COACHES MEETINGS NCA will offer multiple meetings to all coaches to wrap up any last minute championship questions you may have on check-in day. PRELIMINARY COMPETITION Preliminary Competition will take place for the following divisions at approximately 6:00pm: Level 1 Tiny, Level 1 Mini, Level 2 Mini and Levels 1-4 Youth. SATURDAY, FEBRUARY 20 KAY BAILEY HUTCHISON CONVENTION CENTER Preliminary Competition for all remaining divisions will take place on February 20th. Divisions that competed in Prelims on the 19th will have their Final Competition on Saturday, February 20th. Doors are tentatively scheduled to open one hour prior to the first performance. NOTE: Division times will not be available until 2½ weeks prior to the championship. SUNDAY, FEBRUARY 21 KAY BAILEY HUTCHISON CONVENTION CENTER Final Competition for all divisions that competed in Preliminary Competition on Saturday, February 20th. TRAVEL ARRANGEMENTS A & I Travel is a team of dedicated travel consultants that will help you arrange your group s travel. A & I Travel will negotiate group pricing on your preferred airline and provide fare comparisons for up to five airlines. The championship registration is separate from your team s travel arrangements and cannot be paid for with one check. Separate payment arrangements must be made with A & I Travel. NCA TEAM PLACEMENT PROGRAM This program was put in place for all traveling teams. If you are staying in a hotel in the DFW area, you are required to utilize one of the blocks booked by Connections Housing. Numerous hotels around the DFW Metroplex have been blocked for your ease and convenience in booking. Please see the following for more information regarding this program. 2 NCA & NDA TEAM PLACEMENT PROGRAM This program was put in place for all traveling teams utilizing hotel rooms while attending the NCA All-Star National Championship. Being recognized as an official citywide event and capturing the room nights derived from this event are crucial for future date selection and venue availability through the Dallas Convention and Visitors Bureau and the Kay Bailey Hutchison Convention Center. Connections Housing is the official, and only, housing company for NCA & NDA Championship housing. Teams that attend the 2016 NCA All-Star National Championship in February are required to book rooms through Connections Housing in order to participate. NCA & NDA has authorized no other company to book housing for groups attending 2016 Nationals. Numerous hotels around Dallas/Fort Worth have been blocked for your ease and convenience in booking. Within two weeks of receipt of your completed form(s) and payment(s), you will receive an invoice and general information regarding the championship. A deposit of $30 per person is due by October 15, Payments not received at this time will result in the Team not being added to this event. Note: A $30 returned check fee will be charged for each insufficient funds check. Any payments received after January 18th MUST be in the form of a cashier s check, money order, or credit card. 4 INFORMATION YOU NEED KNOW CREDIT CARD PAYMENT OPTION Visa, MasterCard, Discover and American Express may be used for full payment. Only the amount specified on the registration form will be charged to your credit card. Final Payment ONLY will be charged to your credit card per written request. For your convenience, a credit card authorization form has been included in this packet. Note: If you want to add or change a credit card number, you must send the information in writing or fax to NCA. No credit cards will be taken over the phone. Individual credit card numbers will not be accepted for team members. LATE REGISTRATIONS If space is available, late registrations will be accepted after the designated deadlines by sending in the FULL BALANCE due with your registration. If you are registering late, the full payment must be submitted in the form of an official school check, cashier s check, money order or credit card. The standard cancellation policy will apply to all late registrations. CANCELLATIONS AND REFUNDS Notification of cancellations and request for refund must be made in writing, and ed or faxed to NCA. No cancellations will be accepted by phone. Cancellations may be faxed to NCA at NCA is not responsible for faxes not received. All cancellations must be received during business hours: Monday-Friday 8:30 am to 5:00 pm CST. If cancellation is not received during these hours, it will be dated the next business day. No division changes will be accepted after January 29th. NOTE: Due to the complexity of creating and managing the event schedule, NCA will not accept any division changes after January 29th, This effort will help ensure that all gyms know well in advance their performance times, while also limiting any major time shifts. Age Eligibility Requirements - The age of the competitor as of August 31, 2015 will be the age used for competition purposes throughout the season for all divisions. NCA always attempts to ensure teams from the same program do not have overlapping performances during the preliminary schedule. However, NCA cannot guarantee that a performance and practice overlap will not occur during the final schedule. NCA ALL-STAR NATIONAL CROSSOVER RESTRICTIONS 1. Participant may only cross over into a division within the same USASF Level. 2. Participant may only represent up to a maximum of two teams. 3. Participant may only represent one gym. 4. A maximum of five crossover participants is allowed per team. 5. Participants must pay commuter fee minus $15 at date of registration to perform on the second team.

12 ATHLETE ID FORM Dear All-Star Gym Owner, All cheer and dance participants must be registered in the USASF Athlete ID program. Registration is required prior to your first competition of the championship season. If you haven t already registered, please see the quick process below. ATHLETE ID PROCESS STEP 1 Prior to attending your first event in the season, register all cheer and dance athletes by entering information in member profiles at USASF.net. Athlete birth certificates will be uploaded to USASF secure servers for birthdate verification by USASF staff. Once birthdates are verified, birth certificate files are digitally destroyed and not stored on any server. STEP 2 Create and print USASF Official Event Rosters for each team prior to each event and submit at check-in. Please attach the Roster to this form.
