2 The Luisiana Department f Revenue is utilizing tw-dimensinal barcde technlgy fr the resident (IT-540) and nnresident (IT-540B) individual incme tax frms. We are requesting that all participating vendrs prvide a shrt statement that describes what a tw-dimensinal barcde is and why it is being utilized. The statement is prvided in Appendix A. This statement shuld nly be prvided t the custmer if yur sftware reprduces the tw-dimensinal barcde. General Infrmatin The 2D barcde shuld be placed in the upper right hand crner f the frm. The area reserved fr the barcde is 4-5/8 X 1-11/16. This area includes a ¼ blank area arund the barcde. The barcde must fit within this area n the frm. The 2D barcde cntains the same infrmatin that is in the scan band, except the dcument number. The dcument number fr the resident 2D barcde is The dcument number fr the nnresident 2D barcde is The 3 f 9-barcde, which is printed in the bttm left crner is 6855O fr the resident, and 6865P fr the nnresident. The dcument numbers fr the resident schedule pages are 6851 and 6852 (same as scan band). The barcdes fr the resident schedule pages are 6851K and 6852L (same as scan band). The dcument numbers fr the nnresident schedules are 6862 and The barcdes fr the nnresident schedules are 6862M and 6863N. If yu prduce a 2D, yu must use these dcument numbers in the scan band and the 2D. Returns submitted with a payment r indicating a balance due shuld be mailed t PO Bx 3550, Batn Ruge, LA Returns submitted with a refund r n tax due shuld be mailed t PO Bx 3440, Batn Ruge, LA Use a carriage return <CR> t delimit fields. Each barcde field shuld have a carriage return, even if n infrmatin is cntained in the field. N punctuatin is allwed in any field. N hyphens, dashes, parentheses, r ther separatrs shuld be used. All alpha characters shuld be in uppercase. D nt zer fill numeric fields in the 8 clumns in the scan band. Negative amunts are nt accepted (if less than zer, leave blank). Only whle dllar amunts shuld be entered. Dn t include supplemental infrmatin in the barcde. Errr crrectin level shuld be set t 4.

3 Printers: T print a readable barcde, a printer capable f 200 dts per inch (DPI) minimum is required; hwever, 300 DPI r higher is recmmended. If the printer can prduce a graphic such as a tax agency seal r lg, then the printer shuld be capable f prducing a 2-D barcde. Barcde Layut: 1. Header Infrmatin 2. State Specific Data 3. Trailer Header Infrmatin: This infrmatin shuld be placed first in the barcde data stream. The first six fields in the barcde cmprise the fficial header. This infrmatin must be cnsistent amng all barcdes and is defined belw. Header Versin Number will be incremented each time the standards grup alters the physical structure f the bar cdes that were created using multiple header frmats. This value is static fr all bar cdes and is currently T1. Develper Cde, a fur-digit cde used t identify the sftware develper whse applicatin prduced the bar cde. The purpse f the field is t allw frms t be traced t the vendr prducing them. Sftware develper cdes are assigned thrugh the NACTP. Jurisdictin, an alphanumeric identifier indicating the taxing jurisdictin. Use the US Pstal Service s fficial state abbreviatins. Fr Luisiana, use LA. Descriptin, an alphanumeric identifier used t describe the frm being prcessed. The identifier can be used t rute the bar cde infrmatin t the crrect system fr further prcessing. Fr Luisiana, use 6855 fr the resident frm and 6865 fr the nnresident frm. Specificatin Versin, a number that identifies the versin f the specificatins used t prduce the frm bar cde. These specificatins are prvide by the jurisdictin prcessing the frm and describe the data layut in the barcde. Draft versins f the specificatins are nt assigned versin numbers. The final versin shall be 0 ; revisins thereafter will increase numerically. Sftware/Frm Versin, a vendr defined versin number that reflects which revisin f the frm is used t prduce the barcde. Nte: The symbl <CR> is used t represent a single carriage return character. Any frm field that is a required field in the 2D barcde and des nt have a value shuld be indicated by a single carriage return <CR>. This carriage return shuld measure 1 byte f data.

4 Sample Header T1<CR>9999<CR>LA<CR>6855<CR>0<CR>1.0<CR> Trailer: The trailer is the last field in the barcde data stream. The trailer is used t indicate the end f data has been reached. A static string f *EOD* is used as the trailer value. If, upn scanning the barcde, a trailer is nt fund, this indicates that sme data may nt be included in the barcde due t data size restrictins. There will be ne frm layut fr all sftware vendrs t reprduce. If yur cmpany is nt implementing the 2D barcde, yu will still have t reprduce the frm leaving the area (upper right-hand crner) fr the barcde blank. If yu are creating a 2D barcde, yu are still required t prduce the scan band prtin f bth the IT-540 and IT-540B. All vendrs must btain 2D bar cde apprval frm the Luisiana Department f Revenue if they develp: Scannable tax frms cntaining 2D bar cdes using its wn sftware Tax sftware that generate 2D bar cdes t be used with scannable frms develped by anther cmpany A cmpany must btain apprval befre releasing r distributing tax sftware capable f generating 2D barcdes. 5 Testing requirements: The Luisiana Department f Revenue will supply each sftware develper with five (5) different test scenaris fr each return (IT-540 and IT-540B). Sftware develpers are required t submit ne (1) printed tax return fr each f the five (5) test scenaris. All variable data in the scan band and the 2D barcde must be printed.
