Summary of Program Cost Monthly cost $4,000 x 6 month program: $24,000 If you are one of the first 25 partners to sign up by Sept 4 th, Nortel & Westcon will pay ½ of this cost for you ($12,000) YOUR total program cost (for 6 month program) $12,000

Your Payment Options You may choose to use your Nortel Marketing Allowance Funds (MAF) funds to participate in this program. You would need to follow the normal process and work thru your Nortel marketing prime to get a prior opened and approved for $12,000 When opening the prior in the description include: Structured Web Healthcare Marketing Program and open under custom marketing activity. Once your prior is approved see the notes and attachments on the approved prior for specific proof of performance (POP requirements) Provide Structured Web with the Prior (PF #) and they will invoice you directly (2 invoices of $6K each) If your funds have already been allocated into a spend plan that does not give you enough monies you can contact your Nortel marketing prime and or your account manger and ask about realigning your funds to cover this. If you currently don t have enough MAF funding, you can choose to pay the remaining balance via credit card or check directly to Structured Web. You may choose to pay the full $12,000 via credit card or check directly to Structured Web
