4 Post-Acute To Home Program Our Post-Acute To Home (PATH ) program acts as a bridge between hospital and home, by helping patients overcome the challenges of recovering from an injury, illness or surgery. Our goal is to help people get back to living their lives on their terms. Under the supervision of a physician and registered nurse, each person in the PATH program receives skilled therapy and skilled nursing services that contribute to their recovery. Our dedicated team of doctors, nurses, therapists and social workers tailor individual programs to meet patients personal and medical needs.
