1 CONTINUING ANN S CHOICE ROSE GARDENS FACT SHEET Rose Gardens at Ann s Choice - RG RG was built on the pillars of independence, security, topnotch health care and affordability for middle income seniors. RG provides a safe and comfortable environment where you or your loved one can experience independence, privacy, and dignity with few disruptions to daily life. ROSE GARDENS: A 132 bed neighborhood offering 22 short term nursing and rehab therapy rooms, 44 personal care home residences, and 66 long term nursing residences. Rose Gardens residents enjoy remarkable advantages other communities don t provide multiple levels of health care, on-campus physicians, excellent staff ratios with all private rooms, an approved 5-STAR MEDICARE home and named Best Nursing Home by US News and World Report, two years in a row. CARE MODEL: Our Person Centered Approach (PCA) model provides residents a high level of care with an impressive staff-to-resident ratio creating a personalized caregiving relationship between residents and RG staff caregivers. Our unique approach to care focuses on restoring and maintaining an optimal level of independence, enjoying a high quality of lifestyle. A summary is created for each resident; including information of the resident likes and dislikes, talents, hobbies and former lifestyle, enabling individual preferences to be known. Our focus is on individual resident needs. Residents have a voice in their care and are at the center of decision making along with the caregiving teams. Residents help drive what happens in each household. This is truly care on a personal level. ADMISSIONS/HEALTH CARE COUNSELOR: A discussion with the Health Care Counselor regarding an RG stay is the first step for coordination of care at RG. The Health Care Counselor is responsible for helping you plan a move to Rose Gardens, including coordinating a transfer of information from a hospital or physician and the completion of documentation that is necessary for your care (or support) plan. The Health Care Counselor is a resource for residents answering your questions and providing the information you need regarding your care options, levels of care and bed availability. The Health Care Counselor will assist you in managing the details of this hard decision and provide support for the resident/family with this sometimes difficult transition. If RG availability is such that your need cannot be currently accommodated, we work with you through the process of selecting interim alternative services. Communication regarding bed availability and a discussion from facility to facility regarding interim care placement, as opposed to a permanent stay, allows for a mutual understanding and a smooth coordination of return to an RG care bed when available.
2 HOUSEHOLDS/LEVEL OF CARE: CASTLEBRIDGE a 22 bed rehabilitation transitional household RG is an in-patient rehabilitation facility, providing nursing care, physical therapy, occupational therapy and speech therapy. All RG nursing beds are Medicare certified meaning you can receive rehabilitation therapies on any nursing household, however Castlebridge cares for the majority of residents coming primarily from a hospital stay needing skilled nursing /rehab to get back to a healthy baseline; - to safely return to their pre-hospitalized or recommended level of care. Castlebridge is the designated transitional household/temporary insurance covered stay (i.e. Medicare, Erickson Advantage, Managed Health Care). HOMESTEAD, CEDAR HOUSE and VALLEY CREEK are designated long-term nursing care households. Each household offers 22 private resident rooms. Residents in need of round the clock nursing care are private pay residents. The focus of these households is to provide residents with complete assistance with daily activities of living (bathing, dressing, eating, and mobility). TWIN STREAM and PEBBLEBROOK are Personal Care households. Each household offers 22 private resident rooms. Our Personal Care households are designed for individuals who can benefit from some assistance with daily activities of living, while encouraged to use their strengths and abilities maintaining independence with some support. A Personal Care resident enjoys a vibrant lifestyle with extra assistance and more opportunities for socializing with a culture of engagement celebrating life. Residents living in Personal Care are paying privately for their care. We will partner with you to determine which care benefit is best for you. RESPITE CARE: Short-term respite care provides all the daily living assistance, or nursing needs for a pre-determined length of stay. This is generally a vacation stay period of time for those needing some time to get back on their feet, or for caregivers needing a break from the primary caregiving role. DEMENTIA CARE: Our lifestyle and engagement provide a familiar routine tailored to meet the resident s skills and interests. (we do not have a designated dementia care unit). Please speak to the Health Care Counselor further about this level of care for more information. HOSPICE CARE: Hospice services can be provided at RG through the Hospice provider of your choice. Our social services department, clinical care coordinators, or health care counselor can answer any questions you may have regarding this level of care. 2
3 COST: 2015 Daily rates are as follows: HOUSEHOLDS: Personal Care: PEBBLEBROOK AND TWIN STREAM Care Benefits Available: $144 - $315 a day (cost based on individual needs/assessment of resident) HOUSEHOLDS: Nursing Care: HOMESTEAD, CEDAR HOUSE and VALLEY CREEK Nursing daily rate of $ RESPITE RATE: Personal Care Households: $ Nursing Households: $ INSURANCE COVERED STAY: If your stay at RG is for skilled nursing therapy/rehabilitation therapy (CASTLEBRIDGE HOUSEHOLD) your stay is typically covered by insurance. We will need a copy of your insurance cards prior to or upon admission. Medicare Part A pays 100% for the first 20 days of a skilled nursing stay. On Day 21 of a skilled nursing stay, Medicare pays 80%. A copay of $ is either paid privately by you or if you have a secondary insurance we will verify if they will cover the Medicare co-payment per day. We will share the verification information with you and suggest you contact your member benefits representative to verify your current benefits as well. If your bill reflects a co-pay, generally it is because we received a denial from your insurance