CHEER EXTREME TRYOUT PACKET Welcome to the 2012-2013 CEA tryouts! Please read all of the information carefully and sign and initial the back page and return to Krista with your registration fee. The coaching staff will evaluate each athlete on the following criteria: -Tumbling difficulty and form -Athleticism -Dance ability and showmanship -Willingness to accept instruction -Stunting positions (flyer, base, back-spot) -Jump technique Please check the two-team box on your information card if you wish to be considered for two teams. Please be aware that considerations will be very selective. Tryout placements are not finalized until August. Athletes who do not keep skills performed at tryouts may be moved to a different team in August. Those that showed marked improvement may be moved as well. The USASF was created in 2002 to unify the all-star cheer industry with a specific set of rules. Divisions are governed by age. Your child s age on August 31 st of this year will determine his/her competition age for the season. Birth certificates are required if we do not have one on file. Teams are formed by both age and level. 2012-2013 Divisions: Tiny Mini Youth Junior Senior Levels 1-4 Senior Level 5 5 and under 8 and under 11 and under 14 and under 18 and under 12 to 18 years old

Leveling System: Cheer Prep: These teams travel locally, compete at the beginner level and do not attend skills camp. Levels 1-2: These teams travel moderately, compete at the intermediate or novice level, go to a skills camp and have moderate competition fees. Levels 3-4: These teams travel somewhat extensively, compete at the excel or intermediate level, go to a skills camp, and have higher competition fees. Level 5: These teams travel extensively, go to a mandatory skills camp in Myrtle Beach*, have a strict attendance policy, have the highest competition fees and compete at an advanced level. If your athlete is on a team that attends Worlds in April, there will be additional expenses depending on what type of bid is awarded. *Time frame for Myrtle Beach skills camp is June 10-30 ALL Level 5 teams and potentially some Non-Level 5 teams will need to attend. ALL SKILLS CAMPS ARE MANDATORY! We MUST have the following in order to tryout: *Registration Fee: $75.00 per child or $100.00 per family *Completed Registration Form with financial information provided *Signed Waiver *Birth Certificate *Completed Tryout Card *All outstanding fees must be paid in full *Cheer Extreme Terms and Conditions Please initial, sign, and date the last page of this packet. You may pre-register at the front desk beginning on Monday, April 9 th with Krista. There will be a $5.00 discount if you pre-register by Thursday, May 3rd.

Tryout Schedule: Please come to the session that your child s age is on August 31, 2012 Thur., May 10 Open Tumble 5:30-6:30pm Ages 11 and under 7:00-8:00pm Ages 12-14 8:30-9:30pm Ages 15 and older New Parent Meetings: 6:30pm/8:00pm/9:30pm Fri., May 11 Learn Dance/Tumble/Jump Stations 5:30-6:45pm Ages 11 and under 6:45-8:15pm Ages 12-14 8:15-9:30pm Ages 15 and older Sat., May 12 Tryout Day 10:00am Ages 15 and older 1:00pm 12-14 4:00pm Ages 11 and under You are free to leave after your child has completed their tryout. Stunt Evaluations: These will be held the week of May 14 th. Please check the website on May 13 th as to what time/day that week that your child should attend. www.cheerextremeraleigh.com Teams will be posted by Saturday, May 19 th Practices will begin the week of May 21 st First month s tuition will be drafted on Monday, May 21 st Saturday, May 19 th - Raleigh Stunt Fest! 1:00pm-6:00pm

CER CHEERLEADING FINANCIAL POLICIES 1-Electronic Payments: All monthly fees paid to CER Cheerleading, Inc. for all-star cheerleading will be done electronically on the 1 st business day of every month through a bank draft/credit card. NO EXCEPTIONS! 2-2012-2013 All-Star Cheer Pricing: The all-star program is offered as a 12-month program with payments due monthly from May 21, 2012 through April 1, 2013. Annual Registration Fee: $75.00 per child $100.00 per family Monthly Tuition: -First Child -Second Child -Third Child $130.00 for Level 1-4 Athletes $140.00 for Level 5 Athletes $120.00 for Level 1-4 Athletes $140.00 for Level 5 Athletes $110.00 for Level 1-4 Athletes $140.00 for Level 5 Athletes Cross-over fee to more than one team: - $30.00 per athlete 3-Competition and Other Fees: All competition fees will be drafted in six (6) installments on Jun 15 th /Jul 15 th /Aug 15 th /Sep 15 th /Oct 15 th/ Nov 15 th. Your child will not compete unless all fees have been paid. Competition fees are Non-Refundable. No Exceptions! These fees will be announced after teams are formed in May.

4-Other Fees: NSF Fees: CER Cheerleading, Inc. transacts business with its bank and online merchant processors and will pass along any NSF fees. The fee will be $20.00 per occurrence. After two (2) NSF occurrences, all fees will have to be paid by cash or certified funds. Bank Processing Fee: $35.00 per athlete Coaches Fees/Choreography/Music Fees: TBD Competition Crossover Fees: TBD and will be due March 2013 5-Termination: Termination from the 2012/2013 all-star program will be accepted with a 30-day written notice by letter or e-mail to Krista before February 1, 2013. No terminations will be accepted after this date. All fees will continue to be drafted/charged until Krista receives such notice. Termination notices given to coaches or team parents will not be accepted. 6-Refunds: There are no refunds of competition fees or registration fees to anyone who leaves the program regardless of the date of leaving the program. Anyone who leaves or is dismissed is responsible for payment of their final month and all outstanding fees. CHEER EXTREME TERMS AND CONDITIONS: Please initial and sign below: I understand that all monthly tuition fees must be drafted. No exceptions. Fees will be drafted on the 1 st business day of every month through April 2013. (May, 2012 tuition will be drafted on Monday, May 21 st ). I understand that all competition fees are NON-REFUNDABLE. These fees will be due in six (6) installments on Jun 15/Jul 16/Aug 15/Sep 17/Oct 15/ Nov 15. These fees may be paid by check or money order to Cheer Extreme as long as they are received by Krista by 7:00 p.m. on the above dates. Fees not received by then, will be drafted on the above evenings with a $10.00 convenience feed added to the competition fee. I understand that if my bank draft or credit card information changes, I will notify Krista immediately. I understand that the mandatory skills camp fee will be drafted two (2) weeks prior to the skills camp. I understand that a 30-day written notice by letter or e-mail to Krista before February 1, 2013 is required. No terminations will be accepted after this date. All fees will continue to be drafted until Krista receives this notice. Termination notices given to coaches or team parents will not be accepted. I understand that the coaches have the right to close practices as they deem necessary. I understand that if I have an outstanding balance after 30 days my child will be asked to sit out of practices and a replacement will learn my child s position.
