2 TITLE: SPECIALTY: BASE: RESPONSIBLE TO: Specialty Doctor (4 posts) Emergency Medicine Enniskillen Hospital Clinical Director JOB SUMMARY These are new posts established to provide the Emergency Department in South West Acute Hospital with additional senior cover including out of hours and to improve the department s compliance with the European working time directive. The post holder will work under the clinical direction of the Consultants in Emergency Medicine from South West Acute Hospital. The post would be ideal for an experienced SHO equivalent who wishes to pursue a career in Emergency Medicine. It will also be ideal for those wishing to pursue a Non-Consultant Career Grade post in Emergency Medicine. This post would involve clinical work as a Middle Grade in the Emergency Department as well as an organised professional development programme customised to the individual s needs. Educational targets will be established at appraisal. The post-holders will undertake clinical sessions in the Emergency Department at South West Acute Hospital. Out of hours work will be required. There will be no on call commitment.

3 PROFILE OF HOSPITAL The Western Health and Social Care Trust provides services to a 5,000 square kilometres area of outstanding beauty in the west of Northern Ireland. County Fermanagh is one of the most beautiful areas to be found in either the UK or Ireland, and Enniskillen itself is set on an island between upper and lower Lough Erne. A water enthusiast s haven, the region is studded with numerous wooded islands and surrounded by high drumlin farmland. Enniskillen town has a wide variety of shops and bars and some of the best schools in Northern Ireland. Close by in the Republic of Ireland is the spectacular Donegal coast, amongst Europe s finest, and both Belfast and Dublin are within 2 hours drive. The South West Acute Hospital provides emergency medicine, acute surgical, acute general medicine, cardiology (including mobile cardiac car), stroke and rehabilitation medicine and care of the elderly services. It is also the base for maternity, gynaecology and paediatric services in the Southern part of the Trust. The radiology department offers comprehensive services including 24 hour spiral CT. A new fully equipped day case endoscopy/bronchoscopy suite has opened recently. The hospital has full 24 hour laboratory services. TCH now provides non-acute inpatient rehabilitation, ortho-geriatric care and stroke rehabilitation. There is an Urgent Care and Treatment Centre incorporating a cardiology assessment service, a Diabetes Day Resource Centre, and a full range of outpatient facilities. Support facilities include a full radiology department with spiral CT scanning, a dedicated day procedure unit and a purpose built laboratory block. NEW HOSPITAL PROVISION The South West Acute Hospital is a new 295 bedded acute hospital for the south west, and opened in June 2012, all Erne Hospital services have transferred to this new state of the art facility. WORK OF DEPARTMENT The Emergency Department provides a full range of diagnostic services for the local population. Approximately 27,000 attendances are logged each year. The new emergency department includes a 4 bay resuscitation room, four major cubicles, six intermediate/minor cubicles, and a separate children s area with two cubicle spaces. There are 5 Consultant led Clinics per week. Records are held on a computer database (Ascribe Symphony). There is a well established and fully operational PACS and digital radiology system in place. There is a training room equipped with video conferencing facilities.

4 STAFFING The Emergency Department lies within the Emergency Care and Medicine division of the Acute Services Directorate of the Western Trust. There is in place a Lead Clinician for Emergency Care, and there is also a Clinical Director of Emergency Care & Medicine for the Western Trust. The Emergency Department Facilities include departmental offices and two secretaries. The current establishment of Emergency Medicine Posts is: - Mr L Alan McKinney Western Trust ED Consultants (Erne) Mr T Allen Dr C Brown Dr I Crawford Mr M Rahman Dr A Santhosh Clinical Director of Emergency Care & Medicine Trust Locum Consultant Trust Locum Consultant Agency Locum consultant Associate Specialist Specialty Doctor 4 Specialty doctor posts (currently filled by locum doctors) 1 GP Trainee ENPs x 4 Nursing Staff 22.2 WTE KEY DUTIES / RESPONSIBILITIES Clinical duties To assess and treat new patients To act as resuscitation team leader for seriously ill or injured patients, including children while they are in the Emergency Department or To undertake review clinics To assist in the supervision and training of junior medical staff To perform minor surgical procedures including manipulation of fractures.

5 SUPERVISION ARRANGEMENTS The post holder will be supervised on a day to day basis by consultant presence Monday to Friday until 10pm, out of hours there will be an on call consultant available for telephone advice and to attend the department if required, e.g. major trauma. CONTINUING PROFESSIONAL DEVELOPMENT The post holder will have an Emergency Medicine consultant acting as his/her educational supervisor. He/she will be required to maintain registration for continuing professional development with College of Emergency Medicine. He/she will be expected to participate in regular appraisal in accordance with the Trust s arrangements for clinical governance. The appointee will normally be required to undertake yearly audit to ensure that the standards within the Emergency Department are in accordance with established guidelines. There will be protected time available for learning, research and audit. There is a dedicated medical library with links to the queen s university library and multidisciplinary education centre on site. The post holder will be entitled to study and professional leave in accordance with his/her requirements for continuing professional development. The leave, along with adequate financial support, will be made available through the Clinical Director.

