1 Cooper Y Summer Day Camp Summer 2015 Contact Information Kim Pair (402) kpair@ymcalincoln.org Summer Adventure Camp Adventure Campers and Extreme Campers Summer Adventure Camp is a licensed day camp for youth entering1st grade to entering 8th grade. Participants will be divided into groups based on their age/grade level. Children entering1st grade up to age 9 will be our Adventure Campers and Children age 9 (must be 9 by May 26) and older will be our Extreme Campers. See Below. Activities Activities include: arts & crafts, indoor & outdoor sports, music, nature, swimming, field trips, drama and more. Participants must be able to participate in field trips as alternative care or make-ups may not be available. Field trips are included in the cost of the program, you do not need to send extra money with your child. Many of the field trips and special event activities will take place during the hours of 9:00 am to 4:00 pm, please plan accordingly. 2 Summer Adventure Camp (continued) Swim Lessons During Summer Adventure Camp (Adventure and Extreme Campers) are offered the benefit of taking up to five, 2-week, sessions of swim lessons from 9:00 to 9:40am Monday through Thursday, for an additional charge. Participants must be enrolled in camp for both weeks the swim lesson session is offered. Swim Team Participants are offered the benefit of becoming a member of Crocodiles Swim Team from 9:00 am to 9:40am Monday through Thursday during Summer Adventure Camp and Teen Extreme, for an additional charge. Participants must be enrolled in camp for most of the Swim Team session to participate. Registration for swim lessons and swim team is separate and will begin in May. As registration dates arise, you will be able to register at the Front Desk. A reminder postcard will be sent prior to the start of registration with more details. Creative Camp Creative Camps are designed to give your child the opportunity to explore a particular interest area for a week during their summer break, attend one or all! Creative Camp will be held for kids who have completed Kindergarten to age 12 during each session. Day, Time & Location Monday-Friday 7:15 am to 5:45 pm. There will be eight 5-day sessions of the Creative Camps. Camp is held at Scott Middle School. 3 Running Creek A day camp for elementary school aged children, entering 1st grade through age 10. Your child will enjoy exploring the great outdoors, meeting new friends and taking part in many fun activities. Day & Time Monday-Friday 9:00 am to 3:00 pm. Camps will meet at the Cooper Y and participants will be transported to a local park for their activities. Extended care is available. Full payment for each day camp is due at the time of registration. 4 Little Explorers A day camp for preschool age children (3-pre-K) who want to enjoy the fun and excitement of a camp experience! The activities are designed to be appropriate for the developmental needs and skills of children this age. Children must be toilet trained. Day & Time Monday-Friday 9:00 am to 1:45 pm. Camps will meet at the Cooper Y and participants will be transported to a local park for their activities. Full payment for each day camp is due at the time of registration. Registration Information All Camps Required Registration Materials At the time of registration you will be required to provide the following information: Complete 2015 Child Enrollment Form Complete 2015 Child Information Record Current Immunization Record (or one must be on file for your child) Direct Draft Information -VISA, MasterCard, or Discover only (Summer Adventure and Creative Camp ONLY) Title XX Authorization, if applicable Your registration will not be accepted unless all of the required paperwork is turned in at the time of registration, including the full fee and/or the non-refundable and non-transferable $35 registration fee and $20 weekly deposit. Enrollment Forms and other Required Registration Materials must be submitted, in person, at the Cooper YMCA Front Desk. Paperwork that is dropped at the Front Desk, faxed, ed or mailed will not be accepted and your child/children will not be registered. Waitlist Procedures If a program or course is full, we offer the option to add a participant to the waitlist. Being placed on the waitlist does not guarantee a spot in the program. Please ensure that your contact information is current at the time you are placed on a waitlist, as Program Directors will contact waitlisted individuals on a first come, first served basis as space in the program becomes available.

5 Financial Assistance Financial assistance is available for those who qualify, please visit or any Lincoln Y for an application. Please allow 15 days for processing. If you receive financial though the Lincoln YMCA we ask that your deposit be paid in full at the time of registration for Summer Adventure and Creative Camps, we will apply the financial assistance to the program fee. If you are unable to pay the full deposit 50% will be required at the time of registration and you may contact Kim Pair at or kpair@ymcalincoln.org to make arrangements to pay the remainder by May 1. Cancellations Prior to June 1, 2015 families who cancel/withdraw from a week of camp will forfeit their $20 deposit for each week they cancel/withdraw. For all cancellations/withdrawals made on/after June 1, 2015, payment of 50% of the weekly fee for each week being canceled will be required at the time of cancellation. This includes Little Explorers and Running Creek. A Program Change/Withdrawal Form is required for any change or cancellation at least one week prior to the session to avoid being charged the entire weekly fee. The $35 registration fee and $20 deposit are non-refundable and non-transferable. We encourage all families to carefully plan out their Summer Day Camp needs prior to registration. Please take into consideration family vacations, other camps, time spent with family members, etc., so that you only register for the weeks that your child will need care. Prior to June 1, 2015, families who register for Summer Day Camp and later register for any other YMCA of Lincoln Youth Program (such as a Youth Sports Camp or Camp Kitaki) will have their $20 deposit credited to their YMCA account for the week(s). This change should be made using the Program Change/ Withdrawal Form. Photo/Video Disclosure Statement The YMCA of Lincoln, Nebraska may on occasion take photographs and/or video of its members or program participants for use in printed materials or by electronic methods. Your entry into YMCA facilities, participation in YMCA programs or participation in YMCA events grants permission for the YMCA of Lincoln, Nebraska to use these photographs and/or video in its marketing and public relations efforts. Registration Information- Camp Specific Summer Adventure Camp & Creative Camp Adventure & Extreme Campers Registration is on a first come/first served basis. At the time of registration, a $35 registration fee and $20 deposit for each week registered is due. The $20 deposit for each week will be credited toward the weekly program fee. The remaining weekly program fee balance will be drafted from your credit/debit card the Friday prior to the week registered. Both the registration fee and deposit are non-refundable and non-transferrable. If you receive financial though the Lincoln YMCA we ask that your deposit be paid in full at the time of registration, we will apply the financial assistance to the program fee. If you are unable to pay the full deposit 50% will be required at the time of registration and you may contact Kim Pair to make arrangements to pay the remainder by May 1. Payments The remaining balance for each weekly fee is due the Friday before the week your child will attend. Payments must be set up for a weekly credit/debit card draft. We accept Visa, MasterCard, and Discover. Title XX is accepted. The parent/guardian is responsible for the $35 registration fee, per the Title XX agreement between the Lincoln Y and the Nebraska DHHS. Proof of Title XX authorization and registration fee are required at the time of registration. Families responsible for a Family Fee per the DHHS authorization for Title XX benefits, must pay the Family Fee by the 1st of each month.
