BUTTERWICK HOSPICE CARE JOB DESCRIPTION Post : Lead Nurse- Day Hospice Reporting to : Registered Manager Location : Bishop Auckland Hospice Job Purpose All posts, whether providing or supporting the provision of our services, allow our mission and values to be upheld. The purpose of this post is to assist with the organisation and delivery of patient care for patients who have life threatening illnesses: to be responsible for the assessment, planning, implementation and evaluation of patient care in collaboration with the multidisciplinary team and to provide support for carers and families within the Day Hospices. Key Responsibilities To co-ordinate an effective and efficient Day Care service. To ensure that high clinical standards and quality of care are maintained, promoting good health practices and continuity of patient care wherever possible. To undertake home/ hospital visits to assess the needs of the patient, carers and relatives and to contribute to the development and delivery of high quality palliative care. To provide advice and direction within the Day Hospice, ensuring that staff and volunteers have a clear overall direction, understand the contribution expected of them, are able and equipped to fulfil it, and to feedback to the Line Manager any issues arising to ensure the welfare of staff, patients and volunteers remains paramount at all times. As part of the Day Hospice and wider clinical team, provide professional advice, which reflects the highest standards in nursing practice including, research, education and quality. To facilitate and maintain an environment where safe and accountable practice is encouraged by appropriate staff development in a climate of mutual support which will enable continuous development. To develop and maintain innovative and high quality nursing practices and satisfy appropriate codes of conduct and ensure service provision is appropriately focused. To contribute to Hospice planning including audits and quality / best practice initiatives. To attend relevant meetings within the organisation appropriate with role. Where appropriate, promote links with external agencies as well as GP`s,Social Services, Community Hospitals, identifying areas of common interest and enabling the provision of integrated packages of care. To maintain accurate statistics and records as required.
Responsibilities of all staff. To carry out the duties and responsibilities in compliance with Hospice policies and procedures, current legislation and codes of conduct as applicable. To be aware of working practices of any volunteers feeding back any concerns to the Voluntary Services Manager. To undertake the requirements of the role with an awareness of responsibilities under Health and Safety regulations. To comply with current infection control policies and procedures. To maintain good public relations and promote the Hospice philosophy and care in the wider community. To provide support in the form of training to other team members as appropriate. To undertake such other duties and responsibilities commensurate with the grading and Nature of the post. Education/Training and staff development. To identify training and development opportunities in conjunction with the Registered Manager and participate in personal training as may be necessary to meet the duties and responsibilities of the post. To ensure that personal development is completed in accordance with NMC requirements for maintaining registration. Undertake appropriate study in accordance with an individual development plan and in line with the clinical strategy and organisational objectives. Undertake the role of a Mentor to student nurses and ensure up to date mentoring training. This job description reflects the present requirements of the post.