company, or your secondary insurance policy does not include a skilled nursing benefit. We make every attempt to bill all insurers provided to Rose Gardens upon admission. If your insurance should change, please let us know. Erickson Advantage will pay 100% for the first 20 days of a skilled nursing stay. A co-pay of $75.00 for days is required should your rehabilitation stay go beyond day 20. If your health insurance is other than Medicare or EA, we will verify your benefits and let you know if there are any out-of-pocket co-pays you will incur for your rehab stay here at Rose Gardens. We suggest you contact your member s benefits number on the back of your card to verify your current benefits as well. REHABILITATION RESIDENT RESPONSIBILITY: While at Rose Gardens any previously arranged medical appointments made prior to your admission to Continuing Care which date and time will occur while you are rehabbing with us should be changed. We remind you (or a family member) to notify the appropriate people to reschedule your appointment(s) for after you are discharged from Rose Gardens. Any medically necessary appointments ordered while you are rehabbing at Rose Gardens will be discussed with you (or your designee) so you can coordinate your appointment time with the specialist and your escort to this appointment. 3
4 BILLING: If you or your spouse should require a stay in RG, any charges incurred will be included on your Ann s Choice bill. When processing Long Term Care Insurance claims, we recommend splitting Ann s Choice bills for each spouse in order to make the RG billing clear and concise for your LTC company. We also recommend you request to have your bills separated when one spouse moves into RG, this will be to your advantage when the time comes to prepare your annual taxes. A copy of your monthly statement can be sent to your POA, in addition to the statement you receive monthly in your cubby. STAFFING: Director of Extended Care/Administrator licensed by the State of Pennsylvania, the administrator is responsible for all services provided in Rose Gardens. Director of Nursing is responsible for the management and operational planning of all nursing services in Rose Gardens Personal Care Manager Is the administrator of Personal Care and oversees all services in the Personal Care households. Household Nurse the nurse is licensed by the state of Pennsylvania and is responsible for administration of medications, treatments and the clinical care of the residents they are working with on their household. Clinical Care Coordinator -is a licensed registered nurse who manages the overall clinical processes and practices that are needed on the household to manage disease processes and positive clinical outcomes. Household Coordinator -Manages the operations of the household to facilitate the needs of the household so that personal needs are met for the residents. Household Associate Your primary caregiver who is a certified nursing assistant who will provide assistance with activities of daily living. PASTORAL CARE: Our ACH pastoral ministry manager coordinates pastoral care for residents, families and staff. Pastoral Care at RG values the faith tradition of each individual by offering faithbased programs for members of each faith community. You are welcome to request a visit with the Pastoral manager at any time. MEDICATIONS AND PHARMACY SERVICES: Medications will be administered as prescribed by your admitting physician. In compliance with state regulations, residents are only permitted medications, including over the counter drugs, which have been ordered by a physician. Any medications you bring with you to Rose Gardens will be for verification purposes only. Rose Gardens will work with the pharmacy of choice following Pennsylvania regulations for dispensing of medications. 4
5 SOCIAL WORK: A social work staff member will meet with you and your family on a regular basis to discuss your needs as well as establish links with resources when the need exists. In addition, they assist with discharge planning and provide support and guidance with Advance Directives and end of life decision making. PRIVATE DUTY COMPANION/HOME HEALTH AIDE: At times there may be discussion and a decision made to offer supplemental support to residents living at RG. Families/Residents who do this can utilize the services of the Ann s Choice home support department. For any other private duty aide/companion used that is not part of the ACH Home Support department, the Social Work department can assist the resident/family member by providing information that explains all requirements the family/resident is obligated to complete prior to having any services provided to residents. MEALS/DINING SERVICE: At Rose Gardens we feel variety and choice are key concepts when it comes to dining. We offer our residents and family members the opportunity to join the weekly Menu Planning Meeting. At this meeting our residents plan the entire menu for Rose Gardens. This allows our residents to continue enjoying the foods they have always loved. DIETITIAN: The dietitian follows your care and provides nutritional support to residents via group menu planning, individualized counseling as well as collaboratively with the interdisciplinary team. PROGRAMMING: Rose Gardens programs are based upon resident interest and the ideas of residents and staff of each household. There is a varied array of programs to participate in and a staff that is willing to help you. VISITING ROSE GARDENS: As we welcome your friends and family to visit often and spend time with you, we do ask everyone to be mindful of the hour and the care needs of our residents. We respectfully request you to be considerate of other residents living on your household(s) by not allowing visitors to interfere with the comfort, rest, and care of you and others. If you are rehabbing with us (Castlebridge household) visiting hours should not interfere with your rehab /nursing therapies and visitors are not permitted in the rehab gym to allow complete focus on individual therapy goals. We also ask that you be considerate of the importance of rest a rehab resident needs for individual optimal improvement. The Rose Garden s concierge desk is staffed from 7:00 AM to 9:30 PM daily to welcome and assist guests as they come to visit. The Rose Garden s front doors are locked at 9:30PM daily and it is our recommendation you visit during these hours.
