1 British Clumbia Nephrlgy Scial Wrk Scpe f Practice This scpe f practice was develped by the British Clumbia Nephrlgy Scial Wrk Prfessinal Practice Cuncil (BCNSWPPC) in 2008 t identify, establish, and prmte prvincial nephrlgy scial wrk standards f prfessinal practice. The BC Nephrlgy Scial Wrk Prfessinal Practice Cuncil was riginally cmprised f scial wrkers frm Chrnic Kidney Disease practice and later expanded t include all mdalities f nephrlgy scial wrk practice. This scpe f practice was apprved by scial wrkers attending Nephrlgy Days cnference in 2008 and will be reviewed in tw years time. MISSION Nephrlgy scial wrk services supprt and maximize the psychscial functining and adjustment f patients 1 experiencing Chrnic Kidney Disease 2 (CKD) r End Stage Renal Disease (ESRD). This service is extended t families, caregivers, and supprt netwrks. Supprt services are prvided t amelirate scial and emtinal stresses resulting frm the interacting physical, psychlgical and scial factrs fr a persn living with CKD r ESRD. This includes shrtened life expectancy and an altered lifestyle. Scial wrk functins as a part f the interdisciplinary team and is respnsible fr fstering psitive treatment envirnments, plicies, and rutines that respect diversity including individual, cultural, gender, religius and ethnic differences. Nephrlgy scial wrk fsters and facilitates quality f life as perceived by patient, family and caregivers by respecting values f individuality, independence and chice. We prmte verall wellness thrugh all stages f care: preventin, diagnsis, chrnic illness management, treatment, and end f life care. EDUCATIONAL STANDARD 3 We, the BC Nephrlgy Scial Wrk Prfessinal Practice Cuncil, advcate that nephrlgy scial wrkers preferably have a Masters f Scial Wrk degree (MSW) frm an accredited Schl f Scial Wrk. In situatins where MSW trained persns maybe difficult t recruit due t supply r gegraphical factrs, a Bachelr f Scial Wrk degree (BSW) with substantial related experience shuld be cnsidered. Furthermre, educatinal standards fr scial wrkers shuld be cnsistent with thse set by the British Clumbia Cllege f Scial Wrkers and the apprpriate emplyers. 1 Patients has been used t refer t peple with renal disease r clients in recgnitin that this is a frmally recgnized term used by the medical system. 2 CKD refers t the predialysis/ pretransplant stage f the disease whereas ESRD r ESRF refer t the initiatin f dialysis r transplant treatment. 3 BC Prvincial Renal Agency is nt respnsible fr regulating prfessinal credentials r hiring practices fr BC Health Authrities unless therwise specified. 2 ETHICAL PRACTICE The nephrlgy scial wrker is t practice with accuntability, cnfidentiality, ethical guidelines, and withut cnflict f interest. In BC, registered scial wrkers (RSW) must fllw the Cde f Ethics and Standards f Practice as utlined by the B.C. Cllege f Scial Wrkers (BCCSW). Other useful resurces which supprt nephrlgy scial wrk ethical practice are the British Clumbia Assciatin fr Scial Wrkers (BCASW), Canadian Assciatin f Scial wrkers (CASW), and the Canadian Assciatin f Nephrlgy Scial Wrkers (CANSW). PROFESSIONAL PRACTICE STANDARDS 1. Referrals, Assessment and Cntinuity f Care Aim fr psitive health utcmes by reducing the risk f psychscial cmplicatins, delaying disease prgressin, prmting wellness, and facilitating links t apprpriate resurces. Accept referrals frm patients, families, health care prviders, cmmunity partners r any ther party invlved in care. Triage, assess, and manage high risk and cmplex cases. Examples f psychscial risks and issues, which may be screened, are as fllws: Abuse, Neglect, Safety Activities f Daily Living Advance Care Planning Caregiver Stress Cgnitive Deficits Cping, Anxiety, Depressin Grief, Bereavement and Lss Educatin and Vcatin Family Stress and Cnflict Finances Health Cnsent Cmplexities Hme Envirnment Illness Management Immigratin and Settlement Islatin Language and Cultural Needs Legal Resurces Medical Benefits Mental Health Rehabilitatin Self Image Sexual Health Shelter Scial Supprt Substance Misuse Transprtatin Treatment Decisin Supprt Other Cmplex Care Needs Facilitate cntinuity f care