2 PROFILE OF A TEAM MEMBER: A North Adams-Jerome High School, Robot Commander 4568, and robotics team member is: A team player Highly self-motivated; able to see and follow up on opportunities to help Hard-working (without ever being asked) Willing to learn, open to new ideas, asks questions for understanding Is able to receive and respond to constructive criticism Willing to sacrifice other activities for the team. (E.g. Saturdays, evenings, sports, social events, etc.) Required to keep your grade for each class above 70% Punctual Willing to exhibit spirit for the team and HAVE FUN REQUIREMENTS OF TEAM MEMBERS: Must attend a fundraising event (TBD) Must participate in at least one of the following o FLL and/or FTC mentoring o Heritage Day Parade o Hillsdale County Fair Parade o North Adams-Jerome High School Fall Homecoming Parade/Game o North Adams-Jerome High School Winter Homecoming Game If a student signs up for an off-season event and fails to show up they will be removed from the team. Attend work sessions in the fall, starting in early September Attend activities afterschool and on Saturdays during the competition season (January Mid April) Attend competitions during the season (usually two in-state competitions on Friday/Saturday). Absences from school will be excused. Classwork must still be completed. If we qualify, you will be invited to attend the Michigan State Championship and World Championships (mid-april) held at the Delta-Plex in Grand Rapids and St. Louis, MO (late April). Failure to attend fall work sessions WILL result in removal from the team unless the teacher/mentor is notified AT LEAST 24 hrs. prior to the training/work session.
3 ROBOT COMMANDER 4568 STUDENT/PARENT INFORMATION ADDITIONAL INFORMATION Attendance at after school activities and Saturday events is required for team members. Attendance will be tracked. Excessive absences will result in the student not being allowed to attend a competition or dismissal from the team. Students may be removed from the team by consensus of the mentors and teacher for lack of participation or behavior that is not appropriate. During meetings and competitions students will follow rules of conduct of behavior and all procedures as required by the Student Code of Conduct. Violation will result in disciplinary measures as decided by the mentors/teachers. If at a distant site, the students may be sent home at the expense of the parents. Safety is paramount. All students will follow the directives of the mentors/teachers and safety leaders at all times. Questions regarding these directives are to be done in a private conversation with the mentor/coach. Any team member receiving a referral or suspension from school for reasons not related to the team may be subject to a review for possible dismissal from the team. Students must maintain acceptable grades in order to travel and/or participate with the team. Team members will be required to provide regular grade updates throughout the season. Failure to submit grade sheets as required will make the student ineligible to travel with the team to the next competition. Class grades must be maintained at or above expected levels. Student grades take priority over robotics. No individual class grade may be below 70%. Students whose grades fall below 70% will meet with the teacher/mentor to discuss a plan of corrective action; failure to improve will result in removal from the team. After all, school is more important. Transportation for after school activities is to be provided by the parent or students. Regional, state and national event transportation will be arranged by the team. Students are NOT allowed to drive to off campus events. Electronic devices (games, ipods, tablets, etc.) other than cell phones are not allowed at team meetings and competitions.

If you are chosen to be a member of Robot Commander 4568, you are required to attend fall team meetings (TBA). Submission of this application is a commitment to do so. MANDATORY orientation meeting for students and their parents, Wednesday, September 14, 2016 at 7:00 pm in Mr. Voisin s room. What is to be turned in by June 8, 2016? 7 ROBOT COMMANDER 4568 PARENT SECTION FIRST Robotics, Robot Commander 4568, requires significant parental support throughout the season to be successful. There are a variety of ways parents can help from fund raising, chaperoning, coordinating/providing lunches, etc. Involvement: I as a parent will bring to the team (check as many as apply. Some volunteer assignments will be based on availability and prior experiences.) o o o o Scheduling and coordinating food (a few hours and flexible scheduling) o Coordinate procurement of food (using team funds) o Coordinate parent provided meals and snacks Transportation/chaperoning (a few hours and flexible scheduling) o Give rides to and from team events and competitions o Stay for the duration of team events and competitions to provide supervision o Travel with the team for overnight competitions o I cannot travel with the team but will help coordinate travel arrangements Fund Raising (a few hours and flexible scheduling) o Develop new and innovative ways to provide fund raising opportunities o Coordinate fund raising and student involvement o Provide student transportation during selected fund raising events General build season help (January early April, can be a significant amount of time) o I will drop in occasionally o I will work with the team each week: Day How Long Please Note: We are in great need of additional parent mentors and helpers in all areas. New parents need to begin mentoring so they can take over as experienced mentors retire. It is a lot of work but is also a very fun and tremendously rewarding experience. Unfortunately, the team will cease to exist without a solid rotation of committed parent volunteers. I have expertise in and will assist with: Programming Train and supervise in the art and science of programming. Training tools are available on the US FIRST website. Programming tools used includes Labview. CAD/Animation Train and supervise students in application of animation and computer aided design programs to develop component drawings, animations, and conceptual artwork. Tools include AutoDesk Inventor and Autodesk 3DS. Electrical Train and supervise students in proper assembly and wiring of electronics and electrical components, and design circuits as required.
8 Mechanical Train and supervise students in component build including metal work and machining; drive train development and build; maintaining a clean and orderly shop environment. Carpentry Train and supervise students in the building of a practice field; use of wood shop tools (power and hand); building robot crate to specifications, maintaining a clean and orderly shop environment. Communications/Public Relations Train and supervise students in developing a comprehensive program communication plan including designing and creating team buttons, banners, flyers, competition signage, information brochures, etc. Also lead students in the development of FIRST program award applications. Travel Coordination working with adult members to arrange transportation, lodging and meals associated with team competitions; developing and maintain associated budget; negotiating with service providers for group/student discounts. Sponsor Opportunities Identify and solicit sponsorship support from the community including civic and private organizations. If my son/daughter is selected for the North Adams-Jerome High School FIRST Team, Robot Commander 4568, I understand that I am responsible for: o Transportation to and from school (or Meeting place) for my son/daughter o I also acknowledge there may be other expenses incurred with travel associated with participation on Robot Commander o Actively supporting additional Robot Commander 4568 team fundraising events. o Providing some of the food or associated materials (paper ware, plastic ware) for at least one meal for the team during the build season. o Assisting in transportation and chaperoning (for competitions and/or practices) o Picking my child up on time at the end of each meeting or event. 9 TEACHER RECOMMENDATION Participation on the Robot Commander 4568 team requires significant dedication by the student. This application should only be submitted by students who will be committed to the program. Students are to obtain a recommendation from a teacher to whom they have demonstrated their ability to remain committed to a task. A teacher is to complete this form and return it according to the instructions below. Middle school teachers are acceptable for incoming freshmen.
