Examples: tomatoes, apples, potatoes, flowers, and plants. FOOD VENDOR- PROCESSED: Vendors who grow their own food products and perform processing before sale. Examples: jam, jelly, honey, maple syrup, bread, baked goods, and spices. This category also includes cut meat, eggs, and mushrooms. CRAFT VENDOR: Vendors who sell small, one-of-a-kind, non-mass produced crafts. Examples: jewelry, birdhouses, and hand-made wearable items. (please attach representative photos of the products you intend to sell) LANGUAGES SPOKEN (please let us know the languages you speak so we can let potential customers know we speak their language): Do you need translation or interpretation services for trainings or orientations? Yes No WIC, EBT and SNAP PROGRAMS The West Side Farmers Market is committed to fully serving all members of our community regardless of income. In order to qualify for WIC, the West Side Farmers Market needs to have a minimum of 5 certified vendors. For WIC, vendors are required to attend a WIC vendors meeting to receive a vendor number. For WIC, vendors can only sell at WIC certified Farmers Markets. Taking WIC coupons outside of the Farmers Market means loss of vendor number. In order to qualify for WIC, at least one WIC vendor must be present on market day. For EBT, there will be a centralized EBT terminal for use at the market. 4 ATTENDANCE DATE PLEASE SELECT THE DATES YOU WILL BE ATTENDING THE MARKET: JUNE: JULY: AUGUST: SEPTEMBER: OCTOBER: 3 10 VENDOR FEES Vendor stalls will be sold in Full Season or Half Season packages. A Full Season Vendor expects to commit to a minimum of 17 weeks out of the 19 week season. A Half Season Vendor expects to commit to a minimum of 9 weeks. Additional weeks or individual weeks may be added on throughout the season, as desired. The initial packages must be paid in advance. Please place a checkmark in your selection. A Full Season package includes an assigned stall and a Half Season includes as permanent a stall as possible. Stalls will be assigned based on market seniority. Craft stalls are assigned weekly. FULL SEASON: $100 HALF SEASON: $65 SINGLE WEEK (only for those who want more than a half season but less than full): $7. For craft vendors there is an additional fee based on 10% of daily sales INSURANCE Do you have your own insurance? YES NO. Contact the Market Manager for more information. CERTIFICATION of INSURANCE and REQUIRED PERMITS OR 4

5 LICENSES: A certificate of liability insurance and any/all permits or licenses required from the State of Minnesota or the City of St. Paul must be on file with the West Side Farmers Market prior to selling at the West Side Farmers Market. A copy of your insurance policy does not meet this requirement. COMPLIANCE AGREEMENT: I commit to attending the West Side Farmers Market vendor training. WSFM s rules and regulations, city permits and licensing will be covered. I certify that I have received a copy of the West Side Farmers Market Vendor Guidelines, and agree to abide by the rules contained therein, along with all other Federal, State and local regulations that apply. I certify that I will carry the insurance as required by the West Side Farmers Market Vendor Guidelines, including product liability insurance and workers compensation insurance, if required. I certify that the information I have provided above is true and accurate and to the best of my knowledge, and that I will promptly notify the West Side Farmers Market of any changes thereto.