by cmpletin f timely, cmprehensive and cncise psychscial assessments, updating chart crrespndence, reprting t team members, and transferring relevant infrmatin t key service prviders fr seamless transfer f care. Cunselling and Advcacy Cunsel patient and family regarding cping, crisis management, prblem slving, health management, decisin making, resurces, and ther psychscial needs by utilizing apprpriate therapeutic interventins. Share infrmatin abut peer supprt resurces with patient and family t cmplement emtinal supprt and educatin needs. In cllabratin with the health care team, ensure patients, families and caregivers have accurate and adequate infrmatin abut diagnsis and prgnsis s that they are able t make the apprpriate health care decisins. Prvide help with negtiating the cmplex health care system and advcate fr patient and family rights within the health care system. 3. Cultural Cmpetence Cllabrate and wrk effectively in diverse multicultural, sciecnmic, demgraphic, and gegraphic envirnments. Advcate fr fair and equal access t service fr all human beings. Practice respectful interventins regardless f gender, age, ethnicity, language, culture, sexual rientatin, financial status, spiritual, physical and mental abilities. Explre and include patient and family cultural beliefs, values, and needs in care prvisin. 4. Chrnic Disease Management and Self Management Educate and supprt patient and family abut chrnic disease management, delaying disease prgressin and maintaining a healthy lifestyle. Fster self management, engagement and mtivatin fr psitive utcmes such as imprved cnfidence, cmmitment t behaviur change, prblem slving capabilities, and self advcacy skills. Encurage participatin in cmmunity based self care prgrams. Cllabrate with cmplementary chrnic illness prgrams fr cmprehensive and integrated health care. Page 3 f 7

4 5. Advance Care Planning Educate and cunsel patient, family and healthcare team members regarding Advance Care Planning and Advance Medical Directives. Facilitate and identify patient s future health care values, wishes, decisins, prxy designate, legal and financial planning, and review fr change as needed. Crdinate Palliative Care and End f Life care services with patient, family, interdisciplinary teams, and cmmunity resurces, which hnur the patient s Advance Care Plan. 6. Educatin, Teaching and Leadership Develp and prvide individual r grup educatin fr patients, families, and cmmunity partners n such issues as cping, illness management, wellness, treatment, resurces, and ther renal r psychscial tpics. Develp educatin materials in the areas f renal care, scial wrk and interdisciplinary practice fr presentatin t health prfessinals, trainees, students, and cmmunity partners. Prvide mentrship, training, supervisin, and leadership t scial wrk students and staff and all ther multidisciplinary health prfessinals. 7. Data Management and Prgram Develpment Cllect data and submit service trends t apprpriate health care administratrs fr adequate scial wrk resurce allcatin, prgram develpment, plicy guidelines, and health infrastructure. Develp, implement, and evaluate prgrams fr safe, effective, efficient, and high quality patient care. Cllabrate with key stakehlders n building prgrams, which aim fr ptimal health utcmes and best clinical practice. Advcate fr adequate, cmprehensive and prgressive health resurces at lcal, reginal, prvincial, natinal, and internatinal levels. Page 4 f 7

5 8. Research Participate in the prmtin, implementatin, and evaluatin f renal health and scial wrk practice research. Develp and cnduct research fr the advancement f patient, family, cmmunity, and scial health. 9. Cmmunity Partnerships Build and fster nging strng wrking relatinships with Prvincial Renal Agency, Prvincial Health Authrities, gvernment agencies, and cmmunity partners such The Kidney Fundatin f Canada, ethnic rganizatins, cultural assciatins, and ther key grups and agencies. 10. Persnal and Prfessinal Develpment Pursue lifelng persnal and prfessinal learning and develpment. Stay current with clinical practice, theretical applicatins, scial changes, research, plicies, resurces and health infrmatin.
