Job Summary: 1. The Registered Nurse will have responsibilities as a keyworker for patients and families requiring palliative care in the community, to include assessments and reassessments over a 365 day basis. 2. The role will involve being a clinical resource to Assistant Nurses in the community as well as to Administrators within SinglePoint. 3. Registered nurses rostered to the SinglePoint service will provide rapid response to patients in the last year of life within a 2 hour time frame. 4. The role involves deputising for the SinglePoint Clinical Nurse Managers when required. The Registered Nurse will work closely with community colleagues, hospice and hospital staff in order to ensure that patients requiring end of life care receive an integrated service in a timely manner. 5. The role will include working overnight, taking calls from patients and professionals and doing home visits as necessary throughout North East Essex.
2 Key Responsibilities; 1. As a qualified nurse, providing high standards of holistic, individualised care and support to patients and families, through a process of assessment, planning, implementation and evaluation. 2. To act as a keyworker to patients in the community, contributing to the provision of the 24 hour SinglePoint service. 3. To provide a rapid response to patients with palliative care needs within the last 3 months of life within a 2 hour response time 4. To be responsive to changing patient needs and flexibly visit according to suggested triaged response time 5. To work shifts including overnight, triaging calls to the centre and visiting patients as necessary, often as a lone worker. 6. Take responsibility for accurate assessments of patients and families ensuring the care they prescribe is carried out by Assistant Nurses and other providers. 7. Ensuring patient records are accurately written, in line with NMC and hospice regulations. 8. To work with the Clinical Nurse Managers to plan and implement change and development within the Hospice where this would increase effectiveness and efficiency of practice, deputising for them as required. 9. To provide telephone support to patients, families and other professionals 10. To provide supervision and work alongside SinglePoint Assistant Nurses and provide guidance and expert advice. 11. To maintain and promote effective and efficient working and communication with other community professionals, hospital professionals, voluntary, private and statutory agencies and other hospices. 12. Participate in the audit process and encourage colleagues to embrace the opportunities gained from audit. 13. Assist in the identification and meeting of educational needs of Assistant Nurses. 14. To ensure end of life procedures are carried out sensitively and appropriately. 15. To verify death following training and when it is appropriate to do so in line with Hospice policy
3 16. Demonstrate awareness of emotional needs of patients and families and the sensitivity and communication skills to meet these needs. 17. Demonstrate effective communication with patients key workers in order to ensure continuity of care for patients across all sectors. 18. To maintain professional awareness of current developments in nursing and palliative care and evidence based practice in order to remain an effective practitioner. 19. Follow Hospice policy at all times. 20. Participate in Clinical Supervision both as a recipient and a provider 21. Comply with NMC guidelines 22. Be aware of own emotional needs, relating to the palliative care environment, and how to meet them. 23. To act as an advocate safeguarding the patients and family s rights and interests, abilities and aptitudes, encouraging empowerment and informed choice. 24. Liaise with professionals from other agencies for the benefit of the patient and their family. 25. To communicate sensitively and effectively both verbally and in writing with patients, families and colleagues including volunteers. In some cases there may be barriers to understanding or highly sensitive communication required. The post holder is legally required to have regard to the Mental Capacity Act 2005 Code of Practice.
