1 JOB DESCRIPTION 1. JOB IDENTIFICATION Job Title: Department: Practice Nurse Practice Nursing Team Responsible to: Nurse Practitioner / Senior Nurse Accountable to: Practice Manager (Your employers are the partners incorporating Skene Medical Group) Salary Range: 21,925-28,602 (pro rata) Hours of Duty: 15 hours per week - Monday to Friday (flexible between 8am and 6pm) 2. JOB PURPOSE To work as a member of a Practice Nurse Team, providing nursing services to the population of Skene Medical Group The post holder will support the Senior Practice Nurse in the development of the team and responsibility will be shared with nursing colleagues to ensure delivery of nursing services. The post-holder can work unsupervised. There will be shared responsibility for the supervising of lower grades of staff and the delegation of duties appropriate to their grade and experience. 3. 2 4. MAIN DUTIES/RESPONSIBILITIES Professional & Technical Maintenance and enhancement of professional knowledge, ensuring a high standard of clinical care. Awareness of need to work within protocols, policies and NMC guidelines. Contribute to the continued update of Skene nursing protocols and polices using evidence based practice. Keep a Personal Development Plan relevant to professional practice with guidance from Senior Practice Nurse Responsible for the implementation & treatment and nursing procedures. Ability to work unsupervised but able to recognise limitations and seek advice when appropriate. To participate in the nursing management of chronic disease and utilize opportunities to promote health, lifestyle choices offering advice and counselling to patients where required to make effective changes Assist in identifying new roles, as changes in service delivery are required, as a result of audit/ government legislation or for improving patient care. Undertake any training required for new role and participate in the support and mentoring of staff undertaking training, Support the teaching of junior staff and students Planning & Organisation Ability to identify, monitor and evaluate patient needs. Ability to contribute to nurse meetings and communicate information to other staff members. Participate in the identification of care needs within the practice population through practice profile or similar exercise. To liaise closely with General Practitioners, Nurses and members of Primary Health Care Teams and provide nursing input to the decision making processes affecting patient care within practice population. To keep clear, accurate and contemporaneous records in line with clinic record keeping standards and to adhere to the Data Protection Act with regard to patient information held on computer 5. SYSTEMS AND EQUIPMENT Desk top computer Printer Practice Telephone system Microsoft Office and Outlook Practice Computer systems Vision and Docman Nursing Equipment relevant to duties Medical Equipment e.g. ECG Machines, etc 2

KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED TO DO THE JOB RGN with preferably 2 years experience in Practice Nursing Excellent inter-personal and communication skills Good organisational skills Competent IT skills Ability to communicate and work effectively as part of a team Ability to prioritise and manage own workload Ability to use initiative / problem solve Knowledge and experience of Microsoft Office and Outlook Preferred knowledge of Practice-based computer systems Show empathy and tact when communicating with patients Written and audit research skills. To work in collaboration with the community nursing team. To maintain professional knowledge and attend in-service study appropriate to the post. Ability and willingness to delegate work appropriately and clearly. Participate and undertake clinical/management audits 9.
