Denver Botanic Gardens Celebrates Día de los Muertos Restaurant and Culinary Vendor Form FAX NUMBER 720-865-3725 APPLICATION DEADLINE: Friday, September 28, 2012 Projected Event Attendance: 2,000 guests This application does not automatically guarantee participation in the 2012 Día de los Muertos celebration Restaurant and Culinary Vendors will provide Denver Botanic Gardens with: Food: medium-sized samples for Denver Botanic Gardens Celebrates Día De Los Muertos on Saturday, Oct 27, 2012. The restaurant will provide medium-sized portions of two or three different menu items (800-1200 portions of each). Each restaurant must provide all of its own equipment for serving, heating or chilling food. Extremely limited electricity is available. It is highly suggested to prep food at your restaurant and then bring with you to event. Each restaurant must provide its own staffing for booth. (We suggest bringing multiple people) Denver Botanic Gardens will provide the highlights below: Access to event attendees (approximately 2,000 guests). Restaurant name mentioned on Denver Botanic Gardens website. Restaurant name mentioned in our Botanic Buzz e-newsletter that goes out to over 40,000 recipients. You can provide coupons or pamphlets to drive business back to your restaurant. No selling of products or food is allowed onsite. Denver Botanic Gardens will provide each vendor a 10x10 area. This area will include (1) 8ft tables and 2 chairs.

No additional items may be added once accepted unless approved by event management. Booth Fee Booth are provided free of charge since you will be providing samples of food for our guests! Electricity Fee $25 will be assessed for each vendor that requests access to electricity. Certificate of Liability Insurance If accepted, all vendors must provide a copy of Certificate of Liability Insurance with Denver Botanic Gardens added as additionally insured. If you need additional information or clarification, please email specialevents@botanicgardens.org. Rules & Regulations Exhibitors are responsible for collecting and submitting all applicable city and state taxes. For additional information and forms, please contact the Colorado Department of Revenue. If you wish to share a booth with a friend, you MUST both apply together. You cannot add another vendor s product or any additional products after you have been accepted unless approved by event management. Vendors are required to be present and staff their own booths during the event. There are no booth sitters during the Día de los Muertos celebration. Please plan accordingly. Temporary Restaurant Food Establishment License 1) Each food vendor must apply for and receive a Temporary Restaurant Food Establishment License prior to the event. 2) Read Temporary Retail Food Establishment Guidelines Packet NOTE: Page 4 (Hand Washing Station). The Gardens does not provide this. This is your responsibility. 3) Any restaurants that have questions about their menus and regulations can contact Danica Lee, environmental public health program supervisor, at 720-865-5378, to discuss. The Temporary Retail Food Establishment Guidelines Packet should answer most questions. 4) When aforementioned forms have been completed, you will need to take them to Denver Department of Environmental Health for approval. Its new location: 200 W 14 th Ave (southwest corner of 14 th and Bannock), 2 nd floor between the hours of 8 a.m. and 5 p.m., Monday through Friday. 5) Although not required, we recommend calling the environmental health office in advance for paperwork approval. Call Linda at 720-865-5392 or Bernadette at 720-865-5390. 6) Participating food vendors from outside of Denver will also need to bring a copy of the retail food license from their facility to demonstrate that they work out of a licensed and inspected facility. 7) After bringing paperwork to the Department of Environmental Health office and getting approval, participating restaurants will need to bring the completed paperwork to the Department of Excise & Licenses, located at 201 W Colfax Ave, 2 nd floor. Please be advised that Department of Excise and Licenses closes at 4 p.m. 8) Effective January 1, 2010, restaurants only need to come to the Department of Environmental Health offices once annually for temporary restaurant licenses. After you ve obtained a license once, you can go straight back to the Department of Excise and Licenses to get any additional charity or annual event licenses for the year. In order to obtain a no-fee license for a charity event, the restaurant will need to bring a letter of participation from the event organizer. 9) If a restaurant has already obtained the proper documentation and approval from the Department of Environmental Health for another event during 2011, then you need only to bring the letter of participation to the Department of Excise and License, which will then issue the new license for Día de los Muertos.
Release I hereby release and forever discharge the Día de los Muertos event at Denver Botanic Gardens, all Denver Botanic Gardens employees, agents and volunteers from any responsibility, personal liability, claims, loss or damage arising out of or in conjunction with my application to or participation in the Día de los Muertos event. Denver Botanic Gardens will not be responsible for any injury sustained by vendors or guests while within the space designated for exhibits. Furthermore, I understand that I store my product at my own risk. By signing this form I acknowledge that I have read, understand and will abide by the policies outlined in the application and vendor letter. I understand that failure to follow these regulations can mean expulsion from all special events.
