1 子 宮 內 投 藥 系 統 臨 床 指 引 台 灣 婦 產 科 醫 學 會 編 制 子 宮 內 投 藥 系 統 ( 蜜 蕊 娜 ; 簡 稱 IUS) 療 效 長 達 5 年,5 年 內 黃 體 素 levonorgestrel 的 平 均 釋 放 速 率 約 為 4μg/24hrs 衛 生 署 核 准 用 於 :. 避 孕 : 每 年 避 孕 效 果 為 99.8%, 其 5 年 內 所 累 積 的 避 孕 效 果 約 為 99.3%,2,3 2. 月 經 經 血 過 多,4,5,6, 3. 預 防 雌 激 素 補 充 治 療 引 起 的 子 宮 內 膜 增 生,7,8 臨 床 上 子 宮 內 投 藥 系 統 亦 用 於 改 善 下 述 疾 病 或 徵 狀 9,0,,2,3,4, 包 括 :. 經 痛 (Dysmenorrhea) 5,6,7,8,9 2. 經 血 過 多 所 導 致 之 貧 血 (Anemia due to menorrhagia) 20,2 3. 經 前 症 候 群 (Premenstrual Syndrome; PMS) 5 4. 子 宮 肌 瘤 (Fibroids) 22,23,24 5. 子 宮 腺 肌 症 (Adenomyosis) 25,26,27,28,29,30 6. 子 宮 內 膜 異 位 症 (Endometriosis) 6,3,32,33,34,35,36,37,38 7. 慢 性 骨 盆 腔 疼 痛 (Chronic Pelvic Pain) 6,38,39 8. 子 宮 內 膜 增 生 (Endometrial Hyperplasia) 40,4 適 合 安 裝 子 宮 內 投 藥 系 統 之 對 象. 考 慮 長 期 避 孕 的 女 性,42 2. 目 前 想 避 孕 但 隨 時 考 慮 懷 孕 的 女 性,42,43 3. 產 後 欲 哺 乳 且 有 避 孕 需 求 的 女 性,42,44,45 4. 尋 求 可 靠 不 需 每 日 提 醒 之 避 孕 方 式 的 女 性 有 經 血 過 多 且 使 用 藥 物 效 果 不 理 想, 期 望 擁 有 手 術 以 外 的 治 療 選 項 之 女 性 46

2 6. 有 經 痛 困 擾 之 女 性 5 7. 接 受 荷 爾 蒙 治 療 而 需 併 用 黃 體 素 以 預 防 子 宮 內 膜 增 生 之 女 性 那 些 人 不 可 以 或 不 建 議 安 裝 子 宮 內 投 藥 系 統?. 已 懷 孕 與 有 懷 孕 可 能 2. 患 有 婦 科 感 染 或 發 炎 性 疾 病 3. 先 天 或 後 天 子 宮 異 常, 包 括 造 成 子 宮 腔 變 形 之 子 宮 肌 瘤 與 子 宮 或 子 宮 頸 相 關 之 惡 性 腫 瘤 4. 對 子 宮 內 投 藥 系 統 活 性 成 份 過 敏 者 5. 急 性 肝 病 與 肝 腫 瘤 6. 不 明 原 因 的 子 宮 出 血 7. 與 黃 體 素 相 關 之 腫 瘤 ( 含 乳 癌 ) 子 宮 內 投 藥 系 統 的 預 期 效 益 與 對 授 乳 的 影 響. 預 期 之 避 孕 效 果 : 使 用 第 一 年 的 避 孕 效 果 達 99.8%,2 2. 使 用 三 個 月 後 降 低 經 血 流 失 的 效 果 : 8.6%; 六 個 月 後 : 88.0%; 一 年 後 :95.8% 以 子 宮 內 投 藥 系 統 治 療 經 血 過 多,80% 的 病 患 感 受 到 減 緩 經 痛 的 效 果 5 4. 