1 Corporate Resources Legal Services Civic Government (Scotland) Act 1982 Application pack to substitute a vehicle for that designated as a taxi licence or a private-hire car licence This application pack should be used to apply to change your vehicle for a taxi or private-hire car operator s licence. You can complete this licence application form in Adobe Acrobat and print it out, or print it and complete it by hand, using block capital letters. Return the form to your local Licensing and Registration office at the addresses listed on the front page. For more information or if you want this information in a different format or language, please phone or licensing@southlanarkshire.gov.uk Before completing this application form, please make sure you have read the notes on this page. 1 When you are applying to the Licensing and Registration Section you should include the following the completed application form at least two working days notice is required to process this application although during renewal periods this may be extended to three working days. the application fee (a list of fees is available from our offices) the vehicle registration document from DVLA (V5) even if it is not in your name. This is not essential however if the registration number is completed incorrectly on the form you will be charged the full substitute vehicle fee again. valid insurance to operate the vehicle as a taxi or private hire car 2 All vehicles registered as taxis or private-hire vehicles must be less than five years old and can only be kept on as a licensed taxi or private-hire car until they reach 10 years old. All vehicles between five and 10 years old will have to be inspected every year (and receive one random inspection every year). The date is taken from the date of first registration (this can be found on the V5 form). 3 Once you have applied to substitute your vehicle you should telephone ( ) to arrange your vehicle test. 4 If you, or any person who is named in your application, give any statement which is not true, you will be guilty of an offence and could be fined up to 500. Convictions may affect the outcome of your application. 5 Please remember that we will not refund the application fee if you withdraw your application for any reason. 6 We will send a copy of this application to Strathclyde Police for their information. 7 We will not licence vehicles with tinted windows as taxis or private-hire cars.

2 8 After the vehicle has been tested You will require to return to your local office or Fleet Services office. Old rear plate Old fares card (if applicable) Old taxi or private-hire car licence Old door stickers Old window stickers Inspection forms given to you by the mechanic to say your vehicle has passed the test Valid insurance to operate as a taxi/private hire car operator your new plate and licence cannot be issued until your insurance is produced. Once your plate has been issued, you will be given your plate, door stickers and window stickers. You will not be issued with your licence until a member of our staff has checked that your plate has been securely fastened to your vehicle and that the appropriate door and window stickers have been displayed. The information may be passed to the Department for Work and Pensions, Employment Service and Her Majesty s Service as permitted by law. We may check the information provided by you, or information about you provided by a third party, with other information held by us. We may also get information from certain third parties, or give information to them to check the accuracy of information, to prevent or detect crime, or to protect public funds in other ways, as permitted by law. These third parties include Government Departments and local authorities. We will place the details you give us in application forms in a public register that will be available in our offices and on our website. Apart from the information on the application forms, we will not give information about you to anyone outside South Lanarkshire Council and we will not use information about you for other purposes unless the law allows us to.
