Welcome to Christian Health Care Center... Christian Health Care Center (CHCC) has been fostering health, healing, and wellness in northern New Jersey for more than 100 years. Since its inception, CHCC has grown to offer the most complete continuum of senior-life, short-term rehab, and mental-health services available in the area, and serves as a vital resource not only for the people we serve on our campus, but also for the larger community. Senior Life There s no place like home. At Christian Health Care Center, we are known for the exceptionally broad range of services we offer seniors whether they lead active and independent lives, need a little assistance with daily activities, or are in need of 24-hour nursing care. In fact, CHCC offers the area s most complete continuum of services and lifestyle options on one campus, with a dedicated staff that includes full-time, on-staff specialists such as physicians, dietitians, clinical nurse specialists, geriatric nurse practitioners, chaplains, physiatrists, counselors, social workers, and physical, occupational, and speech therapists. The significant skills of our staff members are complemented by the use of advanced technology, including online video communication services and electronic medical records, without compromising the compassionate, loving attention you have come to expect from us. Senior Life Short-term Rehab Mental Health

Assisted Living The Longview Assisted Living Residence provides personalized care in a secure environment that supports and fosters each individual s maximum level of independence, while also providing needed assistance with activities of daily living, such as bathing, dressing, and medication reminders. Residents can choose from an extensive list of services and amenities, enabling them to get exactly the help they need to remain as independent as possible. Each resident enjoys an apartment as well as common areas that promote socialization and a sense of community. A Child Day Care Center, located on the lower level, provides opportunities for residents to enjoy intergenerational programs. Residents also have access to an on-site Wellness Center for physical, occupational, and speech therapy, and a professional suite for physician appointments. The Courtyard: A Caring Environment for People with Memory Impairment Loss of memory shouldn t mean loss of dignity, activity, or independence. The Courtyard at Longview is a specialized residence designed to meet the needs of those with early stages of dementia and memory loss. Geared toward fostering ability and independence, the Courtyard offers safety, comfort, dietary needs, and mobility. Special programs and facilities help reduce dementiarelated anxiety and enhance memory. 4

Skilled-nursing Care At Heritage Manor Nursing Home, we never forget that each person in our care is an individual. Here, our staff seek not only to meet physical needs, but also to acknowledge and enhance each individual s spiritual and emotional well-being. Heritage Manor offers three specialized areas of care: skilled-nursing care, memory-care support for residents with Alzheimer s disease and other forms of dementia, and Valley Hospice at Christian Health Care Center to assist those who need hospice care but for whom in-home care is not an option. Behavior Management Southgate is a special-care skilled-nursing unit that offers behavior management and care for adults who have behavioral disturbances associated with dementia and other illnesses. Behavior management modifies severe behavioral disturbances such as combative, disruptive, and disturbing conduct. Southgate s environment is stimulating and social, as well as helpful in decreasing socially inappropriate behavior. One of only four such facilities in the state, Southgate is part of Heritage Manor Nursing Home. Independent Living Evergreen Court and Hillcrest Residence Independent living at CHCC is designed to offer supportive senior living that fosters independence, privacy, and community connection. When you walk through the front entrance of Evergreen Court, you ll immediately sense the warm, welcoming atmosphere and you ll probably meet a smiling face or two. Here you ll find the cordial, home-like environment you want and the security you need. Short-term Rehab Mental Health 5

Hillcrest Residence independent living is a pre-assisted-living option created for seniors who are in good health and want to maintain their independence, but also find comfort in knowing that assistance is close by if needed. Residents and their families have peace of mind in knowing that other CHCC services, such as physical therapy, nutritional counseling, and adult day care programs, are also available. Adult Day Services With locations in Wyckoff and Wayne, Christian Health Care Adult Day Services is an active, medical program designed to help seniors maintain independence and improve their quality of life, while also offering a welcome respite for their caregivers. In our safe, friendly environment, participants can receive medical care, such as health monitoring and medication administration, personal-care services, nutrition services, pastoral care, and social services. In addition, planned activities and recreational programs, including day trips, educational talks, and themed events, encourage socialization and engagement. Transportation is provided throughout 50 townships in Bergen, Passaic, and Morris counties. 6

Short-term Rehab Getting you well, getting you home. When an injury, or illness, or surgery has you sidelined, CHCC s David F. Bolger Post-acute Care Unit can help you return to wellness and get back to the life you enjoy. Postacute care better known as short-term rehab is care that is received after a hospitalization, injury, or illness with the goal of regaining strength and transitioning back home as healthy as you were before or better. Short-Term Short-term Rehab Mental Health

Our highly skilled, interdisciplinary team of professionals is dedicated to your successful recovery and uses stateof-the-art, innovative approaches to help you achieve the best outcome. We offer physical, occupational, and speech therapies, as well as education, to help you work toward restoring the function that has been compromised by surgery, illness, or injury. You may benefit from short-term rehab after being hospitalized for: joint-replacement surgery; heart surgery; complex medical conditions, such as diabetes or renal failure; or illnesses, such as cardiac disease, gastrointestinal illnesses, renal disease, or pneumonia. 8

Mental Health You are not alone. If you or someone you love is struggling with mental illness, we want you to know that we are here to help. Whether it s depression brought on by life circumstances or a more involved diagnosis, you don t have to go through it alone. CHCC offers specialized programs to care for people of any age, with a wide range of diagnoses. We believe in the value and dignity of all people and seek to meet them where they are, with a team of highly trained, dedicated professionals who provide loving, compassionate treatment and care. Mental Health

Christian Health Care Counseling Center Daily life can be stressful. Relationships, health, employment issues all these and more can make it difficult to face the day. At Christian Health Care Counseling Center, we help clients of all ages learn the coping skills they need to function in all areas of the community. We offer a wide range of outpatient programs, including individual and family therapy sessions and groups. We also offer a large library of educational resource materials. Pathways There is no one-size-fits-all path to mental health, and specialized programs are especially important when a person with a mental illness also has an intellectual-developmental disability. Pathways was established to provide mental-health services for intellectually/developmentally disabled adults (18 and older) and their families, and is the first program of its kind in New Jersey. Ramapo Ridge Partial-hospitalization Program This voluntary, short-term, partial-hospitalization program serves individuals 18 years and older who need comprehensive treatment, but who do not require hospitalization. The program is designed to help clients decrease symptoms of mental-health conditions and develop coping skills needed 10

for community living. It may also help prevent the need for inpatient treatment or serve as a step in the continuum of care following inpatient treatment. Personalized treatment plans include individual and group psychotherapy, medication evaluation, and education in a scheduled full- or half-day program. Ramapo Ridge Psychiatric Hospital Ramapo Ridge Psychiatric Hospital (RRPH) is a 58-bed psychiatric hospital licensed by the State of New Jersey and accredited by The Joint Commission. RRPH consists of two divisions: a general adult psychiatric program and a geriatric psychiatric program which specializes in the diagnosis and treatment of illnesses including, but not limited to, depression and Alzheimer s disease. Both programs are designed for adults who require inpatient care. Attending psychiatrists are available, as well as physicians in other specialties who can treat non-acute medical problems. Patients in both programs benefit from a full range of treatment services and an interdisciplinary treatment team that develops individualized treatment plans to help patients reach their goals. Ramapo Ridge Psychiatric Hospital is licensed to accept both voluntary and involuntary patients. 11