用 於 經 血 過 多 治 療, 較 子 宮 切 除 術 (hysterectomy) 具 有 較 高 的 成 本 效 益 (cost-effectiveness) 產 後 六 週 後 使 用 子 宮 內 投 藥 系 統 對 哺 乳 後 嬰 兒 的 生 長 發 育 及 乳 汁 分 泌 的 質 與 量 並 無 不 良 影 響,42,43 子 宮 內 投 藥 系 統 安 裝, 取 出, 與 更 換 須 知 裝 入 時 機 49. 育 齡 期 的 婦 女, 置 入 之 最 佳 時 機 為 月 經 開 始 的 七 天 內 2. 懷 孕 3 個 月 內 的 流 產 後 可 立 刻 裝 入 產 後 六 週, 若 子 宮 已 復 原 即 可 安 裝 4. 先 前 使 用 其 他 避 孕 方 式 的 女 性 欲 轉 換 為 子 宮 內 投 藥 系 統, 確 認 無 懷 孕 之 虞, 可 立 即 安 裝, 若 月 經 來 潮 七 天 後 才 安 裝, 則 建 議 再 採 取 其 他 的 避 孕 措 施 5. 停 經 婦 女 用 以 保 護 接 受 雌 激 素 補 充 治 療 中 的 子 宮 內 膜 時, 可 隨 時 裝 入, 也 可 在 月 經 來 潮 或 消 褪 性 出 血 的 最 後 幾 天 裝 入 取 出 與 更 換 2

3 . 欲 更 換 新 的 子 宮 內 投 藥 系 統 可 在 月 經 週 期 的 任 何 時 間 進 行 2. 須 在 裝 入 5 年 後 移 除 如 果 使 用 者 希 望 繼 續 使 用 本 避 孕 方 法, 可 在 取 出 同 時 置 入 新 的 子 宮 內 投 藥 系 統 臨 床 衛 教 建 議 內 容. 裝 入 子 宮 內 投 藥 系 統 前, 宜 告 知 使 用 婦 女 有 關 子 宮 內 投 藥 系 統 療 效 風 險 性 和 副 作 用 的 資 訊 2. 裝 置 後, 若 月 經 間 隔 超 過 六 周, 需 考 量 有 懷 孕 的 可 能 3. 懷 孕 時 若 子 宮 內 投 藥 系 統 未 取 出, 應 考 慮 胎 兒 可 能 發 生 男 性 化 或 其 他 畸 胎 的 可 能 4. 育 齡 期 的 婦 女 使 用 子 宮 內 投 藥 系 統 後, 約 有 20% 會 逐 漸 出 現 月 經 量 少 或 無 月 經 的 現 象 無 月 經 現 象 為 子 宮 內 膜 變 薄 之 故, 而 非 不 良 反 應 或 更 年 期 提 早 5. 置 入 初 期, 有 些 婦 女 在 正 常 月 經 週 期 外, 會 發 生 少 量 之 間 歇 性 出 血, 此 間 歇 性 出 血 隨 時 間 逐 漸 緩 解, 而 恢 復 正 常 週 期 因 此, 第 一 個 星 期, 建 議 應 使 用 衛 生 棉 6. 使 用 子 宮 內 投 藥 系 統 的 婦 女 應 在 置 入 後 4-2 週 再 作 一 次 檢 查, 其 後 每 年 檢 查 一 次, 如 出 現 任 何 臨 床 症 狀 可 增 加 檢 查 次 數 7. 如 安 裝 子 宮 內 投 藥 系 統 後 有 過 敏 反 應 時, 宜 向 衛 生 主 管 機 關 報 告 使 用 子 宮 內 投 藥 系 統 相 關 問 題 諮 詢 與 反 應 窗 口 衛 生 署 藥 物 不 良 反 應 通 報 中 心 地 址 :00 台 北 市 中 正 區 羅 斯 福 路 一 段 32 號 2 樓 電 話 : 傳 真 : adr@doh.gov.tw 網 站 地 址 : 適 應 症 請 依 衛 生 署 公 告 核 准 事 項 為 準 References: Bayer.
