We enter into contract with the State of Oregon every two years to deliver services to seniors and people with disabilities. We were created in We serve seniors and people with disabilities in five counties: Clatsop Marion Polk Tillamook Yamhill There are other names for our agency. We are also known as an: Area Agency on Aging Aging and Disability Resource Connection Most of our funding comes from the federal and state government. We also receive donations from individuals and community groups. 4

4 Help with Your Questions NorthWest Senior and Disability Services is here for you. We have trained workers to answer your questions. They can tell you about and connect you with programs and services that may be able to help you. This help is free. We do not discriminate. People contact us for many reasons. Some are planning ahead for the future. For example, an adult child may call us to ask questions about caring for a parent. Others contact us for current needs, such as for help paying their medical and food costs. There are several ways to get answers to your questions. You may want to visit our website. It has a How Do I? area where you can find answers to commonly asked questions. Our website is: You may also contact us by phone or . 5 Financial and Medical Help You may find that you need help with your financial and medical costs. A NorthWest Senior and Disability Services worker can review your income and resources to see if you may be able to get help through one or more programs. These programs include: Medicaid/Oregon Health Plan These programs help pay for medical services. This includes doctor visits, hospital care, and long-term care. Medicare Savings Programs These programs are for people on Medicare. They help with Medicare costs. This may include the Medicare Part B monthly premium, co-pays, and deductibles. Help with Food Costs The Supplemental Nutrition Assistance Program, also sometimes called Food Stamps, helps with food costs. A set amount of money is placed on to a debit card (Oregon Trail Card) each month. In-Home Help You may find that you want to remain in your home, yet you are having trouble with tasks of daily living. A NorthWest Senior and Disability Services worker can talk with you about getting help with the following: housekeeping laundry meals shopping medication management bathing You may also be able to get other help. It depends on your needs. These tasks may be paid for by Oregon Project Independence (OPI) or Medicaid. Employed Persons with Disabilities This program provides or continues medical coverage to people with disabilities who are working or want to work. 9 10

6 Senior Meal Sites People age 60 or older may enjoy freshly made lunches at one of our meal sites. These lunches are well-balanced. Diet options are offered. We ask that you please call ahead to your local meal site. You may contact any NorthWest Senior and Disability Services office to find out how to reach your local meal site. Lunches are free, yet people are welcome to make a donation. Home-Delivered Meals If you are age 60 or older and are unable to make your own meals or travel to a meal site, we may be able to bring meals to your home. Meals are hot or frozen. They are delivered by a friendly volunteer. Health and Wellness Programs If you are looking for a way to keep healthy, improve your health, or manage a health condition, we invite you to find about the free, evidence-based health and wellness programs we sponsor in our five county service area. For example, Tai Chi Moving for Better Balance, National Diabetes Prevention Program, and Stanford s Chronic Disease Self-Management Program. To learn what programs and services are currently being offered, please contact us. Options Counseling Are you unsure about what local resources and services might be right for you? NorthWest Senior and Disability Services can help. We have workers, called Options Counselors, who can discuss with you your needs and share options. This help is free. Care in the Community You may find that you want or need to live in a place other than your home. It can be helpful to know about community care options. They are: Adult Foster Homes Adult foster homes are private homes with family-style living. They offer room, board, and physical care for up to five people. NorthWest Senior and Disability Services licenses, inspects, and monitors these homes in Clatsop, Marion, Polk, Tillamook, and Yamhill counties. Assisted Living Facilities Assisted living facilities have private apartments with a bathroom and small kitchen. They offer dining room meals, housekeeping, physical care, and call systems for 24-hour care. They are licensed and regulated by the Department of Human Services, Aging and People with Disabilities

7 Residential Care Facilities Residential care facilities are group settings that offer room, board, supervision, and physical care for six or more people. They offer either a shared or private room. They do not have to offer private bathrooms or kitchens. They are licensed and regulated by the Department of Human Services, Aging and People with Disabilities. Nursing Facilities Nursing facilities provide nursing care 24 hours a day in a medical setting. They offer services such as on-site physical rehabilitation, recovery after being in the hospital for a serious illness or injury, and restorative services. In Oregon, all people must be screened before entering a nursing facility. This is called a pre-admission screening. The screening is free and helps to make sure a nursing facility is the right fit for your care needs. Nursing facilities are licensed and regulated by the Department of Human Services, Aging and People with Disabilities. Public Files NorthWest Senior and Disability Services has files for all community care options in Clatsop, Marion, Polk, Tillamook, and Yamhill counties. In them, you will find licensing and monitoring information. You will also find any reports of abuse. These files are helpful when deciding on a place to live. Please contact any NorthWest Senior and Disability Services office to set up a meeting to view these public files. Reporting Abuse The truth is that there are cases of abuse. It could be physical, sexual, or mental abuse. It could also be neglect, self-neglect, or financial abuse. Anyone can report abuse. It can be reported for someone who lives at home or in the community. It can be reported for yourself, or you can report it for someone else. You do not have to give your name when making a report. 8 Medicare Counseling Medicare insurance can be hard to understand. The Senior Health Insurance Benefits Assistance (SHIBA) program is a team of trained Medicare counselors. They offer free, one-on-one Medicare counseling to anyone with Medicare. They can help you: understand how Medicare works compare Medicare Prescription Drug Coverage and other insurance options file a Medicare appeal keep safe from Medicare fraud and abuse Please contact a NorthWest Senior and Disability Services office to set up a meeting with a Medicare counselor. Senior Peer Mentoring For people age 60 or older, the Senior Peer Mentoring program can help with life changes and concerns. Trained Senior Peer Mentors offer private, friendly, and helpful mentoring with things such as sadness, stress, chronic health conditions, family problems, the loss of a loved one, and drug and alcohol problems. This help is free. To learn more, please contact us. Family Caregiver Support Program If you are caring for someone age 60 or older, someone with a form of dementia, or your grandchildren, it can sometimes be a stressful job. The Family Caregiver Support Program may be able to help support you. This may include group sessions, individual counseling, access to respite care, and help finding resources. 9 Gatekeeper Program A Gatekeeper is a person, who in the normal course of his or her job, has contact with seniors and people with disabilities. This could be, for example, a grocery store clerk, bank teller, pharmacist, utility reader, or mail carrier. Gatekeepers learn to keep watch for signs that someone may be in need of help. When they notice something concerning, like confusion, neglect, or a poorly kept living space, they contact NorthWest Senior and Disability Services to follow up. Volunteer If you are looking for a way to keep active, please think about volunteering with NorthWest Senior and Disability Services. Our senior meals, Senior Peer Mentoring, and Medicare counseling programs are possible because of many dedicated volunteers. We also have a Senior Advisory Council and a Disability Services Advisory Council. To learn more, please contact us. Emergency Fund NorthWest Senior and Disability Services has an emergency fund. It is for seniors and people with disabilities who have an urgent need and have no other options. It is possible because of community cash donations. Those who are approved for help through the fund are able to get one-time help.
