Our depth of industry experience includes the design and implementation of Accountable Care Organizations (ACO), population health-based outcomes, such as Patient-Centered Medical Homes (PCMH), and Shared Savings Models. In support of these risk-based arrangements, Geneia s model for care coordination has a proven track-record of delivering improvements in financial, quality, and health outcomes for our clients. Geneia s portfolio of products include: Theon, a health analytics platform that provides insights into care delivery performance and population health management strategies; Geneia@home, a telehealth solution which integrates patient data from multiple sources with a system for clinical alerts and deployment to better manage chronic and acute conditions; Clinical Care Solutions, which includes case and disease management and health and wellness services; and The Geneia Institute. The Geneia Institute: In recognition of a growing need to equip health professionals with the knowledge and skills required to effectively work in population health management, risk management, and other emerging care delivery models, Geneia introduced The Geneia Institute in June 2013 an educational initiative that capitalizes on the expertise of Geneia staff to deliver innovative and appropriate curriculum that propels the successful transition to new care delivery models for various health organizations. The Geneia Institute delivers customized educational programs designed to meet the needs of its clients with sessions offered inperson, online, or in combination. 2
3 Geneia utilizes an integrated team of professionals, from nursing, social work, public health, and business backgrounds, to ensure that programs are meeting the needs of the population, while improving upon Triple Aim objectives. The Geneia team is composed of approximately 200 individuals, including 100 nurses, physicians, social workers, and dietitians. All members of The Geneia Institute team hold either a graduate degree, advanced certification, and/or advanced training in the fields previously listed. 3
4 Evolving with the System: The Changing Role of the Medical Assistant in the Delivery of Team-Based Care Goal To prepare Medical Assistants to assume new roles and responsibilities to support implementation of population health delivery models in managing the health of the patient population and in meeting the goals of the Institute of Healthcare Improvement s Triple Aim increase patient satisfaction, improve quality, and control cost and utilization. Description The curriculum for this six (6) session course focuses on the following: Introduction to Population Health Management and Care Coordination Working to the Top of the License: Mobilizing Skills of All Care Team Members to Improve Satisfaction, Increase Quality, and Reduce Costs Care Coordination An Overview of Roles, Skills, and Functions The Role of Medical Assistant in Coordinating Care Across the Continuum Building Collaborative Relationships with Patients Motivational Interviewing Health Coaching and Education Outcomes Management Making a Difference The Effect of Patient Engagement on Improving Population Health Outcomes Health Literacy and Cultural Competency in Care Planning Educational Objectives By the end of the course, participants will be able to: 1. Identify the principles of Population Health Management and Care 4
5 Coordination. 2. Demonstrate familiarity with Population Care Coordination Models and their evolution. 3. List the roles and responsibilities of the Medical Assistant within new care delivery models. 4. Utilize population health tools and technology applicable to job responsibilities. 5. Incorporate core concepts of motivational interviewing and health coaching theory into daily work. 6. Practice patient engagement techniques to improve patient experience and health outcomes. 7. Apply care coordination methodology to achieve Triple Aim objectives. 8. Identify potential responses to current and future challenges associated with changing and shifting responsibilities of a Medical Assistant within the evolving healthcare landscape. Course Delivery The course is a mix of synchronous and asynchronous education. Participants engage in six (6) synchronous course sessions, which will run twice-a-week and feature a 90-minute classroom discussion and exercises. To prepare the students to participate in the discussion and exercises, students watch short instructional, asynchronous videos. Through the videos, students will learn the concepts, theories, strategies, and content related to population health management, health coaching, and patient engagement. On occasion, a brief homework assignment will be assigned to supplement classroom activities. In order to accommodate staffing issues, the course is offered in time slots each week that have been identified as feasible by the organization. Typical offerings include: early morning, lunch hour, and after the work-day. 5
6 Course Outline Instructor Session One Triple Aim, Population Health Management Medical Assistants, Coordination of Care and Share the Care Model Working to the Top of the Licensure Session Two Introduction to Health Coaching Health Promotion and Readiness to Change Patient Goal Setting Session Three Health Literacy Cultural Competency Session Four Motivational Interviewing Core Components Patient Engagement Session Five Health Coaching Basics- Starting the Conversation Impact on Behavioral Change Session Six Chronic Disease States- The impact on US Health Care Four primary chronic conditions (Basic understanding of risks, warning signs, self-management) Bobbie Ayers, RN is an Educational Consultant for Geneia. Her responsibilities include development, implementation and presentation of quality educational programs through The Geneia Institute. She serves as a clinical subject matter expert in areas such as population health management, care delivery models, quality outcomes requirements and population health management tools. Bobbie s educational background includes a Bachelor s of Science in Nursing from The Pennsylvania State University. She is a licensed Registered Nurse 6
7 (Commonwealth of Pennsylvania), holds a certification in Guided Care Nursing through the Institute for John s Hopkins Nursing, and a certified Chronic Care Professional through the Care Continuum Alliance. Prior to her role with Geneia, Bobbie served in a supervisor role, overseeing a team of Registered Nurse Care Coordinators responsible for providing high risk care management, transitions of care and chronic care management support to two Accountable Care Arrangements (one integrated health system and one independent physician group) for Capital BlueCross members. She also assisted in the development and implementation of a Patient Centered Medical Home (PCMH) in two Family Practice locations. She teaches The Geneia Institute s six session course, Evolving with the System: the Changing Role of the Medical Assistant in the Delivery of Team-Based Care. The course prepares the Medical Assistant to assume new roles to support the implementation of the PCMH through Population Health Management and Patient Engagement through Motivational Interviewing. She also serves on the curriculum development team for The Geneia Institute.
