Life Saving Rules. From July 1, 2009 Shell's Life-Saving Rules have been fully enforced. Shell s Life-Saving Rules set out clear and simple do s and don ts covering activities with the highest potential safety risk. They are not new. Most people comply with them every day but there are still breaches taking place. The Life-Saving Rules help to make sure that rules are followed and people are protected. Compliance with the Life Saving Rules is mandatory, and the Life Saving Rules are applicable to all operations under Shell s operational and / or governance control. Each reported non-compliance will be investigated. And failure to comply will result in disciplinary action, up to and including termination of employment for Shell employees, or for employees of contractors or sub-contractors, removal from the site and disqualification from future Shell work. Supervisors are held accountable to communicate and ensure compliance.

If you choose to break the rules, you choose not to work for Shell. The 12 Life-Saving Rules are: Work with a valid work permit when required Conduct gas tests when required Verify isolation before work begins and use the specified life protecting equipment Obtain authorisation before entering a confined space Obtain authorisation before overriding or disabling safety critical equipment Protect yourself against a fall when working at height Do not walk under a suspended load Do not smoke outside designated smoking areas No alcohol or drugs while working or driving While driving, do not use your phone and do not exceed speed limits Wear your seat belt Follow prescribed Journey Management Plan 6. Gantry Top Loading Instruction 1. Driver will park his vehicle in an orderly manner, outside the depot s gate. 2. He will proceed to the main guardhouse and surrenders his ID card (IC or SIT card) in exchange for a gantry security pass. 3. Drivers must wear proper PPE before loading the product. 1. Long Sleeve Shirt with reflective strips 2. Safety Goggles 3. Safety Shoes 4. Safety Harness 5. Hand Gloves 6. Safety Helmet with Chin Strap 4. Driver must comply with Shell life saving rule ( LSR ) 5. He will then report to the despatch office with the following Documents: Carrier advice / Notification, Haulier/Driver HSE Checklist duly filled, signed and stamped.

6. Once the order is confirmed, the driver will be given the i-button (Personnel and equipment), seals and instructed to which bay to load. Driver will counter sign on the loading instruction to ensure product & quantity are accurate 7. Driver will also fill in his name, IC. Vehicle no., date and time into the register book at the security. 8. Driver will drive his tanker through the gate and tap at the card reader (gantry security pass) to open the entrance barrier, located beside the despatch office window. 9. A few meters after the barrier, the driver will stop his vehicle for an inspection. 10. The following items will be checked by the security officer: a. Tanker must be visually well maintained. b. 2 x 9kg. & 1 x 2kg. fire extinguishers. They must be fully charged and in good working condition. c. All light covers must not be broken. d. Appropriate hazchem signs must be displayed from Left to Right and Back. Except for Caradol and Dot 3 (Brake Fluid). e. Tyres must have treads depth of not less than 2mm. f. All discharge valves must be fully closed and dust cover must be in place. 11. Upon successfully passing the inspection, the Hauler trailer will be allowed to proceed to the bay he is assigned. 12. Drivers must align his truck along yellow line in which bay he is assigned. 13. Park trailer truck properly, apply trailer brake, switch off engine / master switch and put wheel chock. 14. Driver will use the lanyard to connect the safety harness and walk across. Open the top hatch cover. To ensure Isotank are free from contaminants, Driver will instruct the loading assistant to what chemical product he is taking. L.A will clip earthing to the tanker. 15. Loading assistant will slowly insert the correct loading arm into the compartment and ensure arm is fully extended to the manhole. rope arm secured properly. 16. Loading assistant will hook up the portable high-level sensor / Overfill protection device in the compartment. 17. Loading assistant will advise the driver that loading arm are secured and can proceed with loading. 18. Driver will activate the loading. Touch i-button at the RDI (Red /Personnel i-button) 19. Touch i-button at the RDI (yellow / Tanker i-button) 20. Key in trip number

21. Select loading arm and Compartment number 22. Press start button 23. Watch out for the Deadman switch when the light comes out need to depress until light goes off in order the loading will continue / failure to clear the light the loading will stop. 24. When loading is completed, driver will inform the loading assistant L.A. will remove safely the arm after ensuring that it has been drained dry, with the drip bucket in placed. 25. Before closing the hatch cover with seal, driver must ensure that product loaded is accordingly to the assigned quantity. Any discrepancy must be reported to the despatch office. 26. Driver will close the hatch cover with Rubber / Wood mallet and put product seals. 27. Loading assistant will return the vehicle earthed ground / bonding cable in place. 28. Loading assistant will return the overfill sensor in place. 29. Loading assistant should ensure that drawbridge is secured in its original position and check that it is locked, look for the drawbridge lights. 30. Green light shows that drawbridge is secured. 31. Red light shows that drawbridge is engaged. 32. Driver must perform final check the truck from front to back in Clockwise Direction from any obstructions such as drawbridge, earthed ground, loading arm and wheel chocks have been removed and returned to their proper positions before starting his vehicle. 33. Start engine, when air pressure achieved, release trailer brake and leave the bay. 34. Driver will now move his vehicle out of the bay and park at the designated holding area before proceeding to the despatch office. 35. At the despatch office, he will return the 2 i-buttons before collecting the documents. Next, he will sign and return the 4th copy of the delivery ticket. 36. Security officer will retain the Carrier copy and check that the seals are correct and secured. Driver will drive off to depot and tap his gantry security pass to open the exit barrier. 37. The driver will park his tanker safely outside the depot and return his visitor s pass to the main guardhouse.
