4 Bob & Ginny Bob and Ginny love their active lifestyle that keeps them on the water, in the garden, and involved in the community. They didn t expect to be facing injury and surgeries until they were much older. After a combined ten inpatient and outpatient stays at Broad Reach, they now say they ll be dancing well into their 80 s. We feel so strong and that is why we continue to come back to Broad Reach. 5 Rehabilitation Inpatient and Outpatient Care The services at both Liberty Commons and our outpatient clinic are designed for individuals who have an acute illness, injury, or are recuperating from surgery. Our singular focus is to enable patients to return home and to resume their normal daily routines. Inpatient Rehabilitation: We understand that a positive experience and comfortable accommodations are an integral part of the recuperation process. In addition to luxury suites and an array of special amenities, our cheerful staff is focused on providing a relaxing waypoint on your path to recovery. Our clinical team of physicians, nurse practitioners, nurses, physical, occupational and speech therapists, dietician, and social workers is widely recognized for its comprehensive approach to recovery in which a customized treatment plan is developed to ensure the highest level of success for return to independence. Outpatient Rehabilitation: Broad Reach Outpatient Rehab Clinic enhances the continuity between inpatient and outpatient care and offers services ranging from post-operative and post-injury therapies to health and wellness and lymphedema programs with a specialization in sports therapy. Patients of all ages are treated by our team of highly-trained and deeply-committed physical, occupational, and speech therapists. We emphasize one-on-one therapy and match each new patient to therapists who will manage their rehabilitation from start to finish. 6 Muriel & Elinore Both Muriel and Elinore arrived at The Victorian within two months of each other. It took no time at all for them to realize that they had many common interests and came from similar backgrounds. Before long, they arranged to dine at the same table, were sharing family news and stories, and attending programs and activities together. Friendships like Muriel and Elinore s are only one of the many fostered by the unique community at The Victorian.

7 Assisted Living At The Victorian The Victorian is our residential setting on the Broad Reach Community of Care campus. Our assisted living at The Victorian offers an attractive option for those wishing to maintain the flexibility of an independent lifestyle, yet may need some assistance. With only 40 suites, The Victorian offers gracious living in an intimate setting. Each of our private suites has a comfortable sitting area, kitchenette, and an emergency alarm system. Residents at The Victorian enjoy beautifully appointed living rooms, lounges, and a dining room where the executive chef prepares exceptional gourmet meals. The common rooms are perfect for developing relationships and visiting with loved ones. Wide porches and courtyards overlook manicured grounds and deep blue hydrangeas that bloom all summer. 8 Hilda When life had become difficult and isolating for Hilda at her Arizona home, she and her family made the decision to bring her home to Cape Cod. Coming to stay at Liberty Commons proved to be a great fit for Hilda, her son Raymond, and their loved ones. It has not only brought her closer to her own family, but has also enabled her to foster new relationships with staff members like Emily. Within days of her arrival, Hilda began participating in the many activities offered and her fun and outgoing personality began to shine. Her family has great peace of mind knowing her physical, emotional, and social needs are being met in a wonderfully supportive setting.

9 Long Term Care At Liberty Commons Liberty Commons is the right choice for those who need long-term, skilled nursing care and rehabilitation. Our dedicated professionals create an environment in which residents and their families truly feel at home. We have directed our attention to the details that enhance the quality of life for our residents, working to deconstruct the institutional feel that had become synonymous with long term care. 10 SteLvio When Stelvio s loving wife, Libby, knew he was reaching the later stages of his disease, she turned to Broad Reach Hospice for the support they needed during this difficult time. The hospice team has helped us to reflect on the past and focus on the good times. We couldn t ask for more love and care; it really is like being part of a family. The hospice team s commitment to providing Stelvio and his family with a unique approach to care has brought them great comfort and a sense of peace during this last phase of life.

11 Hospice & Palliative A Time of Care, Understanding, & Comfort Broad Reach Hospice provides compassionate care for individuals and their families facing life-limiting illness. Our program focuses on alleviating the stresses that often times become overwhelming for patients and families during a difficult time. Our experienced team of professionals including physicians, nurses, hospice aides, social workers, pastoral care, and trained volunteers come together to provide loving support and a plan of continued treatment that is tailored to meet the needs of each patient and family. This personalized care allows for meaningful and peaceful days. Palliative Care provides comforting care and consultation to patients and families during any phase of a disease or terminal illness. The Hospice team at Broad Reach works together with the patient, family members, and medical team to develop the best possible plan to care for those in need of its services.
