4 PART 3: TERMS AND CONDITIONS OF REGISTRATION I. Registration Entitlement 1. Upon payment of registration fee, delegates will gain access to the GTF Forum, GTF Exhibition, BFR Workshop (if applicable) and all related materials provided by The Organiser. 2. Fees for accommodation, flights and tours are NOT included in the registration fees. II. Registration Process and Payment 1. For University Lecturers and Students to complete the registration, a letter or any documentary proof from the relevant department or learning institution should be submitted to confirm the Lecturer or Student status. The document should be sent to the GTF 2015 Secretariat via at or fax to (603) The Organiser reserves the right, and will inform Registrants on any alteration to any of the arrangements for the GTF 2015 including the cancellation of the event should unforeseen circumstances require such action. In the special circumstances that the entire event has to be cancelled, the Organiser will refund the registration fee to the Registrants. However, the Organiser will not be liable for other associated costs, e.g. accommodation and travelling expenses, and any inconveniences incurred to the Registrants due to the cancellation. All cancellation of registration must be done in writing and submitted to the GTF 2015 Secretariat via at or fax to (603) Any cancellations received on or before 15 September 2015 will be charged a handling fee of MYR 105 (USD 30). The balance of the registration fees will be refunded after deducting other related fees and charges incurred in connection with such refunds, including costs of foreign exchange conversion, Credit Card cancellation and/or banking transaction. Any cancellations after 15 September 2015 will not be refunded. 3. All refunds will be made in MYR at the prevailing foreign exchange rate. 4. Refund for cancellations will be made within three months after the confirmation of cancellation by the GTF 2015 Secretariat. Any charges imposed by banks and Credit Card issuers will be borne by the Registrants. V. Liability 1. The Organiser reserves the right to make changes where deemed necessary, with or without prior notice to parties concerned. All disputes are subjected to the Malaysian Law. 5 PART 4: PERSONAL DATA NOTICE / NOTIS DATA PERIBADI 1. Purpose of notice The Organisers of GTF 2015 i.e. Global Transformation Forum, BFR Institute Sdn. Bhd., a subsidiary of Performance Management and Delivery Unit, Prime Minister s Department, Government of Malaysia and the GTF 2015 Secretariat ( Organisers or we ) are required to comply with the Personal Data Protection Act 2010 (the Act ), which regulates the processing of personal data in commercial transactions. For the purpose of this Personal Data Notice, the terms personal data and processing shall have the meaning as prescribed in the Act. 1. Untuk tujuan Notis Data Peribadi ini, terma-terma data peribadi dan pemprosesan akan mempunyai maksud sepertimana ditakrifkan dalam Akta. 2. Processing of personal data The personal data processed by us may include, but is not limited to, your name, IC number, address and other contact details. 2. Pemprosesan data peribadi Data peribadi anda yang diproses oleh kami termasuk, tetapi tidak terhad kepada, nama, nombor kad pengenalan, alamat dan maklumat hubungan lain. 3. Purpose of processing personal data The personal data is collected for, amongst others, the following purposes: (a) to process your registration form for training events (including conferences) organized by GTF 2015 Secretariat and collaborative partner(s) for administrative purpose such as attendance list, issuance of invoice, official receipt and certificate of attendance; (b) to communicate with you on the confirmation of the events and matters in connection to GTF 2015 Secretariat, events and Forum; (c) to send out for our future conferences or workshops which may be related to you; (d) to mail the official receipt and certificate of attendance; (e) to respond your enquiries or complaints; and (f) other administrative purposes which relate to the above. 3. Tujuan memproses data peribadi Data peribadi yang diambil adalah bagi, antara lain, untuk tujuan berikut: (a) untuk memproses borang pendaftaran program latihan (termasuk persidangan) yang dianjurkan oleh Sekretariat GTF 2015 dan rakan kerjasamanya untuk tujuan pentadbiran seperti senarai kehadiran, pengeluaran invois, resit rasmi dan sijil kehadiran; (b) untuk berkomunikasi dengan anda mengenai pengesahan acara ini dan sebarang perkara yang berkaitan dengan Sekretariat GTF 2015, acara dan persidangan; (c) untuk menghantar emel bagi persidangan atau bengkel akan datang yang mungkin melibatkan anda; (d) untuk penghantaran resit rasmi dan juga sijil kehadiran melalui pos; (e) untuk memberi maklum balas kepada apa-apa pertanyaan atau pengaduan; (f) apa-apa tujuan pentadbiran lain yang berkaitan dengan perkara di atas. 4. Disclosure of personal data The personal data held by us shall be kept confidential. However, we may disclose your personal data to: (a) Co-organisers within the GTF 2015 Secretariat; (b) To our collaborative partners (in terms of name for issuance of certificates of attendance under paragraph 3(a) above); (c) Other parties authorised by you; (d) Regulatory and governmental agencies as permitted or required by law, authorised by any order of court or to meet obligations to regulatory authorities. 4. If we disclose any of your personal data to any entities, we will require them to appropriately safeguard the personal data provided to them. 5.
