4 Rehabilitation We understand the unique physiological needs of seniors and offer encouragement and personalized rehabilitation services to ensure clients goals are met. Our Geriatric Physical Therapy Program provides innovative therapy strategies with dedicated therapists, and our Clinical Residency Program encourages innovation and application of the latest therapy practices. Mom s therapist was great, really encouraging her to a level of independence we did not think was possible. She looked beautiful dancing at her great grandson s wedding. Family Member Offered in Multiple Settings A continuous relationship with our therapy staff is possible as rehabilitation services are offered in three different settings to meet individual needs: Short Term Rehabilitation post-hospitalization at our Goldstein Rehabilitation Center Outpatient Therapy at One Post Road, Fairfield In-Home Therapy provided by our Friedman Home Care Agency Three Types of Rehabilitation We provide an interdisciplinary and integrated approach to a client s recovery, with training in neurologic, cardiac, orthopedic, and post-surgical rehabilitation. We offer: Physical Therapy strengthening and flexibility programs, gait training and assistive device use, and fall prevention Occupational Therapy fine and gross motor coordination training and activities of daily living Speech Therapy swallowing disorders, cognitive communication disorders, and speech language disorders 4 JEWISH SENIOR SERVICES JEWISH SENIOR SERVICES 5
5 Rehabilitation Short Term Rehabilitation at the Goldstein Rehabilitation Center Recovering from an injury or illness requires clinical expertise and personalized attention. At the Goldstein Rehabilitation Center, our physicians, nurses, and therapists work with clients throughout their stay to support steady progress and a return to optimal functioning. Individually designed discharge plans ensure continuity of care after returning home. Amenities Café and Gift Shop Recreational activities, computer access, and library Outpatient Therapy We offer a personalized therapy plan, based on physician s orders, that integrates physical, occupational, and speech therapy services as needed. Our tailored programs include evaluations, therapy, and exercise. We also offer specialized senior exercise classes and gym equipment to support ongoing wellness. 6 Home Care Friedman Home Care Agency Personalized Care in Your Home Our Friedman Home Care helps you at home whether a private home, apartment, assisted living, or independent living community after a hospital stay, or while recovering from an illness. Our team provides one-on-one care to promote recovery, independence, and overall health. I was so comforted to recover at home with such a talented team of professionals. They gave me the confidence to get back on my feet again. Client Medical Home Care Our experienced nurses arrange for skilled nursing visits; physical, occupational, and speech therapy; home health aides; and other home support services. Coordinating with physicians, patients, and families, our team develops an individualized plan to restore health and independence to people living at home. We provide Medicare-certified and licensed home care professionals. Our staff is always available 24 hours a day, 7 days a week to ensure optimal client safety and service. Non-Medical Home Care (Companions or Live-In) Non-medical home care is also available to support families on either a short term or long term basis. Our expertise includes finding the right fit between clients and their caregivers, giving peace of mind and support when they need it most. 7 Hospice Chaifetz Family Hospice The hospice team coordinated all of Dad s care and made him comfortable so we could enjoy the time we had with him. Resident s Son Our team of specially trained nurses, physicians, social workers, home health aides, clergy, volunteers, and bereavement specialists provides comfort care, pain and symptom management, emotional support, and spiritual guidance for patients and families. Chaifetz Family Hospice is a covered Medicare benefit. 8 Day Grasmere by the Sea Adult Day Center Grasmere by the Sea Adult Day Program meets clients medical needs in a social environment, improving the lives of older adults. The program is designed to help families who wish to keep their loved ones at home but need affordable assistance during the day. Our dedicated nursing staff will coordinate care with physicians and help with medication management. We provide a variety of recreational options, hot meals, and personal care services to enrich lives. Our beautiful waterfront facility has bright, spacious rooms and a lovely terrace. Professional nursing services Medication and care management Personal care and bathing Outpatient rehabilitation and exercise programs Therapeutic recreational programs and social and cultural trips I don t know what my Mother and I would do without Grasmere by the Sea. Mom comes home with a smile on her face every day she attends. 9 Advocacy and Education Institute on Aging The Institute on Aging provides guidance and support to older adults, supplying crucial answers and advice on senior care issues. We offer a consultative service for older adults, with coordinated medical, psychological, and social interventions that lead to a comprehensive plan of care. Our experienced team comprised of physicians, care managers, social workers, and consultant specialists strives to help manage and limit disease progression and identify support to optimize each client s quality of life. Center for Elder Abuse Prevention The Center for Elder Abuse Prevention is the first shelter in Connecticut, and the second in the nation, founded specifically to offer victims of elder abuse a safe haven. The Center aims to champion safer communities for older adults through advocacy, awareness, education, and outreach. I lost sleep almost every night worrying about Dad until we hired our Care Manager, who has been my eyes and ears ever since. Client s Son I never thought someone we all trusted could do this to my Mom until it happened. The Center saved her life. 10 Long Term Care Protection Senior Choice at Home The first of its kind in Southern Connecticut, Senior Choice at Home is a comprehensive membership program for independent older adults. It is an option for individuals and couples who wish to remain in their homes, ensure their care is paid for, and maintain their lifestyle as their health care needs evolve. The program offers the benefits of a Continuing Care Retirement Community without requiring a move or the spending of assets. Each member s short and long term care needs will be provided for, preserving the highest possible level of independent living and preferred lifestyle. When services are needed, one call to the program s care manager is all it takes. All care and services are provided by Jewish Senior with no forms or paperwork to file. It s a great opportunity for the rest of our lives; not to have to worry and our children don t have to worry.
