2 Liverpool Community Health NHS Trust Overview Who we are Liverpool Community Health NHS Trust is at the heart of the delivery of healthcare within the communities of Liverpool and Sefton reaching a population of approximately 750,000 people. Our staff and services come into contact with hundreds of thousands of people in our local communities every year and therefore are often involved not just with patients, but also their families, carers and the communities in which they live. This means our staff can help to understand the holistic health and social needs, including education, prevention, early detection and self-care Liverpool Community Health have a team of around 3,500 staff delivering 60 health services, from over 100 community locations and within patients own homes. Our services enable people to be cared for in their own community and remain as independent as possible, rather than having to go into hospital. Many of these services are delivered out-of-hours, with 24 hour, 7 day a week access. We work with a range of partners to help promote and develop public health and ensure we can provide high quality care and make a real difference to people s lives Despite the fact that Liverpool has been one of the UK s fastest growing cities, with a significant growth in the local economy of 72.9% between , Liverpool remains one of the most deprived cities in Europe, which has impacted directly on the health needs of the local population. Across Merseyside, deaths from heart disease and cancer are still significantly higher than the national average. Liverpool has the second worst life expectancy in England for males and the lowest life expectancy in England for females. Almost 25% of residents in Sefton are over 60 years old and this is expected to increase significantly over the forthcoming years. Liverpool Community Health want to transform the provision of community health care in the area, so that we can really focus on delivering the quality care that the local population need, but also help prevent many instances of poor health in the first place. 6 Foundation Trust We are currently in the process of becoming one of the first Community Foundation Trusts (CFT). We aim to do this in CFT s are still part of the NHS and as a CFT we will continue to provide the same high level of care and advice to our communities, free to all and based on what our patients and communities need. CFT's are public benefit corporations, they are not governed centrally like NHS Trusts are but they still have to meet the same high standards of care and quality that NHS trusts do. We will be responsible for our own management, and will have more flexibility to invest our resources into developing services and new practices to benefit local people. What CFT will mean for our Communities As a CFT we will be able to develop and fund services to meet the needs of the community. Most importantly, we will be accountable to the people and communities we serve, giving them the chance to have a say in how we meet their health needs. To make sure we are accountable to local people, our governance arrangements will change. We will have a membership made up of members of the public and our staff. Through our membership people from local communities, members of staff, patients and partners can have a much bigger say in the health services we run. From our membership, Governors will be elected to the Council of Governors. This Council will have an important role in representing the membership to the Board of Directors, providing feedback on the views and opinions of the members on how we run our services and on our plans for the future. You will act on behalf of the Director of Operations/Executive Nurse in any delegated responsibility. Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes. Responsible for compiling business plans, developing strategy and ensuring implementation and that performance targets, strategic objectives are met. Responsible for proposing & implementing service/departmental policies/ responsible for policy implementation and service development across the operational divisions. Principal Responsibilities Operational Leadership Line manage the divisional managers within operations Contribute to the meeting the strategic objectives of LCH by driving the performance of the Operations Directorate across all 4 Divisions Lead on performance and contract management of clinical services Leading change to transform clinical services to be meet the demands of services users and all partners Create a cohesive team with the Operations Directorate Page 8
