Contemporary RN Case Manager Certificate Program April 14-16, 2015 Metropolitan Chicago Healthcare Council 222 South Riverside Plaza (SE Corner of Canal & Adams Streets) 19th Floor Conference Center PROGRAM OVERVIEW The Contemporary RN Case Manager Certificate Program is designed specifically to address the educational needs of registered nurses who are expanding their practices to include the essential roles of case management and care coordination within a dynamic healthcare system. This comprehensive two-and-a-half day training program will provide both new and experienced hospital RN case managers, discharge nurses, and nurse leaders with the knowledge needed to develop and implement practice change, practice to potential, and sustain a positive vision for the future during this period of rapid change/transition. The first day will focus on nurse case management scope and standards of practice, health policy, including evolving health care delivery systems, and health information technology in care coordination. On the second day, expert faculty will turn to patient-centered nurse case management, health literacy, health promotion and patient engagement, and collaborative team-based leadership concepts. The course will conclude with an overview of evidence-based strategies for effective home visiting, medication reconciliation, nursing assessment, diagnosis and care planning, and telephone case management. A small student-to-teacher ratio in a classroom setting will allow for individualized guidance, dialogue, and consultation throughout the program. Following the face-to-face session participants will be given access to the case-based Acute and Transitional Care Coordination supplemental online learning module, which addresses the RN case manager role in acute, transitional, and accountable care organization settings. Attendees who successfully complete both the face-to-face program and online learning module will be eligible to receive 27 nursing CEUs. TOPICS COVERED This program is consistent with the Affordable Care Act's requirements for system-wide transition to a patient-centered, coordinated, safe, cost-effective, and efficient health care delivery system; the Institute of Medicine s Future of Nursing Report; and national nursing and case management professional associations and certifying bodies. It addresses the following topics: Role of the registered nurse as contemporary case manager and care coordinator. Case management principles, strategies, concepts, ethics, and care coordination practice models. Healthcare systems management, quality and safety, collaboration and delivery. Evidence, ethics, health policy, and emerging opportunities and trends. Implementation strategies for nurse case managers in various practice settings.

WHO SHOULD PARTICIPATE This program is designed for both new and experienced hospital RN case managers/care coordinators and discharge/transitions nurses, advanced practice nurses in transitional care, and hospital case management and care coordination leadership. RN licensure is a requirement for attendance. FACULTY Kelly Kruse Nelles RN, APRN-BC, MS, and Mary Jo Borden RN, APRN-BC, CCM, MSN, co-directors, National RN Case Manager Training Center. With over 60 years combined expertise in nursing education and clinical practice, Mary Jo Borden and Kelly Kruse Nelles bring their experience to the education and training objectives of today s registered nurses and health care organizations who are seeking high quality, safe, effective, value-added care. Their mission is to provide cutting edge, evidence-based education and training that supports the transition of nurses from traditional to new and emerging case manager/care coordinator roles across care delivery settings. The evolution of a patient-centered, coordinated, safe, cost-effective, and efficient health care delivery system serves as the underpinnings for the context of the role and skill development curricula that they provide. CONTINUING EDUCATION UNITS This activity has been approved by the Wisconsin Nurses Association for approval to award 23 continuing education contact hours. The Wisconsin Nurses Association is accredited as an approver of continuing nursing education by the American Nurses Credentialing Center s Commission on Accreditation. (Provider Approval Numbers: 0561 001, 0561 002. CE is applicable to Illinois licensed RNs.) The Contemporary RN Case Manager Education Program, approved for 23 Continuing Education Units for the 2 ½ day face-to-face program, also includes three (3) homework assignments. Four (4) additional CEUs can be obtained by completion of a post-session supplemental online training module. PRESESSION ASSIGNMENTS This certificate education program includes preparatory homework that accompanies the face-to-face content. Each of the face-to-face sessions is preceded by a preparatory homework assignment. Each homework assignment is assigned two hours of continuing education credit, and completion of the assignments is required to best prepare the learner for the session content and to receive the full 23 CEUs offered for completing the face-to-face portion of the Contemporary RN Case Manager Certificate Program. DATES AND TIMES Registration and a continental breakfast will begin daily at 7:30 a.m. Lunch is included and will be provided onsite on April 14 and 15. LOCATION MCHC is located in the building that is bordered by Adams Street on the north, Jackson Street on the south, Canal Street on the west, and the Chicago River on the east. The main entrance to our building, Fifth Third Center, faces Adams Street. Ample public parking is available, with the closest lot being the Union Station Self-Park, 320 S. Canal, which has entrances on Canal and Clinton. The Council is adjacent to Union Station and directly over the tracks (from Union Station tracks, come up the escalator towards Canal Street, around the staircase to the lobby, and take the elevator to the 19th floor), two blocks south of the Northwestern Station (Metra Ogilvie Transportation Center), and easy access from CTA bus and L stops. Please note: The management of MCHC s headquarters building has instituted enhanced security measures for the safety of all staff and visitors. Please allow extra time prior to the scheduled start of the program to obtain a visitor s badge at the security desk in the lobby. REGISTRATION AND FEES Advanced registration and payment by check or credit card are required, and registrations will not be confirmed until payment is received. Please note that invoicing is not available for this program. This class is offered in a small classroom setting, and both minimum and maximum enrollments apply. Cancellation Policy Written notice of cancellation may be made via email to Helena Lefkow at hlefkow@mchc.com. A $50 service charge applies to any refund of registration fees. Cancellations will be accepted and refunds issued for requests received by February 27, 2015. After February 27, 2015, cancellations will be accepted and refunds issued only if there is a waiting list for the class and another paid registrant is available. Substitutions may be made at any time, and we appreciate being informed of any name changes in advance of the program. Nursing Demographic Information Registrants will be asked to provide background information on their education and case management experience through a brief online survey, for which a link will be emailed to participants prior to the start of the course.
