3 1 ASSET DECLARATION SYSTEM ONLINE 1.1 Online Online allows users to submit their asset declaration online. The details are described as below. Figure 4- Online-1: Main Page Open Online Screen 1 Login to module. Print Form 1 Click on Print icon 1
4 Print Acceptance Letter 1 Click on Print icon 1.2 Form A Submission Form A submission is for Declaration of no difference on asset owned. Staff has to make Declaration upon Appointment, Schedular Declaration and Declaration upon End of Service. The details are described as below. Figure 4-Form A-1: Main Page Submit Form A 1 Click on Fill in Declaration Form button. System will display Form Selection page [Figure 4-Form A-1]. 2 Select Form = A Declaration of no difference on asset owned from dropdown list. 3 Select the desired Declaration Type from dropdown list. 4 Click on Continue button. System will display Staff s Declaration page [Figure 4- Form A-2] 5 Click on Submit button. System will display Confirmation page [Figure 4-Form A- 3] 6 Click on View form to be submitted link to view asset declaration form in PDF format. 7 Click on Confirm button. 6 1.3 Form B Form B submission is for. Staff has to make Declaration upon Appointment, Additional, Schedular Declaration and Declaration upon End of Service. The details are described as below. Figure 4-Form B-1: Form Selection Page Enter Form B Info 1 Click on Fill in Declaration Form button. System will display Form Selection page [Figure 4-Form B-1]. 2 Select Form = B from dropdown list. 3 Select the desired Declaration Type from dropdown list. 4 Click on Continue button. System will display Form B page Family tab [Figure 4-Form B-2]. 5 If staff s spouse is employed by IIUM, spouse s employment data will be displayed. For staff s spouse employed by other employer, staff needs to fill in spouse s employment information. 4

Owner Relationship with Staff: Type in relationship iv. Asset Address/ Registration No/ Certificate No/etc: Type in asset address/registration number/certificate number/etc v. Asset Acquisition Date: Type in date vi. Amount/Acres/ Square Feet/Unit (If house, please state the land measurement: Type in amount/acres/ square feet/unit vii. Value during Asset Acquisition: Type in asset value viii. Estimated Current Value: Type in current value ix. Source of Acquisition: Type in source x. Other Information (If Applicable): Type in info 18 Click on Add button. System will return to Liability Tab Main Page [Figure 4-Form B-5] displaying the newly added information. 19 Click on Add Loan button to add loan information. System will return to Add Monthly Liability page [Figure 4- Form B-8] displaying the newly added information in Financial Sources to Acquire Asset field. 22 Click on Add Disposed Source button to add income from asset disposal information. Edit Loan Information 1 Click on Edit button. 2 Edit the desired information. 3 Click on Add button. Edit Income from Disposed Source 1 Click on Edit button. 2 Edit the desired information. 3 Click on Add button. Save Asset Information 1 Click on Save button. Date of Disposal: Click on calendar icon to select date ii. Sale Price: Type in amount iii. Method of Disposal: Type in method 4 Click on Add button. System will display Information of Asset Disposal page [Figure 4-Form C-3] displaying the added info. Edit Disposed Asset Information 1 Click on Edit button. System will display Select Asset to Dispose page [Figure 4- Form C-3] 2 Edit the desired information. 3 Click on Add button. Delete Disposed Asset Information: 1 Select the desired disposed asset and click on Delete button. Save Asset Information 1 Click on Save button. System should display saved message page [Figure 4- Form C-4] message: Saved! Your declaration has been saved for later editing. 15

System will display Form Selection page [Figure 4-Form D-1]. 2 Select Form = D Company/Business Declaration from dropdown list. 3 Select the desired Declaration Type from dropdown list. 4 Click on Continue button. Relationship: Select relation from dropdown list iii. Position in the Company: Select position from dropdown list iv. Number of Share Held: Type in share amount v. Share Value: Type in share value 12 Click on Add button. System will save the information and display Financial tab page [Figure 4-Form D-3] displaying the added info. 25 1.6 Form G Form G submission is for Application for permission to apply and own shares/government land. The details are described as below. Figure 4-Form G-1: Form Selection Page Enter Form G Info 1 Click on Fill in Declaration Form button. System will display Form Selection page [Figure 4-Form G-1]. 2 Select Form = G Application for permission to apply and own shares/government land from dropdown list. 3 Select the desired Declaration Type from dropdown list. 4 Click on Continue button. System will display : Form G page [Figure 4-Form G-2]. 5 If staff s spouse is employed by IIUM, spouse s employment data will be displayed. For staff s spouse employed by other employer, staff needs to fill in spouse s employment information. 23
