Preference will be given t yuth pursuing animal related studies including pre-veterinary sciences. Special cnsideratin will be given t applicants with extensive participatin in the 4-H Dg Prject. One (1) $1000 Schlarship Awarded. If n eligible applicants selected, the schlarship will be carried ver t the fllwing 4-H prgram year. Page 3: "Why I Wuld Like t Have the Schlarship and What I Plan t D With It" "Why I Need This Schlarship" (Explain/describe financial need) Current 4-H hrse prject recrd bk. Outline summary f hrse activities utside 4-H (nt t exceed ne page, i.e., scuts, breed assciatins, hrse rganizatins). Hrseman f the Year Must have cmpleted at least junir year f high schl and have cmpleted at least tw years in the Hrse prject including current year. All applicants are limited t tw years f eligibility/participatin. Previus first place winners will be ineligible t cmpete. First and secnd runner s up and all ther applicants will be able t cmpete. The interview will be cnducted at the State 4-H Hrse Events. Additinal Applicatin Materials required (see yur Extensin Agent fr mre details). Osbrne 4-H Hrsemanship Schlarship 4-H members wh are participants f the Area B Hrse prgram including Alachua, Baker, Bradfrd, Clay, Clumbia, Dixie, Duval, Flagler, Gilchrist, Hamiltn, Jeffersn, Lafayette, Levy, Madisn, Nassau, Putnam, St. Jhns, Suwannee, Taylr, Unin Cunties. Award will be judged accrding t the criteria used fr the 4-H Hrseman f the Year award. Interested applicants shuld cmplete all requirements fr that award. 6 Prcedure fr Requesting Schlarship Payment Payment f schlarship funds is made directly t the accredited cllege r university r technical/trade schl n behalf f the recipient. In the event f special educatin needs, payment f schlarship awards can be directed t the recipient upn prir apprval f the Assciate Dean fr 4-H. Under these cnditins an IRS Frm 1099-MISC will be filed with the Internal Revenue Service and with yu fr the calendar year in which it is received. Winner must submit the fllwing t the State 4-H Headquarters: Cllege/schl name, cntact persn's name and address Enrllment verificatin Cpy f thank yu letter written t dnr Scial Security Number Student ID (if used fr registratin way at designated higher educatinal institutin) Cnditins Gverning State Schlarships A schlarship shall be used within ne (1) year f the date f the award. In the event that the schlarship is nt used within the year f limitatin, the winner must present in writing t the Assistant Dean fr 4-H a request fr time extensin.
