supervised by either the current consultant within the unit or the 2
successful applicant for this post, depending on their experience. accommodation and secretarial support will be provided at the Unit. Office The successful applicant will take part in a 1 in 8 senior (consultant) CAMHS on call rota. The on-call rota covers the Borough of Barnet (including the Beacon Centre). The on-call consultant is only expected to provide telephone advice to the on-call CT trainee. General Responsibilities In addition to these principal areas of clinical work, the post-holder will have other responsibilities including: Compliance with legal obligations covering clinical practice (e.g. Mental Health Act and Code of Practice, The Children Act etc.). Compliance with proper and safe discharge procedures for inpatients (e.g. in line with DoH Guidelines on Discharge and Caring for People: The Care Programme Approach). A commitment to ensure that people from all ethnic groups have equitable access to appropriate services. Ensuring successful multidisciplinary working. Compliance with procedures whereby doctors report concerns about the professional conduct or competence of medical colleagues. Compliance with Trust procedures that involve users in their care. Joining the consultant on-call rota for Child psychiatrists for Barnet Compliance with confidentiality as advised by professional and DoH Guidelines. Consultants have a continuing responsibility for the care of patients in their charge and for the proper functioning of their departments. They are expected to undertake the administrative duties associated with the care of their patients and the running of their clinical departments. All consultants take responsibility, both singly and corporately, for the management of junior medical staff. In particular, they are expected to have responsibility for approving and monitoring junior staff holidays and study leave and junior staff locum arrangements where appropriate. Consultants are expected to be part of the management structure within the Trust. There are monthly meetings with consultant colleagues and an active participation in the Senior Medical Staff Committee is expected. Office/Secretarial Support The post-holder will work from a dedicated office and have access to secretarial support. Trainee Medical Staff Support Trainee medical staff are allocated by agreement with the Training Programme Director in accordance with service demands and training requirements. This post will have support from a core trainee doctor for 3
inpatients (Beacon Centre). There will be equitable support from all the trainee medical staff and non-training grade medical staff for outpatients, in agreement with the Service Manager. IT Support/Informatics The Trust uses the RiO computerised records system. The post-holder will have a computer, e-mail and full IT support provided by Hewlett-Packard CDS UK Teaching The consultant staff in the unit are expected to provide routine teaching to medical students who are attached to the unit. Students from UCL are regularly attached for a 3 week period. Provisional Time-table The timetable for the Job Plan is as shown which includes the nature and proposed timings of fixed commitments. The job plan will be subject to review each year and revisions may be proposed by the consultant to the Clinical Director. The timetable below is indicative only, and the post-holder will agree a job plan once in post. The job plan will be based on the Consultant Contract and will include up to 2.5 SPAs, which will include CPD, Trust management roles, plus audit, teaching, supervision and special interests, as agreed during the job planning process. N.B. The planning of fixed commitments should be arranged in discussion with the Clinical Director. Sharing of consultant responsibility will usually be by mutual agreement. Where agreement cannot be reached, a decision will be taken by the Clinical Director. Teaching and Research The appointee will take an active part in academic life. This will include teaching medical students and trainees and the development of an interest in research. To support this, the Trust is willing to negotiate honorary academic sessions with the Royal Free/UCH/Tavistock Clinic/Imperial College for a candidate who may have specific academic interests. Continuing Professional Development The post holder will be expected to participate in medical and multidisciplinary audit and other procedures for clinical governance. The Trust will support continuing professional development in terms of study leave and reasonable reimbursement of course fees. The Trust follows a study leave policy supporting doctors in accessing study leave time according to national guidance. Funding for study leave agreed in job planning is subject to approval through the Service Line Manager and Clinical Director. The post-holder will be expected to comply with the regulations of the Royal College of Psychiatrists on Continuing Medical Education. The post-holder will be expected to join the CPD Peer Group of consultant colleagues in the unit that meets weekly. This group also provides peer supervision. Accountability The post-holder will be professionally accountable to the Clinical Director and will relate managerially to the Lead Clinician, Service Manager and Assistant Service Director. Annual Appraisal will be agreed with the Medical Lead for the Service Line. Clinical Governance Clinical governance is assuming ever-greater importance within the NHS and there is strong emphasis on this area within BEHMHT. The appointee will be expected to contribute and participate in governance activities in depth and breadth. A strong desire to improve governance standards is essential. The post-holder will comply with the Trust s clinical governance requirements and participate in related initiative where appropriate. This will include participating in clinical audit and review outcomes, working towards achievement of national and local performance management targets, 5
complying with risk management policies, and participating in the consultant appraisal process. The post-holder will also be responsible for maintaining satisfactory patient records and, when relevant, for entering data onto a computer database in accordance with the rules and regulations of the Data Protection Act. The Trust has moved to electronic records using the RIO programme and the post-holder will be expected to use this system. Confidentiality All employees and honorary appointees are required to exercise discretion and maintain confidentiality at all times. Conflict of interest All applicants to any post within the Trust are required to declare involvement directly with any firm, company or organisation, which has a contract with the Trust. Failure to do so may result in an application being rejected or, if discovered after appointment that such information was withheld, this may lead to dismissal. Criminal Record In view of the nature of the work this post is exempt from the provision of section 4 (2) of the Rehabilitation Offenders Act 1974 by virtue of the Rehabilitation of Offenders Act 1974 (Exemption order 1975). The Authority accepts no responsibility for damage to or loss of personal property with the exception of small valuables handed to their officials for safe custody. Staff are therefore recommended to take out an insurance policy to cover personal property. Cover for Leave So far as it is practical, the Consultant appointed will be expected to share in the provision of cover for the annual and study leave of other Consultants in the specialty. Place of work The appointee may be required to work elsewhere within the Trust and/or at Community Resource Centres (hosted by other health agencies) within the catchment area in accordance with the trusts principal aim of flexible working by staff to enhance patient care and he/she will be fully consulted. No Smoking BEH MHT operates a No Smoking Policy which does not allow smoking at work. Personal Property The Trust cannot accept liability for loss or damage to personal property on official premises by burglary, fire, theft or otherwise. Staff are advised to provide their own insurance cover. Private Practice All consultants should adhere to the national Code of Conduct for Private Practice. Private Practice should as far as possible, be undertaken within the BEH MHT Trust s facilities, always ensuring that the needs of the practitioner can be met. In this way income generated can be used to further develop patient care within the Trust. Recruitment & Selection All employees who are responsible for recruiting new staff are required to attend a recruitment and selection-training course before they are allowed to be involved in the recruitment process. Relocation Expenses Assistance may be given to newly appointed consultants with part of the cost of their removal expenses, providing (generally) this is the first appointment in the National Health Service and also that the removal is necessary to comply with the Authority s requirements concerning place of residence. Residential Criteria The appointee will be required to live less than ten miles by road from the main unit, unless the Authority determines that residence at a greater distance is acceptable. The distance of ten miles usually equates with a travelling time of thirty minutes or less. A consultant must be contactable by telephone.