If it is not possible to agree a Job Plan, either initially or at an annual review, there are agreed procedures for facilitation and appeal with the final decision being binding on both the doctor and the Trust. 9 TERMS AND CONDITIONS The National Terms and Conditions of Service for Specialty Doctors will apply. As per the current salary scale under National Terms and Conditions of Service for Hospital staff as updated via DHSSPS circular on rates of pay, the most recent HSS (TC8) 3/2010, with effect from 1 April 2010 to 31 March 2011 and all uplifts thereafter. The full-time appointment will be for 10 PA s per week. The successful applicant will normally be appointed to the grade for a one year probationary period which, if confirmed, will be extended without term and held until retirement under the terms and conditions of service. The appointee will maintain registration with the General Medical Council (London). The Western Health and Social Care Trust is financially responsible for medical negligence occurring in the course of a practitioner s employment with the Trust. The successful candidate will be responsible for making his/her own arrangements to provide cover for non-nhs work. Annual leave entitlement is 5 weeks (25 days) per year until 2 years service have been completed in the grade and 6 weeks (30 days) thereafter for practitioners remaining in the grade. Where the appointee in his/her immediately previous regular appointment attracted the 6 weeks annual leave per annum, this entitlement would remain. You are required to live within a distance of 30 minutes or 15 miles by road from your base place of work unless the employing organisation agrees that they may reside at a greater distance. Your private residence shall be maintained in contact with the public telephone service. He/she will be expected to be flexible and to co-operate with reasonable requests to cover for their colleagues absences where they are safe and competent and where it is practicable to do so. Where doctors undertake duties in accordance with this paragraph and such duties take place outside of their contracted hours, they will receive either an equivalent off duty period or remuneration. The post will be subject to termination at any time, by three months notice in writing given on either side. Occupational Health - He/She will be required to undergo a preemployment medical by the Occupational Health Department before

10 taking up the post. The candidate shall be required to produce a Certificate of Medical Fitness for Exposure Prone Employment. Accept that this job description will be subject to review in the light of changing circumstances and should be regarded as providing guidance within which the individual works rather than something which is rigid and inflexible. Working with Children and Vulnerable Adults The Trust is committed to carefully screening all staff and volunteers who work in a regulated position and have substantial, unsupervised access to children and vulnerable adults in accordance with the Protection of Children and Vulnerable Adults (NI) Order (2003). Confidentiality Clause All information concerning patients and staff must be held in the strictest confidence and may not be divulged to any unauthorised person. A breach of confidentiality will result in

11 disciplinary action being taken in accordance with the Trust s disciplinary procedure and may lead to dismissal. Data Protection Computer data should only be accessed if this has been authorised and is necessary as part of your work. Unauthorised access to computer data or helping others to access such data will result in disciplinary action being taken in accordance with the Trust s disciplinary procedure and may lead to dismissal. Records Management As an employee of the WH&SCT you are legally responsible for all records you hold, create or use as part of the business within the Trust including patient/client, corporate and administrative records whether paper-based or electronic and also including s. All such records are public records and are accessible to the general public, with limited exceptions, under the Freedom of Information Act 2000, the Environmental Information Regulations 2004 and the Data Protection Act As an employee you are required to be conversant with the Trust s policy and procedures on records management and to seek advice if in doubt. Whole Hospital Policies apply. ADDITIONAL POINTS From 1 January 1990, medical staff have not been required to subscribe to a Medical Defence organisation. It should be noted, however, that the Trust's indemnity only covers the Trust's responsibilities and therefore, the appointee is advised to maintain a membership of a recognised professional defence organisation for any work which does not fall within the scope of the Indemnity Scheme. 12 PERSONNEL SPECIFICATION POST: SPECIALTY DOCTOR FACTOR QUALIFICATIONS & EXPERIENCE MINIMUM CRITERIA ESSENTIAL Candidates must: Hold Full Registration and licence to practise with the GMC (London). Have at least 4 years full-time postgraduate training (or its equivalent gained on a part-time or flexible basis) at least 2 of which will be in a specialty training programme or as a Fixed Term Specialty Trainee in a relevant specialty; or Have equivalent experience and competencies. DESIRABLE It is desirable that candidates: Hold a higher diploma e.g. MRCP, MCEM. Hold ALS, ATLS, APLS certification. Have some training in teaching methods. ELIGIBILITY FITNESS TO PRACTICE LANGUAGE SKILLS HEALTH CLINICAL SKILLS ACADEMIC / RESEARCH SKILLS Eligibility to work in the UK. Is up to date and fit to practice safely. All applicants to have demonstrable skills in written and spoken English that are adequate to enable effective communication about medical topics with patients and colleagues which could be demonstrated by one of the following: That applicants have undertaken undergraduate medical training in English; OR Have the following scores in the academic International English Language Testing System (IELTS) Overall 7, Speaking 7, Listening 6, Reading 6, Writing 6. However, if applicants believe that they have adequate communication skills but do not fit into one of the examples they need to provide evidence.
